Prostaglandin E1 and prostaglandin F2 alpha in exudate in nickel allergy.

Ten nickel-allergic patients and 5 healthy control subjects participated in a study of the kinetics of the flux and concentration of migrated leukocytes and extracellular PGE1 and PGF2 alpha during a 48 h period, using a skin chamber technique. The patients were provided with two skin chambers, one with and one without nickel challenge. A higher flux of leukocytes, PGE1 and PGF2 alpha was observed during the second day of allergen exposure, while the concentrations probably due to dilution were unchanged or diminished, indicating an unspecific role of the prostaglandins during the contact allergic reaction. No correlations were found within the groups between the migration of leukocytes and the prostaglandin content.

Familial Netherton's disease.

A familial occurrence of Netherton's syndrome with typical features in two sisters is documented. Detailed illustrations of the associated hair shaft abnormalities and results of extensive immunologic investigations are presented.

Acute febrile neutrophilic dermatosis and abnormal bone marrow chromosomes as a marker for preleukemia.

Acute febrile neutrophilic dermatosis or Sweet's syndrome is a rare disease, which occasionally is seen in patients with myeloid leukemia. We present a case of Sweet's syndrome in a patient with an abnormal chromosome pattern in bone marrow aspirate. Initially the patient had flu-like symptoms with high fever. Two weeks later raised, erythematous and painful plaques appeared on the skin. Various antibiotics were ineffective, but the symptoms vanished after administration of prednisone. Six months later a fulminant acute myeloid leukemia developed, the course of which was complicated by a fatal subdural bleeding. It is concluded that Sweet's syndrome may be a cutaneous sign of a neoplastic myeloid proliferation and that a complete hematological examination including chromosome analysis is mandatory in these patients.

Contact dermatitis to methyl methacrylate.

2 cases of contact dermatitis to methyl methacrylate monomer are presented. The patients are nurses who mixed bone cement at orthopedic operations. During the procedure, they used 2 pairs of gloves (latex). Butyl rubber gloves are recommended for methyl methacrylate monomer to avoid sensitization and/or cumulative irritant contact dermatitis on the hands.

Ultrasonic B-scanning of the human skin. An introduction of a new ultrasonic skin-scanner.

A high frequency, high resolution dynamic ultrasonic skin-scanner is described. The skin-scanner is shown to give a cross-sectional image of the skin and provides an accurate, simple and non-invasive method for measuring full-thickness human skin. In addition to the skin thickness it is demonstrated that the underlying subcutaneous fat and muscles can also be non-invasively explored with the possibility of identifying a variety of skin and underlying tissue lesions.

Prostaglandin E1 in normal human skin: methodological evaluation, topographical distribution and data related to sex and age.

A methodological evaluation of a radioimmunoassay technique for PGE1 measurement was applied to normal human skin. The detection limit of the assay was 15 pg and recovery (mean +/- SD) was 90 +/- 6%. The mean value of PGE1 (+/- SEM) in nine punch biopsy specimens (4 mm in diameter) from a piece of skin surgically removed from one person was 117 +/- 14 pg/mg dry weight. The individual variation of PGE1 activity in biopsy specimens from the same topographical area in ten persons belonging to the same sex and age group was 33 +/- 4 pg/mg dry weight. The influence has been elucidated of temperature and tissue processing, local anaesthesia, sex, age and topographical distribution on endogenous PGE1 activity.

Treatment of rheumatoid arthritis with prostaglandin E1 precursors cis-linoleic acid and gamma-linolenic acid.

20 patients with active rheumatoid arthritis were treated for 12 weeks with the prostaglandin E1 precursors cis-linoleic acid and gamma-linolenic acid in the form of primrose evening oil (Efamol) and the co-factors zinc, ascorbic acid, niacin, and pyridoxin (Efavit). There was a slight fall in skin reactivity to UV light during the treatment, but no effect on plasma or urine concentrations of PGE1, cAMP or cGMP. There was no effect of the treatment on ESR, P-fibrinogen, number of tender joints, number of swollen joints, the duration of morning stiffness, or on the patient's estimation of pain.

[Treatment of condyloma acuminatum with 0.5% 5-fluorouracil-solution, A double-blind clinical trial].

30 males and 29 females with condyloma acuminatum took part in a double-blind randomized trial with 0,5% 5-fluorouracil (5-FU) salicylic acid containing varnish for warts versus the varnish without 5-FU. The females were treated twice weekly at the clinic, the males were treated once daily at home by themselves. The 5-FU solution was found significantly superior to placebo (P less than 0,01). 60% of the 5-FU treated patients were healed with a median healing time of two weeks in the males and three weeks in the females. In the placebo group 27,6% healed with a median healing time of 2,5 weeks (males) an 3,5 weeks (females). Blood tests obtained before and after the trial were normal. Patch tests with 0,5% 5-FU revealed no sensitization after the therapy.

Heat-separation of normal human skin for epidermal and dermal prostaglandin analysis.

Heat-separation was introduced as a simple, reliable method of obtaining pure epidermis and dermis for prostaglandin (PG) analysis. Heating of normal human skin at 60 degrees C for 1 min resulted in a distinct separation of the epidermis from the dermis. After heat-separation the mean concentration +/- SEM of PGE1 activity in normal epidermal tissue was 2906 +/- 281 pg/mg dry weight. The PGE1 level in the corresponding dermal samples was 30 +/- 4 pg/mg dry weight and the mean leakage of PGE1 from the tissue into the buffer used during heating was 426 +/- 54 pg/ml.

Ethambutol-induced lichenoid eruption.

The tuberculostatic drug ethambutol is an infrequent skin offender. A lichenoid skin eruption restricted to the light-exposed areas in a 67-year-old male was proved by withdrawal and challenge to be caused by ethambutol. Patch tests with ethambutol 0.1%, 0.5% and 1% in watery solution were negative.

Prostaglandin E1 in suction-separated human epidermal tissue in primary irritant dermatitis.

Experimental dermo-epidermal separation by suction was performed on primary irritant dermatitic and normal symmetrical skin areas. Primary irritant dermatitis was induced by 24-hour patch tests with an aqueous 10% benzalconium chloride solution in 12 healthy volunteers. PGE1 activity in the epidermis, dermis and blister exudate was measured by a radioimmuno assay technique. The mean values in the epidermis (blister roof) was 1.57 ng/mg dry weight, dermis (blister base) 0.08 ng/mg dry weight and suction exudate (blister base) 0.08 ng/mg dry weight and suction exudate (blister fluid) 1.61 ng/ml. The corresponding control values were: in epidermis 0.86 ng/mg dry weight, dermis 0.04 ng/ml dry weight and blister exudate 0.50 ng/ml. Thus significantly increased PGE1 activity (p less than 0.02) was measured in the suction-separated samples in primary irritant dermatitis and furthermore, the highest level of PGE1 activity was detected in epidermis.
