[Results of coloscopic polypectomy in respect of the dignity (author's transl)].

The commonest type of polyps in the large intestine in our series of 506 polypectomies in 262 patients were adenomas (64%) especially tubular adenomas. Severe epithelial atypia being present in 11% of all adenomas and it was three times more discovered in villous adenomas than in tubular forms and with increasing adenomas size. Because severe epithelial atypia could be observed in polyps with diameters less than 5 mm it should be demanded to remove each polyp for an exact histological examination independent of his size. Single forceps biopsies of polyps do not allow the estimation of their further development in view of malignancy.

Campylobacter enteritis in infants and young children.

Campylobacter has recently been recognized as a common pathogen of the intestinal tract in pediatric practice. We report on 21 patients who were diagnosed as having enteritis due to Campylobacter jejuni infection. The most common symptom was diarrhea, accompanied by fever, vomiting and abdominal pain. The in vitro sensitivity test demonstrated the efficacy of aminoglycosides, chloramphenicol and erythromycin in the treatment of this disease. All the patients were symptom-free when discharged from the hospital.

[Carbohydrate tolerance and intracellular sodium concentration in rabbits with dietary hypercholesterolemia].

The transport of sodium ions and glucose is a specialized membrane function. Variations in the cholesterol content of the cell membrane influence these functions. The influence of a hypercholesterolemia upon the intracellular sodium concentration and the glucose tolerance was investigated in cholesterol/rape oil and in cholesterol fed rabbits. The cholesterol/rape oil fed rabbits had lower KG-values and higher peripheral insulin levels than the control animals. The Na+Ery was decreased after 12 weeks of hypercholesterolemia. The na+Muscle was not changed. A negative correlation was found between the KG-value and the Na+Ery (Na+Muscle) in normal rabbits. In the hypercholesterolemic rabbits existed a negative correlation between the KG-value and the Na+Muscle and a positive correlation between the KG-value and the Na+Ery. It was postulated that membrane functions determined these correlations.

[Behavior of intracellular electrolytes in the diabetic metabolic state].

By animal experiments and examinations of test persons with healthy metabolism could be confirmed that a high intracellular content of sodium coincides with a low glucose assimilation coefficient. Moreover, the intracellular sodium concentration seems to have influence also on the peripheral insulin content after glucose load as it was shown by the negative correlation of the quotient Na(Plasma)/Na(Ery) with the insulin concentration surface in the rabbits. In maturity-onset diabetics with and without hypertension compared with a control group the intracellular sodium concentration was increased. This intracellular shift of electrolytes might have an influence of the utilisation of glucose in maturity-onset diabetes.

[Value and results of coloscopic polypectomy].

From August 1976 till the end of February 1979 158 polyp in 83 patients (48 male and 35 female) were removed by means of the rectocoloscopic polypectomy with the high frequency diathermy loop. 66% of the polypi had a diameter of less than 1 cm. Most the polypi were with about 70% in the sigma and in the rectum. With 60.7% the adenomas appeared most frequently, followed by the hyperplastic polypi in 20% and by the inflammatory ones in 13.3% of the cases. With the increase of size of the polypi the proportion of the adenomas clearly increased in comparison to the other kinds of polypi. Severe epithelial atypias (altogether 23.1%) were more frequently in adenomas larger than 2 cm than in smaller ones. An invasive carcinoma was found in no polypus. Thus the endoscopic polypectomy in every case meant a curative therapy with great economic use. In patients a haemorrhage appeared as a complication of the coloscopic polypectomy. Only the removal of the polypi in toto allows an exact histological classification and the answer of the decisive question, whether in one adenoma only severe epithelial atypias are present and thus the polypectomy is therapeutically sufficient or whether there exists an adenocarcinoma, for the removal of which an operation according to the principles of the carcinoma surgery must be carried out. The endoscopic ablation of the adenomas thus means a genuine prophylaxis of cancer.

Studies on the preparation and properties of ribonucleoprotein particles from rat liver nuclei.

Ribonucleoprotein particles of 38 S were extracted from rat liver nuclei with isotonic salt buffer under concomitant sonication. The fate of the endogeneous nuclear RNAases assayed with poly(A), high molecular weight yeast RNA and rapidly labeled hnRNA was followed during the preparation of 38-S nuclear ribonucleoprotein (nRNP) particles. Essentially all the RNAase activity could be removed from the particle preparation. The effect of synthetic RNAase inhibitors on the nRNP particles was studied. Upon extraction of nuclei with 0.14 M NaCl, approximately 38% of the total nuclear radioactivity was found in the 38-S nRNP particles. By two successive extractions of the remaining chromatin with either isotonic or 0.22 and 0.3 M NaCl, an additional 25 and 9% of rapidly labeled hnRNA of 38 S particle were dissociated from chromatin, respectively. The chromatin components, DNA, nonhistone proteins, histones and RNA were determined after successive salt extractions. Particularly alterations in the nonhistone proteins and RNA were found. The protein patterns upon SDS-acrylamide gel electrophoresis of the salt-extracted chromatin preparations were compared with those of the 38-S nRNP particles. Particularly proteins in the molecular weight range of 32 000-43 000 were dissociated from chromatin after treatment with 0.22 or 0.3 M NaCl.

[Ultrastructural and functional changes of rabbit myocardium after cholesterol feeding].

The myocardium was ultrastructurally investigated immediately after 12 weeks of feeding in one part of the animals and in the remaining animals 27--30 months after termination of the feeding period. At the same time, parameters of the myocardial contraction were measured immediately after the feeding at isolated trabeculae of the left ventricle and 27--30 months after the termination of the feeding at the left ventricle in situ. After the 12 weeks of feeding the morphometrically estimated intracellular lipid content rises from 0.23% (normal) to 5.26%. The mitochondria decline from 36.98% (normal) to 29.08%. The sarcoplasmic reticulum (SR) changes in a vesicular or vacuolar manner respectively. The contractility index Vmax of isolated trabeculae decreases insignificantly. The resting potentiation and the diastolic stiffness are increased. 27--30 months after termination of feeding the lipid content and the mitochondria are normal in comparison to controls of the same age. The vacuolar changes of SR and its cellular portion are increased. The contractility index of the heart in situ max (dp/dt)/P is decreased insignificantly. The velocity of relaxation is enhanced in comparison with controls. By comparing ultrastructural and functional findings it can be seen that variations in the contraction performance are accompanied by stronger changes of the cellular structure immediately after the feeding than 27--30 months after the termination of the feeding. At this time continuing small alterations in the contraction performance of the heart in situ are observed in addition to the reversibility of various characteristics of damage together with an increase in the Ca++-transporting SR and its alterations.

[Method for the immediate measurement of myocardial pressure velocity relationships in situ].

A method for instantaneous measurements and representations of myocardial pressure-velocity relations permitting the analysis of various contractility parameters is described. Basing on a modified two-element model of cardiac muscle the measurement of the relative shortening velocity vce = dp/dt(p) of the contractile elements is carried out by a special analog computer, which calculates the quotient (dp/dt) to the simultaneous left ventricular pressure p. Electronic differentiation of the logarithm of pressure-proportional input voltages is used. The p-v-diagram can be displayed on x-y-oscilloscopes. Over an input voltage range of 20 mV---20V the quotient can be measured within the range of 20 sec-1---250 sec-1. Output voltages can be calibrated automatically. Between (dp/dt)p-1 determined by conventional methods and the dlnp/dt calcuated electronically there exists a correlation gamma = 0.995. An additional electronic circuit which permits the determination of a contractility parameter indicates the point of (dp/dt)max on the p-v-relation display. The application of the method in experimental studies under inotropic changes in the rabbit heart in situ following beta-receptor blockade is demonstrated.

[Correlation between roentgen morphometrically obtained data and cortical thickness indicative of anchorage strength of screws in human cadaver vertebral bodies].

Through the morphometric and histomorphometric examinations on the vertebral of the corpses, a functional connection between roentgenmorphometric plain facts (in transverse- and sagittal diameter of the vertebral body) and the cortical thickness can be established. The stress of the vertebral body through a screw can already preoperatively be assessed by roentgenmorphometric examinations that the cortical thickness plays an important dimension in the anchorage tightening of the screws on the vertebral body. By use of this knowledge the guarantee of operation on vertebral body is raised.
