Pyrimethamine plus azithromycin for treatment of acute toxoplasmic encephalitis in patients with AIDS.

A prospective study was conducted to evaluate azithromycin in combination with pyrimethamine for treatment of acute Toxoplasma encephalitis in patients with AIDS. Of the 14 patients given 75 mg pyrimethamine and 500 mg azithromycin daily for four weeks, eight were evaluable for clinical response. Five responded favorably, one had an intermediate response and two an unfavorable response. Of the nine patients evaluable for radiological response, six responded favorably, and three had an intermediate response. Eleven adverse events occurred in nine patients: rash (n = 5), abnormal liver function (n = 2), vomiting (n = 3) and hypoacousia (n = 1). This pilot study suggests that the combination of pyrimethamine and azithromycin may be further investigated and that the optimal dosage of azithromycin has yet to be determined.

[Radiology in the medicolegal identification of cadavers. A new technique of radiography of the maxilla].

After removal, the maxilla and the mandible are placed on a standard radiologic cassette and a X-ray is performed with a low energy beam. Different views can be obtained if required. These radiographic images provide a complete, permanent and detailed record of dental anatomy and restorations, for comparison with antemortem radiographies. Two examples are provided. Moreover, this technique enables the determination of the dental age of children.

[Prognostic value of x-ray computed tomography in acute pancreatitis. Value of a new classification].

CT plays an important role in the screening of complications of acute hemorrhagic pancreatitis. There are three classifications at present, one based on a bioclinical score (Ranson) and the other two consisting of gradual stages of severity and resorting to CT imaging (Balthazar and Hill). The authors report about a study on 34 cases of acute pancreatitis, including 10 that became complicated, to which they applied a new classification including the existence of glandular necrosis, the number of extrapancreatic streaks, related to Ranson's bioclinical score. This classification makes it possible to classify acute pancreatitis into three stages: a mild stage with a favorable outcome, a medium stage requiring close surveillance, and stage 3 involving a high (75%) risk of severe complications.

[Value of techniques of measurement of bone. Radiological methods and contribution of ultrasonics].

The current interest in bone mineral density measurements is related to the frequency, the morbidity and the cost of osteoporosis, which is considered to be a public health problem in developed countries. The main risk factor of osteoporotic fractures is the reduction of bone mineral density. This risk factor can be prevented by hormonal therapy in post-menopausal women. So it is of interest to develop the bone mineral density measurement methods, in osteoporotic bone areas or the entire skeleton (absorptiometry). But the interpretation of the result in term of individual fracture risk, the choice of the bone site to be examined, and the indication of the measurement are still being discussed. A new method of bone status evaluation is currently available: the broad band ultrasonic attenuation, which measures the attenuation of ultrasounds through the calcaneus bone. The device is portable, inexpensive and radiation free, and may give quantitative and qualitative assessment of bone. However, the normal values, the relation with the absorptiometry results and the contribution of this method in fracture risk evaluation are currently studied. Osteoporosis risk evaluation is an important point of health management in post-menopausal women and elderly people. Large prospective studies with the different methods are in progress.

[The role of the radiologist in the medicolegal procedure after an aviation accident].

The role of radiologic imaging studies in the forensic medical procedure following an airborne disaster is now well established. This report summarizes the experience and the results acquired with a recent air crash, and insists on three suggestions: the necessity of a prepared identification team consisting of specialist in forensic medicine, in forensic odontology and specialist in forensic radiology, and the usefulness of a suitable structure for the study and the storage of the dead bodies, the need for a compilation of radiological informations, notably dental X-Ray examinations for the flying personnel.

[Role of MRI in the diagnosis of endocrine tumors of the pancreas].

Eight patients affected with endocrine tumor of the pancreas were examined, within the same period of time, by MRI and CT. Results from those two examinations were similar for the detection of the primary tumor (succeeding to visualize the lesion 5 times out of 8) and the evaluation of locoregional and vascular extension. No tumor smaller than 3 cm was diagnosed by MRI. Most of cases the pancreatic tumor appeared as hypointense in T1 and hyperintense in T2 sequences. MRI was a little more efficient than CT for the detection of liver metastases. MRI seems to be an interesting method for the follow-up of those patients needing a regular and prolonged surveillance after primary tumor ablation.

Subcapsular hepatic necrosis in liver transplantation: CT appearance.

Eleven cases of subcapsular hepatic necrosis were found in 47 hepatic transplantation patients who underwent CT examination between the second and 14th postoperative day. CT examinations of all 11 cases showed nonenhancing hypodense subcapsular areas with irregular contours in the liver. Major vessels were free of obstruction. Anatomic correlation, possible in one case, confirmed the diagnosis. Size disproportion between the graft and the recipient abdominal cavity reduced hepatic blood flow and caused abnormal pressure points. One or both of these factors could result in ischemia in subcapsular areas and explain the subcapsular necrosis. Although it has good prognosis without treatment, subcapsular hepatic necrosis is important to recognize to avoid confusion with liver necrosis after vascular thrombosis.

[A criterion of validity of the x-ray computed tomographic measurement of femoral neck anteversion].

A wrong patient's position on the CT scan table may lead to mistakes in the measurement of femoral neck anteversion. We present an experimental demonstration using a wood manufactured femur model. If the slices are performed with flexion of the thigh, the measurement is lower than the true value. Thus, to check the measurement on the picture of added slices, it is necessary to have a superimposition of the pictures of the upper part and the lower part of the femur. Otherwise, the measure is wrong and the mistake depends of the direction of the shifting: --if the lower part of the femur is higher, the anteversion measure is lower than expected. --if the lower part of the femur is lower, the anteversion measure is higher.

Hyperacute rejection in liver transplantation: CT findings.

Hyperacute rejection is rare after liver transplantation. We report a case of early graft failure following liver transplantation in a 46-year-old woman. On the 5th postoperative day, progressive loss of graft function occurred. Liver histology showed submassive hepatic necrosis. Computed tomography features showing large geographic areas of necrosis without anatomic distribution were suggestive of hyperacute rejection.
