Efficacy and tolerability of diclofenac potassium sachets in acute postoperative dental pain: a placebo-controlled, randomised, comparative study vs. diclofenac potassium tablets.

This double-blind, randomised, parallel-group trial compared the analgesic efficacy of single 50 mg doses of diclofenac potassium sachets and tablets with placebo in 184 patients with moderate/severe pain after third molar extraction. The primary efficacy variable was the average pain reduction from baseline during the first 2-h postdose, using a visual analogue scale (VAS). During the first 2-h postdose, sachets and tablets significantly reduced pain (p < 0.05) vs. placebo with an incremental benefit seen for sachets over tablets (p < 0.05). Onset of analgesic effect (VAS) was at 30 min for sachets and 45 min for tablets. Pain reduction vs. placebo (VAS) was maintained for 8 h for sachets and tablets (p < 0.05). VAS-findings were confirmed by pain relief and intensity verbal scale assessments. Fewer patients re-medicated vs. placebo. No safety issues were identified. This study demonstrates that both diclofenac potassium sachets and tablets offer patients suffering from acute pain conditions an effective treatment with incremental analgesic benefits seen for sachets.

Weed control in sunflower (helianthus annuus L.) with post-emergent herbicides.

Sunflower is the most important oil crop in Hungary, is the base of the production of cooking oil and moreover takes an important part in production of margarine too. Extracted sunflower groats as a secondary product origining from the mentioned procedure can be used in forage successfully. The amount of harvested sunflower reaches the 20-25% of the EU's yield. The sowing area approaches 500 thousand hectares. The essential condition of successful crop production is the perfect weed control. Sowing areas are infected with monocotyledon and dicotyledonous weeds too. Annual dicotyledonous weeds are the most troublesome. The worst species is the Ambrosia artemisiifolia L. Many other weed species as Abutilon theophrasti MEDIC., Datura stramonium L. and Xanthium strumarium L. can cause serious damages. In our model experiments we examined the herbicide sensibility of two commercial sunflower cultivars as "Iregi szürke csikos", "Marica II" and a sulfonylurea-urea tolerant new hybrid "PR63E82". The experiment was set up under greenhouse conditions with the use of four important weed and different post-emergent herbicide as Modown 4F (bifenox), Pledge 50 WP (flumioxazin) and Granstar 75 DF (tribenuron-methyl). We applied normal and double doses too. Sunflower was cultivated to 4-6 leaf stage. Post-emergent herbicides were sprayed out when weeds were in 2, 2-4 and 4-6 leaf stage. Weed killer and phytotoxic effects of post-emergent herbicides were examined. We declared that development of weeds had significally effect on the effectiveness of different herbicides.

Vegetative reproduction and chemical control with post-emergent herbicides of field bindweed (Convolvulus arvensis L.).

It is clearly seen from data that roots of Convolvulus arvensis L. have more and less intensive regenerative period during growing season. The more intensive period is in autumn, because in that time roots culminate nutrients, carbohydrate as starch and sugar. The less intensive regenerative or shoot-growing period is in spring, called "late spring bud dormancy". Experiments were conducted to get more information and further details about the regenerative capacity of roots close to and far from the collar of Convolvulus arvensis L. Root segments closer to collar have an intensive regenerative capacity than those ones further to collar. By data of Bakke et al. (1939) is well known, roots exhumed from deep soil layers are able to create shoots with low intensity. So finally we can exclaim that regenerative capacity is decreasing further to collar. Using mechanical weed control it is sufficient to till the upper layer of soil, but many times. Chemical treatments are most effective in the integrated weed control. It is clearly seen that auxin-type herbicide such as 2,4-D, fluroxipir, MCPA. dicamba give the best result. They gave 95% weed control effect used them separately or in combination with other herbicides. Combination of Banvel 480 S (dicamba) and Logran 75 WG (triasulfuron) introduced 95% weed control effect. Only one time got absolutely 100% weed control effect, in the case of Glyphosate active substance. Caused total plant destruction. Excellent result was given with the application of Pledge 50WP (flumioxazin). Herbicides mentioned above are absolutely allowed to take an important and significant part in chemical plant protection against Convolvulus arvensis L. Other herbicides like Granstar 75DF (tribenuron-methyl), Basis 75DF (rimsulfuron + tifensulfuron-methyl) and Huszár (jodosulfuron-methyl-sodium + mefenpir-diethyl) are not so effective against Convolvulus arvensis L., as compared to the previous ones.

[Selection of the method of surgical approach in plate osteosynthesis of mandibular fractures].

BACKGROUND: Plateosteosynthesis by intraoral approach of mandibular fractures gives the best cosmetical result but presents a rather difficult procedure. AIMS: Comparison of intra- versus extraoral approach in plateosteosynthesis of mandibular fractures. METHODS: Plate type, experience of the surgeon, age and sex of the patient, as well as hospital stay and rate of the septic complications were analysed retrospectively in 248 patients of the OMF Department (University Freiburg) with mandibular fractures with respect in view of the operative approach used. RESULTS: The method of osteosynthesis was found to be the strongest determinant. Concerning the surgical approach significant correlation was found between age and sex, and surgical experience. Operative approach was found to have no effect on hospitalization and septic complications. CONCLUSION: Operative approach does not have either medical nor economical consequences in the surgical management of mandibula fractures.

[The role of dalacin C in the management of odontogenic inflammations].

The penicillin-metronidazole combination is well tried antibiotic therapy for the treatment of inflammations of dental origin. However, because of the rising number of penicillin-resistant bacterium strains and penicillin allergies, other antibiotics too are being applied ever more frequently. The literature data and the authors' own experience indicate that clindamycin (Dalacin C) may be used effectively against mixed infections of the oral cavity. A detailed account of the patients and the methods will be reported in the following article.

[Experience with Otten's maxillomandibular fixation method].

The authors are reporting on a method of mandibulomaxillary fixation worked out in their institute through a modification of those defined in literature. A wire hook is fixed by a screw to the mandibula and maxilla and, by the help of it an intermaxillary fixation is formed. The procedure is simple, quickly accomplishable and cheap. Its application is recommended to be introduced for other institutes, as well.

[Malignant tumor developing in transplanted tissue in the oral cavity (case report)].

Primary and metastatic malignant tumors appears frequently in the head and neck region. The occurrence of malignancies in tissues--transplanted to this area--are extraordinary rare. Authors report about a case, where the tumor developed in a cylindroid jump flap, used to the closure of the cleft palate. They discuss theoretical aspects, as well as the course of the disease.

[Long-term studies after intra-arterial cytostatic therapy of head and neck tumors].

This study on selected tumour patients demonstrates relationship between the effectiveness of preoperative, postoperative and palliative intra-arterial cytostatic treatment of tumours of the head and neck and the long-term survival. During the period ranging between 3.5 and 9 years after preoperative treatment of 11 cases, palliative treatment of 6 cases, resulting in 100% tumour regression, and postoperative-adjuvant treatment of 23 cases (selected from 254 cases receiving intra-arterial treatment), local recurrence was observed in 10 cases. The question is raised of the necessity of radical surgery corresponding to the original tumour after preoperative treatment resulting in a completely tumour-free state.
