The purpose of the current study was to examine the rates of family and child dysfunction in a prospectively, consecutively admitted sample of children. One hundred eleven parents of children consecutively admitted for burn treatment were interviewed using a demographic questionnaire during their child's stay. Results of the questionnaire indicated that in 36% of the families, at least one parent reported significant dysfunction (history of substance abuse, involvement with Child Protective Services, incarceration, or psychiatric history). Results also indicated that 29% of the children over the age of 4 years had significant dysfunction (physical or sexual abuse, or both, psychiatric disorder, behavior problems, or learning disability. It was concluded that (1) parents of children with burns had a substantially high rate of dysfunction, (2) psychologic and preventive interventions should be focused on parents as well as children, and (3) burn injuries often should be regarded as a symptom rather than a cause of psychosocial problems in the family.
Haemodialysis patients are subject to many restrictions and face a lifetime attempting to adhere to various regimes. This small study was designed to explore the relationship between fluid gain, compliance and staff reinforcement. This was achieved by measuring dietetic and nursing educational input using interdialytic weight gain as a barometer of patient compliance. This study suggests renal nurses, dietitians and other members of the multi-disciplinary team may have less influence than previously assumed.
This study examines the correspondence between medical records and Medicaid claims to determine whether claims are a valid source of data for monitoring quality of asthma care. A total of 460 claims for care encounters were matched to medical records of the encounters. While most of the diagnoses and procedures recorded on the claims were documented in medical records, claims failed to identify 29% of encounters with asthma diagnoses and 45% of nebulization procedures administered during encounters. About 30% of documented asthma prescriptions were not associated with filed claims, and about 30% of filed claims for asthma medication were not documented in medical records.
A multinomial image model is proposed which uses intensity-level information for reconstruction of contiguous image regions. The intensity-level information assumes that image intensities are relatively constant within contiguous regions over the image-pixel array and that intensity levels of these regions are determined either empirically or theoretically by information criteria. These conditions may be valid, for example, for cardiac blood-pool imaging, where the intensity levels (or radionuclide activities) of myocardium, blood-pool, and background regions are distinct and the activities within each region of muscle, blood, or background are relatively uniform. To test the model, a mathematical phantom over a 64 x 64 array was constructed. The phantom had three contiguous regions. Each region had a different intensity level. Measurements from the phantom were simulated using an emission-tomography geometry. Fifty projections were generated over 180 degrees, with 64 equally spaced parallel rays per projection. Projection data were randomized to contain Poisson noise. Image reconstructions were performed using an iterative maximum a posteriori probability procedure. The contiguous regions corresponding to the three intensity levels were automatically segmented. Simultaneously, the edges of the regions were sharpened. Noise in the reconstructed images was significantly suppressed. Convergence of the iterative procedure to the phantom was observed. Compared with maximum likelihood and filtered-backprojection approaches, the results obtained using the maximum a posteriori probability with the intensity-level information demonstrated qualitative and quantitative improvement in localizing the regions of varying intensities.
