[Treatment for squamous cell carcinoma of the trachea].

Treatment of squamous-cell tracheal tumors was given to 85 patients. Surgery was carried out in 48 (56.5%), radical resection--22 (25.9%). Distant metastases detected preoperatively in 19 (22.4%) patients were an absolute contraindication for radical surgery. Combined therapy was received by 14 cases without any grave postoperative complications. Three- and five-year survival after combined and surgical treatment was 59.9 +/- 10.5 and 40.9 +/- 10.5%, respectively. Radiation was given to 48 (56.5%) patients. This group included 26 cases of palliative operations and exploratory laparatomy. Three- and five-year survival after radiation was 30.1 +/- 6.7 and 27.6 +/- 6.7%, respectively.

[Radiotherapy for cystadenoma of the trachea].

Radiation as a method of treatment was carried out in 16 (25.0%) out of 64 patients with tracheal adenocystoma (radical dosage of 50-70 Gy (12) and palliative--40-48 Gy (4). The dosimetric profile consisted of the cervical, cervico-thoracic, thoracic and bifurcation sections of the trachea. Complete resorption of tumor was registered in 12, partial--3, no effect in 1 cases. Three-, five- and ten-year survival was 65.2 +/- 12.6; 55.9 +/- 13.5 and 10.4 +/- 13.1%, respectively.

[The role of vacuum and tactile stimuli during milk ejection from the human breast].

Milk ejection in lactating women was performed by breast pump with vacuum and tactile (touch-compression) components. It was found that milk flow rate changed in a pulsatile manner at about 1-2 min interval and peaks milk flow coincided with peaks intramammary. Switch off the tactile component increased the latency of milk ejection 1.5-2 fold and decreased total flow rate 2 fold. It was concluded that touch-compressions stimuli necessary for generation milk ejection reflex and milk ejection from the women mammary gland.

[Transluminal balloon valvuloplasty in the treatment of congenital critical aortic valve stenosis in infants in the 1st year of life].

Balloon valvuloplasty was performed in 14 patients with congenital valvular aortic stenosis. Their age was 28 days to 12 months. All the patients had severe heart failure. Balloon valvuloplasty was successfully conducted in 8 patients. The systolic pressure gradient between the aorta and the left ventricle decreased, on the average, from 81.2 +/- 21.4 to 31.3 +/- 8.2 mm Hg, i.e. by 61.5%. Only 2 patients developed moderate aortic insufficiency after balloon valvuloplasty. The left ventricle could not be catheterised in 6 infants: in 4 patients due to critical valvular stenosis, in 2 due to a. lusoria. One of them died when the guide was attempted to be introduced through the narrowed aortic valve, which resulted in aortic sinus perforation. Long-term results were studied in 4 patients 3 to 24 months after surgery. Their clinical condition improved. The systolic pressure gradient between the left ventricle and the aorta was in agreement with the values obtained just after transluminal balloon valvuloplasty. Transluminal balloon valvuloplasty can be successfully performed in 1-year-old infants with congenital critical aortic valvular stenosis and yields good immediate results. A further accumulation of clinical findings, development of procedures for transluminal balloon valvuloplasty and study of long-term results will allow indications for this tool to be worked out in this group of patients.

[Dynamics of neurological symptoms during immediate and long-term periods after radiotherapy of patients with brain metastases of malignant tumors].

The paper treats of 60 patients with metastases to the brain, subjected to radiotherapy coupled with hormonal treatment. The control group was made up of 17 patients who did not undergo radiotherapy. It has been concluded that radiotherapy noticeably increases (4-fold) the lifetime of patients with metastases to the brain and that its efficacy depends on timely diagnosis of the latter ones and early terms of the treatment onset.

[Formalization of a choice procedure in dosimetric planning of tele-irradiation in gamma-ray therapy].

The authors proposed a model of assessment of the usefulness of any mode of distant irradiation in gamma-beam therapy chosen for the treatment of each individual patient. The criteria were defined, functions of the calculation of the criteria of assessment of dose field quality were defined, functions for the calculation of the significance of the criteria presented for each individual patient by a radiologist, was given. The use of a plotted model to compare two dosimetric programs of irradiation of lung cancer was described.

[Comparative multidimensional analysis of the personality traits of patients with different forms of gynecologic pathology].

The personality characteristics of gynecological patients with hormonal dysfunction versus hormone dependent pathology were compared, using a factorial analysis of answers to 377 questions of the Russian modified variant of MMPI and a cluster analysis of intergroup subdivisions. A group of patients with uterine myoma (UM) differed considerably from all the others in relation to six "varimax" factors. A new factor scale was elaborated which makes it possible to differentiate a group of patients with UM from both healthy women and other gynecological patients. The results obtained are discussed in the light of the hypothesis about the psychosomatic nature of UM. The factor scale presented may be utilized for detecting a group of high risk for UM development.

[Clinical features of mental disorders among gynecologic patients with hormonal dysfunction and hormone-dependent pathology].

Using clinical procedures supplemented with a method of multiple personality investigation, the author found nonpsychotic psychic disturbances in 56.6% of 297 gynecological patients with a hormonal dysfunction of central or peripheral (inflammatory) genesis and with hormone-dependent pathology (uterine myoma, endometriosis, cystoma and cysts of the ovaries); another 22.4% of these women showed marked accentuation of personality features. The most typical psychopathological symptomatology was represented by affective disturbance (predominantly depressive, seen significantly more frequently in patients with hormonal dysfunction of central genesis rather than in hormone-dependent pathology), the syndrome of senestopathic hypochondria and psychopathlike conditions with a tendency to form super worship ideas, paranoid reactions (occurring significantly more often in patients with hormone-dependent pathology as compared with hormonal dysfunctions of central genesis). A conclusion is drawn about pathogenetic relationships existing between alterations in the psychic state and characteristics of the reviewed gynecological hormonal and hormone-dependent pathology.
