Nursing informatics: yesterday, today and tomorrow.

Today, at the beginning of the 21st century, nursing informatics has become a part of our professional activities. Informatics has advanced the field of nursing by bridging the gap from nursing as an art to nursing as a science. This article discusses the major milestones that have influenced the growth of computer technology and the specific efforts that have influenced yesterday's creation of the nursing informatics movement. It also provides an overview of nursing informatics in today's health care industry, while highlighting the administrative, management, research and educational applications and initiatives. In addition, it provides an overview of Saba's Home Health Care Classification, a nursing language system that has influenced the field. Finally, it provides a prediction of how computer technology will impact the nursing profession in the 21st century.

Electronic access to military/uniformed services nursing research.

The Graduate School of Nursing of the Uniformed Services University of the Health Sciences, with the help of Cinahl Information Systems, has created a virtual electronic military/uniformed services nursing research (EMUSNR) database. This database is an integral component of the newly created specific interest category for military and uniformed services, which will tag all their documents in the Cumulative Index of Nursing and Allied Health Literature database. The virtual research database will allow easy access to military/uniformed services nursing research documents completed by students at the Graduate School of Nursing and final reports of the TriService Nursing Research Program. The EMUSNR offers electronic searching and retrieval of citations to the nursing research documents. The citations will be fully indexed, consisting of an informative abstract, unique subject headings, cited references, and other research concepts. Individuals both in and out of the military and uniformed services around the world will benefit from this new resource.

Video tele-education systems.

This paper provides an overview of video tele-education distance learning systems that are being used to educate nursing students. These systems are using communication technology to teach courses to nursing students in one or more remote sites. The types of video tele-education systems are described. Also, the educational strategies to be concerned with and the technical requirements to implement a system successfully are presented.

Twenty care components: an educational strategy to teach nursing science.

The HHCC System's twenty care components and nomenclature provide a framework for the development of teaching strategies to prepare nurses for practice in the 21st century. The use of an integrated system of instruments accessible to students on-line allows for a smooth transition from course to course and later to clinical nursing practice. Faculty and/or instructors at Georgetown University School of Nursing use this innovative educational strategy and conceptual approach to facilitate collaboration with other healthcare providers and to model professional behavior for students.

A look at nursing informatics.

This is a companion article to the article on Medical Informatics. It focuses on the new nursing specialty-Nursing Informatics. This article provides an overview, scope, definitions, data standards, goals, and research initiatives designed to advance the status Nursing Informatics. Seven research priorities have been proposed which not only provides the direction for Nursing Informatics research, but also the focus for computer-based nursing information systems.

Nursing informatics in nursing education.

Full integration of nursing informatics education at all levels at academic institutions and in practice is critical to nursing as the profession faces the 21st century. Nursing informatics, which encompasses computer hardware, software, and network systems, supports the profession's goals of achieving computer literacy by the year 2000. Strategies for successful integration of nursing informatics into curricula are identified, and specific tactics that promote desired outcomes suggested. The benefits that nursing informatics brings to education, administration, research, and practice are stated and the role of professional nursing organizations in promoting nursing informatics' status as a recognized nursing specialty is noted, and the specialty's role in promoting the profession's future development described.

Toward a uniform language for nursing in the US: work of the American Nurses Association Steering Committee on databases to support clinical practice.

This paper reports on the work of the American Nurses Association Steering Committee on Databases to Support Clinical Practice, in existence since 1989. Responding to its broad charges, the Steering Committee has laid down the foundations for its work in declaring the nursing process as the framework for nursing data in database systems, and in endorsing the Nursing Minimum Data Set as the set of minimum elements for any system designed to carry health-related data that reflects nursing care. In addition, the Steering Committee has begun initiatives to: 1) promote the inclusion of nursing-related data in large health-related databases, and 2) develop a Uniform Language for nursing through a phased approach. The Steering Committee also works directly with the International Council of Nurses to promote the inclusion of nursing data in internationally used classification systems and to develop an international language that describes nursing care.

A new paradigm for computer-based nursing information systems twenty care components.

This paper describes a new paradigm for computer-based nursing information systems. Twenty Care Components provide a means for coding, classifying, and statistically analyzing nursing taxonomies. They provide the structure for documenting the nursing process using a holistic approach. They can be used to assess, document, cost, evaluate, as well as link the nursing process phases. The components are classes and can be used to compare nursing care across health care settings, nationally and internationally. They make it possible for nurses around the world to communicate on-line and interact with each other.

Home health care classification.

This article is the second in a series on the Home Health Care Classification research study conducted at Georgetown University School of Nursing (see the March 1992 CARING). The purpose of the study was to develop a method to assess and classify home health Medicare patients in order to predict their need for nursing and other home care services, including their outcomes of care. To accomplish this goal, data on actual resource use that could be objectively measured were used to predict resource requirements. A third article will provide more detail on the descriptive findings, statistical analyses, and the Preliminary Classification System.

A new home health classification method.

The Home Health Care Classification Method used with the assessment instrument and scoring methodology can predict resource requirements in terms of three cohorts of time and volume of nursing and all provider visits. This HHCC Method can be used to predict care requirements for planning purposes, including the staffing and provider mix, to determine the scope and type of services, to identify the fiscal needs and constraints, and to fulfill other administrative needs. It also can track the home care process, measure patient outcomes for home care quality assurance, facilitate efficient documentation, and form the basis for predicting costs of home health care. The coding schemes developed for the Classification of Home Health Care Nursing Diagnoses and Interventions can be used not only to code but also statistically to analyze home care nursing data. This classification provides the mechanism to link nursing diagnoses to nursing interventions. It can be used in other research studies and has the potential for the design of computer systems for home health care.

A Nursing Intervention Taxonomy for home health care.

One way to develop nursing theory is to extract it from what is actually being done by nurses in practice. Saba et al. provide this service for home health nursing services. Not only does their taxonomy help define the field, but it also develops a structure for care reimbursement.
