Effect of flufenamic acid on uterine contractions and plasma levels of 15-keto-13,14-dihydroprostaglandin F2alpha in preterm labor.

Flufenamic acid (FA), an inhibitor of the synthesis and action of prostaglandins, was administered to 18 women with preterm labor during the 28th to 36th week of gestation. In 15 patients delivery postponed, the mean admission/delivery interval being 21.5 days. 2 patients with cervical dilatation of 4 cm delivered within 48 h despite medication. The peripheral plasma levels of 15-keto-13,14-dihydroprostaglandin F2alpha (KH2 PG2alpha) was high on admission (216 +/- i4 pg/ml, mean +/- SEM), declined by 50% within 2 h of instituting treatment and remained near the normal level seen in 13 controlled women after the 24th hour.

Attempted induction of granulosa cell tumor in Balb-C mice by gonadotropin administration.

An attempt of granulosa cell tumor (GCT) induction by prolonged administration of exogenous pregnant mare's serum gonadotropin (PMSG) to young, mature, and middle-aged Balb-C mice resulted mainly in stromal luteal cell proliferation. 48.9% of young mice, 64.5% of mature mice and 65% of middle-aged mice developed luteal cell proliferation. This effect seemed to be dependent on duration of treatment, mainly in young mice. Young mice injected for less than 6 months developed significantly less luteal cell proliferation than those injected for more than 6 months. Only 1 GCT was found. This study seems to support the contention that besides gonadotropin stimulation, other factors may be involved in GCT development. A large number of PMSG-injected mice developed generalized lymphosarcoma, the mechanism of which remains unknown.

Elective cervical suture in preventing premature delivery in multiple pregnancies.

Of 133 pregnancies conceived after ovulation induced by menotropins and chorionic gonadotropins, a group of 20 women with multiple gestations--15 twins, three triplets and two quadruplets-had elective cervical suture during the 12th to 15th weeks of pregnancy. A comparable group 20 women with multiple pregnancies without cervical suture served as the control for comparing the mean duration of pregnancy, fetal survival rate and mean length of hospitalization. Duration of pregnancy was significantly longer in the sutured group (P less than 0.05). The perinatal loss was significantly lower--16 of 47 infants in the sutured patients, as compared with 25 of 47 infants in the control group. The mean hospitalization period was shorter in women who underwent cervical suture (P less than 0.025).

The hypothalamus as a regulator of reproductive function.

In this review of the role of the hypothalamus as a regulator of reproductive function we have presented a short history of the important anatomical and physiological findings, the structure and properties of gonadotrophin releasing hormone and its function and mode of action in the reproductive process. The latest clinical studies have been presented and we have discussed the therapeutic and diagnostic applications of GnRH. Although the progress in the last few years has been relatively enormous, research on the physiological and clinical implications of these discoveries has probably only now reached its peak and it was thus felt that the time was ripe to write this summary.
