[Results of combined therapy for malignant tumors of the nasal cavity and accessory nasal sinus depending on the fractionation of preoperative radiotherapy].

Data on the treatment of 215 patients with locally advanced malignancies of the nasal cavity and accessory nasal sinus are presented. All patients received preoperative radiotherapy using different fractionation schemes under a total target dose (TTD) of 32-40 Gy. Surgery was carried out two weeks later. Whenever radicality of surgery was in doubt, postoperative gamma therapy included exposure to 2 Gy 5 times a week (TTD--30-40 Gy) (n=121). Group I (n-57) received radiation at standard fractionated dosage. In group II (n=66), fractionation, tentatively considered "dynamic" was used while, in group III, fractionation was dynamic in actual fact. In group IV (n=10), superfractionation (1+1 Gy) was employed in group V (n=6),--gamma-neutron therapy. Patients in group VI (n=51) were operated on as scheduled and received 5 Gy, twice a week (TTD--20 Gy). According to 5-year follow-up, the best results were obtained with the use of larger doses of preoperative radiation (groups II, III, VI). Postoperative radiotherapy proved ineffective and it would be useless unless ablative procedures were definitely faulty during surgery.

[Chemoradiation therapy in conjunction with physical means of radiomodification for locally advanced laryngeal cancer].

The study deals with the results of examination and treatment of 1040 patients with cancer (T3-4N0-3M0) of larynx and laryngopharynx. Radiotherapy was given to 358 patients concurrently with systemic polychemotherapy and different means of radiomodification. The remaining patients (682) receiving the same treatment minus systemic polychemotherapy were in control. Relapse-free (72.6%) and overall survival (82.7%) in the study group was significantly higher than in controls (54.7% and 63.4%, respectively) (p < 0.001). Moreover, an organ's function was preserved in 57.1% compared with 28.5% in control (p < 0.001).

[Intratracheal lymphotropic ozone therapy in erosive-ulcerous tracheitis].

Under observation there were 452 patients with chronic stenosis of hollow organs of the neck having cannulas during the period from 3 months to several years. During the tracheobronchoscopic examination it was found that 35 patients had trachea ulcers, 46 patients had erosive tracheobronchitis. The ulcers were localized on the anterior wall of the thoracic part of the trachea. Their diameter was from 1 to 2.5 cm. The ulcers were accompanied by diffuse bilateral bronchitis of the II-III degree of the inflammation intensity. 2-3 ml of ozonated sodium chloride solution with the concentration of ozone in it 5 mg/l were introduced into the ulcer edges, i.e. lymphotropically, into the submucous membrane. The same solution (40-60 ml) was used for daily sanitation of the tracheobronchial tree. Complete epithelization of the ulcers and cleansing of the bronchial tree took 3-4 curative bronchoscopies.

[Serotonin level in endocrine cells in bronchial mucosa and stomach in chronic obstructive lung diseases].

85 patients aged 26-60 years with pulmonary chronic obstructive diseases (PCOD) and gastric and duodenal erosive-ulcerous lesions were investigated. In PCOD the increased bioamines level in endocrine cells of bronchial ciliated epithelium and gastric glandular epithelium was detected before the treatment. The complex treatment in the form of bronchoscopy with lymphotropic administration of antibiotic and immunomodulator against the background of intragastral ozonotherapy contributes to the decrease in tissue edema, restoration of their trophicity and inflammatory process resolution. Against the background of therapy the serotonin content in endocrine cells of bronchial ciliated epithelium and gastric mucosa decreased to the norm.

[Bronchoscopy in complex examination and treatment of patients with lung abscess].

Lung abscess was diagnosed during a complex examination in 115 patients. All the patients had local treatment through the bronchoscope. Cleansing of the abscess cavity takes place 1,3 times quicker in the patients who were given laser irradiation in addition to sanitation bronchoscopy or to whom immunomodulators and antibiotics were introduced lymphotropically intrabronchially, than in those with usual sanitation. The administration of 20 ml of glycerol in the abscess cavity allowed cleansing of the cavity to be 2-3 times quicker.

[Thermotherapy for laryngeal cancer].

An analysis of the data on the treatment of 91 cases of advanced cancer of the larynx showed that local UHF-hyperthermotherapy is more efficient than the SHF one as a component of radiotherapy. Use of the latter procedure was followed by a higher rate of late-onset radiation injuries. Moreover, combined application of hyper-fractionated irradiation and UHF-hyperthermotherapy involved practically no grave complications while 5-year survival increased.

[Fast neutrons in the treatment of malignant neoplasms].

More than 250 patients, mainly with locally advanced tumors of the head, neck and breast were treated with a I MeV beam of fast neutrons emitted from a BR-10 reactor. The fast neutron contribution to the total dose of the radical component of complex gamma-neutron therapy was 20-40%. Said modality was shown to improve the results of treatment for laryngeal and breast tumors. The 5-year actuarial survival following complex gamma-neutron therapy for laryngeal cancer was 89.25, while in controls--65.2% (p < 0.005). The 5-year actuarial overall survival in breast cancer patients treated with fast neutrons was 66.5%: gamma-neutron therapy alone--46.0% (p < 0.05).

[Combined therapy of malignant tumors of the upper jaw and nasal cavity using pre- and post-operative radiation with metronidazol radiomodification].

Seventy-five patients with malignant tumors of the upper jaw and nasal cavity have received combination therapy. Administration of metronidasol (5-8 g/m2 body surface) in combination with large doses of preoperative radiation was shown to significantly improve 3-year total and recurrence-free survival-from 50 to 60% and 32 to 57%, respectively. As a result, no postoperative radiotherapy was required. Postoperative radiation treatment is indicated when means of radiosensibilization are not used. The use of radiomodifiers is mandatory in combination therapy of non-differentiated cell carcinoma, cylindroma, transitory-cell papilloma and non-epithelial neoplasms.

[The hematopoietic characteristics of laryngeal cancer patients undergoing thermoradiosensitization].

Peripheral blood and bone marrow were studied in 46 patients suffering laryngeal cancer. Two schedules of radiotherapy with and without local hyperthermia were used. Complex hematologic study included electron microscopy of the bone marrow. Apart from general hematologic parameters, partial erythro- and granulocytograms of the bone marrow, mitotic index of karyocytes and morphologic anomalies of erythrokaryocytes were studied. Hematologic studies were conducted both before and after a course of radiotherapy. Adverse effect of hyperthermia and radiation therapy on cells of the granulocytic and immunologic component of hemopoiesis was established.

[Combined radiomodification in the treatment of patients with disseminated laryngeal cancer].

The paper is devoted to comparative estimation of radiation and combined therapy of laryngeal cancer patients using combined radiomodification including local hyperthermia, total hyperthermia, chemotherapy and a method of dynamic dose fractionation. Complete and significant tumor regression at a dose of 32 Gy was noted in the study group (77 patients) and in the control group (64%). Radiotherapy against a background of combined radiomodification tends to increase the 3-year survival up to 86% as compared to 66.7% in the control group.

[The treatment of laryngeal cancer by using local hyperthermia].

Potentiation of the effect of radiation and combined treatment of laryngeal cancer by local hyperthermia was studied in 43 patients. In 51 cases of the control group (radiotherapy), conventionally-dynamic fractionation of dose was used. At 32 Gy, complete or partial regression of tumor was observed in 72.1% in the study group and 64.7% of controls. An increased frequency of radiation-induced damage of the oral mucosa was observed whereas radiation epidermitis did not occur. Three-year relapse-free survival following a course of radical thermoradiotherapy was as high as 76.5% compared to 40% in controls. Overall three-year survival (including the results of combined treatment and treatment for relapse) increased from 66.7 to 83.3%.

[The functional state of the thyroid gland in cancer of the larynx].

The paper is concerned with comparative evaluation of the influence of some therapeutic methods, based on the use of different radiation sources, dose fractionation regimens and radiomodifiers, on thyroid function in laryngeal cancer patients. Thyroid function was retrospectively assessed with regard to a course of disease within 2 yrs. after a radical course of radiation or combined therapy. Concomitant gamma-neutron therapy and radiotherapy in combined radiomodification were shown to produce the most inhibitory effect. Irrespective of a course of disease after a therapeutic course thyroid function in laryngeal cancer patients is decreased within 2 yrs. of observation.

[X-ray irradiation of donor blood in the complex treatment of suppurative diseases].

The authors discuss the use of donor blood after isolated exposure to X-rays in the complex treatment of 65 patients with various suppurative diseases. The therapeutic complex included in all cases transfusion of 250 ml of irradiated blood and subsequent antibiotic therapy. Study of antibiotic (penicillin) pharmacokinetics showed increase of its biological activity and prolongation of its presence in the blood in therapeutic concentrations for 24-48 hours. A scheme of antibiotic therapy in transfusion of irradiated blood is suggested. A formula for calculating the optimal irradiation dose is given. It is concluded that transfusion of irradiated blood followed by injection of an antibiotic raises the efficacy of antibiotic therapy, reduces the term of treatment, and increases the effect of postoperative management.

[Hemostasis in cancer patients during radiation therapy using metronidazole].

Indices of platelet function, plasmic coagulation and fibrinolysis were studied during gamma-beam therapy in patients with laryngeal, gastric and cervical tumors (potentially at risk for thrombosis) against a background of metronidazole administration. During the above therapy the thrombophilic status of gastric cancer patients was preserved. These patients were considered at risk for thrombosis. In patients with laryngeal and cervical cancer platelet capacity to adhesion was considerably reduced during radiotherapy with the use of metronidazole. As a result of it the thrombogenic blood potential and danger of intravascular thrombogenesis were decreased.

[Tumor markers and hormonal homeostasis in laryngeal cancer].

A radioimmunoassay of 3 tumor markers and 13 hormones was performed before therapy in 166 laryngeal cancer patients with relation to tumor spreading. The levels of these markers and hormones excluding testosterone, were within control indices recommended in instructions for certain test kits. At the same time direct correlation was found between the content of ferritin, calcitonin, parathormone and tumor spreading. Testosterone concentration in all the patients was below the control level. Control of the hormonal status of laryngeal cancer patients was recommended during therapy with the use of radiomodifying agents to determine the effect of various radiomodifiers on the endocrine system, for the assessment of therapeutic efficacy and, probably, the prediction of the course of the disease.

[Dose multifractionation in radiotherapy of laryngeal cancer].

A study was made of the comparative results of radiotherapy of 332 laryngeal cancer patients of whom 237 were treated by routine methods and 95 by multifractionation (double fractionation of a daily dose by 1 Gy with a 4-5 h interval). The method of dose multifractionation was mathematically substantiated. Direct efficacy of radiotherapy was assessed in the delivery of a dose of 40 Gy. A radical dose of irradiation was 66-70 Gy. At 40 Gy complete tumor resorption in the patients of the control group was noted in 33.3% of the cases, in the study group in 58.9%. The 3-year recurrence- and metastasis-free survival rate after radical radiotherapy was 47.4% in the control group and 65.3% in the study group. A decrease in the number of late postradiation edema of the laryngeal mucosa from 28.7% in the control group to 8.7% in the study group was noted.
