[Correction of extensive defects in soft tissues].

It was established on the basis of treatment of 7 patients that in approximately half of the patients with post-burn defects and deformities of tissues, for whom the method of tissue stretching is indicated, the use of two and more tissue-stretching instruments is expedient. This makes it possible to prepare a large skin area, use its uninjured parts, and attain the best results of plastic operations. In transposition of dermal-fat and dermal-fascia grafts the use of certain described technical manipulations is needed, they make rational utilization of plastic material easier.

[Surgical treatment of post-burn deformities of the neck].

Surgical treatment of 205 patients is analysed. It is shown that the traditional methods of correcting post-burn deformities fail to produce good results, either functional or cosmetic. The authors correct post-burn deformities and contractures of the neck by plasty with a graft of uninjured skin taken from the anterior surface of the chest. This method provides the possibility for improving considerably both the functional (in 80.3%) and the cosmetic (in 74.2% of cases) results of restorative-reconstructive operations. It is concluded that this method for correction of post-burn contractures of the neck is the operation of choice.

[Treatment of postburn cicatricial deformations and contractures of the neck].

Postburn cicatrices and contractures of the neck have been corrected in 130 patients by the routine methods (Z plasty, free transplantation of the skin, combined methods). Analysis of the late results has shown that contractures have been eliminated or reduced in 70% of the patients, but a good cosmetic effect has been achieved in only 30%. In 75 patients plasty of the postburn cicatricial deformations of the neck has been made with the use of dermo-fascial flaps of the intact skin of the thoracic wall. This technique developed by the authors has completely removed the contracture, and the shape of the neck has remained unchanged: a full-value natural skin integument has been created on the anterior and lateral surface of the neck. Complications (marginal necrosis of the transplanted flaps), recorded in 10% of the patients, have not much influenced the out-come of surgery.

[Use of surgical flaps for plastic surgery in posterior adduction cicatricial contracture of the thumb].

Methods of flap and combined plastics in dorsal adduction cicatricial contracture of the thumb are discussed. Different variants of plastics are substantiated on the basis of study of the anatomical features of the contracture. The formation of a commissure by a trapeziform flap of skin and fatty tissue of the palmar layer and palm is the foundation of the operation. Depending on the degree of deficiency of the cicatricial layer of the fold in the first interdigital space, the wounds to the sides of the main flap are closed with local tissues or by free dermatoplasty. The operative methods were approved in more than 200 patients and good functional and cosmetic results were produced in all of them.

[Variants of plastic surgery using the flaps in the area of the Achilles tendon].

Operations were performed on 10 patients with persistent ulcers of the scars in the zone of Achilles tendon. Plasty with L-shaped whole thickness flap raised from the area of the external surface of the foot, ankle and lower third of the shin with a proximal pedicle was fulfilled in 9 patients after a radical dissection of ulcerated scars. In one patient plasty was fulfilled by translocation of a two-pedicle flap. Different variants of operations and results of treatment are described. A conclusion is made that scarry-trophic ulcers in the zone of Achilles tendon can be successfully treated by plasty with a whole thickness flap raised in the area of the lower third of the shin, external ankle and the lateral surface of the foot.

[Methods for the surgical treatment of postburn contractures of the shoulder joint].

Based on their experience with the treatment of 93 patients with postburn adduction contracture of the shoulder the authors propose a classification related with localization of scars. Marginal contractures are successfully treated by plasty with a trapezoid skin-fat flap from the axillary cavity with/without a combination with the displacement of scarry tissues or free skin plasty; the medial contracture--by one or several pairs of trapezoid skin-fat flaps cut from leaves on the sides of the scar fold; the total contracture with the absence of intact skin in adjacent fields can be adequately eliminated by mobilization a quadrangular island in projection of the dropped axillary cavity to the normal level of the cupola, fixation of narrow sides to the wound margins on the anterior and posterior surface of the humeral articulation and closure of wounds on the shoulder and chest with the splitted skin; the anterior-posterior contracture is eliminated either in the same way as the marginal one, or by cutting a quadrangular flap in the center of the axillary cavity as in the total contracture.

[Resection and plastic surgery of the arteries in malignant tumors of soft tissues of the lower limbs].

An experience of the vascular surgery department with reconstructive interventions on large arteries of patients with malignant tumors of the lower extremities soft tissues, located in the zone of large vessels of the inguinal region, femur and popliteal fossa, is reported. According to the author's opinion, it is necessary to take into account the fact that in reconstructive procedures on large arteries in patients, subjected to massive repeated radiotherapy, there is an increased danger of occurrence of arrosion hemorrhage, extensive thrombosis of vascular grafts and arteries changed under the effect of ionizing radiation.

[An electron-autoradiographic study of capillaries in malignant tumors].

Using electron microscopy the ultra structure of endothelial cells in malignant tumors (myogenic sarcomas), removed in 6 patients, was investigated. It was found that endothelial cells of blood capillaries in malignant neoplasms show some peculiar features in the basal layer structure and in formation of a multitude of orifices of various diameter in the capillary wall. Electron radiographic investigations of endothelial cells of a transplantable sarcoma strain in the experiment have evidenced a greater number of ribosomes in the cytoplasm and enhancement of the DNA synthesis in nuclei. These findings are indicative of increased proliferative properties of these cells.
