[Surgical treatment of complicated inguinal hernias].

An analysis of 685 operations for complex inguinal hernias has been made. 486 patients were examined within the terms from 1 to 14 years, recurrences being noted in 48 patients (9,87%). The authors recommend a method of radical operation for strengthening the posterior wall of the inguinal canal by an inferior graft of aponeurosis. This method is followed by only 4,44% of recurrences.

[Prolonged pharmacologic sleep in the complex treatment of patients with severe forms of alcoholic delirium].

Results of treating 235 patients suffering from grave alcohol delirium complicated with somatic pathology are presented. Use was made of various methods of inducing prolonged drug sleep: by neuroleptics, barbiturates with analgesics sodium gamma-hydroxybutyrate alone or in combination with barbiturates or viadril. The method of using the combination of sodium gamma-hydroxybutyrate with barbiturates or viadril G appeared to be the most effective. It use resulted in a rapid elimination of psychomotor excitation, quick induction of deep sleep, considerable shortening of the psychotic period, rapid normalization of vitally important body functions, and shortening of the duration of the post-delirium asthenization.

[Errors in the differential diagnosis of acute appendicitis in children].

According to the authors' findings among 1027 children operated upon for appendicitis 61 children had diseases not related with inflammation of the appendicular process. Mesenterial lymphadenitis was observed intraoperatively in 34 cases, pneumococcic peritonitis-in 15, invagination of the small and large intestine-in 3, diverticulitis of Meckel's diverticulum-in 3, ovarian apoplexy-in 3, atonia of the urinary bladder-in 2. Diagnostic errors occurred as a result of improper analysis of the anamnesis data, rare incidence of the aforementioned diseases, and also due to the absence of reliable methods of objective laboratory and instrumental investigations of such patients.

[Quantitative evaluation of periodicity of cell movement in the process of aggregation].

A method of quantitative estimation of quasiperiodic cell movements is considered which allows to approximate the process under study by the sum of trigonometric functions. The possibilities of the method are deomonstrated in the analysis of cell and aggregate movement during the development of a sponge Ephydatia fluviatilis from dissociated cells, taken as an example. The spectral composition of the movement of cell aggregates was found to change with time; the high frequencies disappear gradualy and the lower ones appear.

[Preoperative care and anesthesia in surgical treatment of thyrotoxicosis].

The authors made an analysis of clinical materials pertaining to 1234 patients with thyrotoxicosis, who were given different preoperative management and operated upon under local and general anesthesia. It is noted, that there is less number of postoperative thyrotoxic crises after mercosolyl administration in combination with reserpine and nonspecific drugs than after Plummer preoperative preparation (0.1% versus 1.2%).

[Complications of acute appendicitis in children].

Among 976 children with acute appendicitis 966 were operated upon, 169 of them (17.4%)-for common appendicitis, 401 (41.2%)-for phlegmonous appendicitis, 396 (40.4%)-for gangrenous appendicitis; 10 children were subjected to surgery due to the presence of appendicular infiltration. Preoperatively, in 137 (13.7%) patients peritonitis was observed: local-in 90, diffuse and progressive-in 47. There was noted a dependence of complications on the terms of the disease and patients' stay at the hospital prior to surgery. Following the operation in 98 (10.1%) patients different complications were observed: the postoperative wound suppuration (61), inflammatory processes in the abdomen (infiltrations, abscesses-in 33), intestinal obstruction (2), intra-abdominal hemorrhage (1), enteric fistula (1). There were no lethal issues. The preoperative complications were conditioned by gravity and advanced forms of the principal lesion, technical drawbacks in operation and treatment of purulent peritonitis.
