Immunoreactive insulin (IRI) dynamics in GH-deficient patients following intravenous glycine loading.

In hypopituitary short-statured patients glycine intravenous injection (250 mg/kg of body weight) provokes a mild and inconstant stimulation of IRI release followed by a significant fall down. Though insiginificant (P 0.064), the prevalence of IRI responsive individuals to glycine in the patients group of idiopathic etiology could be of pathophysiological importance.

Mechanisms and significance of QRS macrovoltage in the absence of cardiac hypertrophy.

The electric stimulation of the lateral ventricular walls carried out in experiments on dog heart (open-thorax), as well as the analysis of the clinical, radiological and electrocardiographic data recorded in 462 cases with QRS macrovoltage led to the following conclusions: a) in 22% of the cases (hospital cardiologic examinations) this anomaly cannot be accounted for either by age, blood pressure or cardiac hypertrophy; b) a temporarily perturbed development of the ventricular depolarization, i.e. a "jerky" depolarization, not stagnant enough to produce an intraventricular block, may generate great negativity and positivity myocardial masses responsible for the appearance of large dipoles, namely of the increased QRS voltage; c) the coincident ischemia and macrovoltage of the QRS major wave, as well as the subsequent evolution of incipient CHD in a series of patients point to the hypothesis according to which the regional myocardial ischemia may induce a QRS macrovoltage by means of the above mechanism.

[Epidemiological aspects in the evolution of epidemic cerebrospinal meningitis (ECSM) in the city of Bucharest in the period 1951-1973].

Analysis of the evolution of the main epidemiologic indices showed that epidemic cerebrospinal meningitis evolved during the last two decades in our country according to the general evolution of this disease in most of the developed countries in the temperate zone, however with certain particular elements revealed by the data obtained in the country and in Bucharest. Details are given for the town in Bucharest, during the 1951-1973 period, concerning the general trends of the frequency and gravity indices of the cases, the structure per age groups of the morbidity, the forms of manifestation of the epidemiologic process. Stress is laid on the seasonal character (winter-spring) of the increase in the number of cases and the periodicity of the epidemic waves.

[Characteristics of the course of the last epidemic of epidemic cerebrospinal meningitis (ECSM) in the city of Bucharest].

Active survey of the epidemiologic potential of cerebrospinal meningitis, in Bucharest, starting in 1967, made it possible to carry out a complex study of the last epidemic wave in 1968-1972. Details are given concerning the results of the complex epidemiologic, clinical and laboratory investigations, which revealed the particularities of this epidemic episode, including the frequency and gravity of the cases, territorial distribution, prevalent affection of certain age groups and children's communities, the prevalence of certain clinical forms, antigenic structure of the pathogenic agent and its sensitivity to antibiotics, comparative efficiency of the different methods of laboratory diagnosis, real and apparent contagiousness, etc. The authors discuss the efficiency of present methods applied in the prevention and control of meningococcal infections in general and of epidemic cerebrospinal meningitis in particular.

[Details of the evolution of the influenza epidemic process in the city of Bucharest in 1974].

The present paper reports on the results of a complex epidemiologic survey of the epidemiologic potential of influenza in Bucharest in 1974, conducted on the basis of a complete, unitary methodology including; (a) Dynamic survey of the morbidity and mortality from influenza, with statistical-mathematical processing of the data per age group and total population; (b) Monthly sero-epidemiologic survey of the antiinfluenza immunologic profile of the population, determined in lots of 540 sera (annual total 7020 serum samples), with statistical-mathematical processing of the serograms; (c) Serodynamic determinations of 67 paired serum smaples collected from patients presenting influenza syndromes during ascension of the epidemic morbidity from influenza; (d) Complex epidemiologic surveys in representative influenza foci in children, adolescent and adult communities. Based upon the result obtained the authors discuss the evolutive particularities of the epidemiologic process in Bucharest, particularly during the epidemic ascension of the first trimester of 1974, caused by the intensified circulation of influenza virus type B. The orientative value of certain elements for the epidemiologic prognosis is emphasized, such as: the immunologic profile of the population per age group with regard to the circulating influenza virus strains (autochtonous or imported strains), active control of the incidence of influenza in communities (technical schools etc.) or enterprises with a large number of employees, laboratory etiologic determinations in cases of a clinical diagnosis of influenza in a preepidemic season. The authors' ten years experience in the active survey of the active epidemiologic potential of influenza in the town of Bucharest shows that the methodology applied was efficient both for scientific assessing of the epidemiologic situation and for an orientation in the choice of preventive and control measures.

[The epidemiological characteristics of influenza in a large urban center (B.) of Romania in 1991].

The peculiarities of the 1991 influenza epidemics in a big urban centre (B.) of Romania were evaluated through clinical and epidemiological active survey and laboratory assays. The results revealed the low rate of endemic morbidity and its significant risk during the fourth trimester as compared to the first, the important implication of the infant population in maintaining the endemic morbidity and of the less than one year old children in the seasonal peaks of the first and fourth trimesters. The laboratory assays revealed the simultaneous circulation but with variable intensity of type A(H3N2), A(H1N1) and B influenza viruses, inducing a high humoral specific protection level in the population. So the risk is low of an important epidemics during 1992. The opportunity is discussed of the prophylactic vaccination with type A(H3N2), A(H1N1) and B circulating strains of all high risk groups, especially of the preschool children from collectivities.

[The evolutionary characteristics of an influenza epidemic in a large urban center (B.) of Romania between 1988 and 1990].

The report presents the peculiar characteristics of influenza epidemics in a big urban centre (B.) of Romania, between 1988 and 1990, surveyed by clinical, epidemiological and laboratory methods. Among the peculiarities of the epidemics: the low rate of endemic morbidity, the seasonal and preseasonal peaks, the very high implication of infantile population in influenza A(H3N2), A(H1N1) and B virus circulation, as well as the high level of mass specific humoral protection against these viruses. Elements of epidemiologic prognosis are suggested for 1991. Opportunity of vaccine prophylaxis using WHO recommended virus strains of the three types: A(H3N2), A(H1N1) and B, for risk groups is discussed.

Growth hormone (GH) release by arginin-vasotocin (AVT) given intranasally in sexually immature children.

Eighteen healthy children were given 50 micrograms synthetic AVT intranasally and samples of serum for GH, magnesium and alpha-amino-nitrogen were drawn 30, 60 and 120 minutes later. There was a significantly different mean of GH peak level (13.1 +/- 3.02 microU/ml) which occurred generally at 30 minutes. The number of GH responders (i.e. peak level greater than 15 microU/ml) was of 7/18. The means of chemical determinations elicited little though opposite changes: slight increases in magnesium and slight decrease in alpha-amino-nitrogen. In a separate day, a sleep test of 30 minutes-duration performed in the morning resulted in a significantly greater GH peak level (28.2 +/- 9.7 microU/ml) but the number of GH responders was nearly the same (6/18). The serum magnesium levels following sleep decreased slightly and unsignificantly. The involvement of serotonergic neurons in mediating GH-releasing effects of AVT is briefly discussed.

[The epidemiological aspects of respiratory tuberculosis recorded in the last 12 years in those over 60].

As the elements of geriatric epidemiology, pathology and sociology are more and more studied nowadays, the paper reports on several epidemiologic aspects resulting from the evolution of the tuberculous endemic, at the ages over 60, in the last 12 years (1976-1987) in the Constanţa County. The data presented show, in the 12 years of our study, important decreases of the incidence of the tuberculosis of the respiratory system at ages over 60, to values below 20% in the last years, with characteristics and evolution tendencies, generally superimposable, over the data recorded for the same time interval on a national scale. The paper insists on the epidemiologic impact, on a mass scale, of the patients with tuberculosis over 60 years old, dispersed among the whole population and having the most various possibilities of contact and transmittance of the infection and disease towards the receptive population mainly children and youngsters.
