Identification with monoclonal antibodies of T-cell subsets in peripheral blood and ascitic fluid of patients with alcoholic hepatic cirrhosis.

In an attempt to investigate an aspect of the immunoregulatory mechanisms operating in alcoholic hepatic cirrhosis with ascites we have enumerated T-cell subsets in the peripheral blood and ascitic fluid of 16 patients with this disease. The peripheral blood analysis revealed a reduction of OKT3+, OKT4+ and OKT8+ cells compared to those of the control group (p less than 0.001) and an increase of OKT4+/OKT8+ ratio. In the ascitic fluid the OKT4+/OKT8+ ratio, compared to that of the peripheral blood, was decreased (p less than 0.001) because of increase of OKT8+ cell proportion. We suggest that the sequestrated splanchnic OKT8+ subset might be represented by suppressor lymphocytes transuding from the liver.

[Bromhexine in symptomatic treatment of Sjögren syndrome].

In a group of 11 patients with Sjögren's syndrome bromhexine improved tear secretion in 7 cases and salivary secretion in 4. The efficacy of the drug, in particular in patients in whom the syndrome has been present for a short time, was thus confirmed.

[Proglumetacin for a wide spectrum of activity in clinical medicine].

Twenty-nine patients with gouty arthritis, rheumatoid arthritis and osteoarthritis were treated as outpatients with proglumetacin, at different doses, during 5-30 days. In all patients with gout and osteoarthritis a significant improvement of pain and inflammatory symptoms was observed, together with a recovery of joint mobility. Seventy-nine per cent of patients with arthritis responded well or very well to the therapy. Tolerance was considered good or very good in 93% of patients.

[Basic therapy of rheumatoid arthritis with thiopronine. Preliminary results].

A long term open study of the use of thiopronine in the treatment of 38 patients with ARA classified "classic" or "definite" rheumatoid arthritis is reported. Doses of the drug were increased from an initial 250 mg per diem by the addition of 250 mg every 15 days to 1 g per diem. After 6 months testing, results were satisfactory (good and fair) in 65% of the cases in both biological and clinical terms. In 15% of the cases, treatment was interrupted mainly due to mucous and cutaneous side effects. These preliminary results confirm reports in the literature and encourage further experiment.

[Salivary immunoglobulins G, A and M in liver cirrhosis].

Mixed saliva of the patient with cirrhosis of the liver contains often large quantities of immune globulins. IgA and M transport is facilitated, but whereas IgA globulins are present in all cases, the IgM appear in the saliva only where plasma concentration is greater than 200 mg %. Owing to a simple passive diffusion mechanism, the IgG reach high saliva levels in all subjects studied.

[Heterotopic ossification in the site of a surgical scar].

The pathogenesis and clinical picture of heterotopic ossification at a surgical scar location are discussed on the basis of the information reported in the most recent literature and personal series observed in one year in a surgery department. Stress is laid on the importance of the metaplastic process in the development of this picture, its benign nature, the nearly total lack of recurrences and the need for correct differential diagnosis, particularly as regards tumoural metastases.

[Hypomagnesemia with magnesium-dependent hypocalcemia and hypopotassemia in a case of alcoholic cirrhosis].

An account of the main causes of low blood magnesium and the predominant rôle of this ion in the organism is followed by the description of a case of encephalopathy due to magnesium deficiency in a chronic alcoholic with cirrhosis of the liver. This neuropsychological condition, together with its concomitant low blood calcium and potassium, rapidly regressed when magnesium sulphate was administered. Stress is laid on the desirability of giving attention to and preventing the onset of this electrolytic alteration in clinical practive.

Efficacy and tolerability of nimesulide in elderly patients with osteoarthritis: double-blind trial versus naproxen.

A double-blind trial was carried out to assess the therapeutic efficacy and tolerability of nimesulide versus naproxen in the treatment of elderly patients with osteoarthritis. A total of 40 elderly female patients with osteoarthritis of the hip and/or knee were entered into the study and treated with either 200 mg/day nimesulide or 500 mg/day naproxen for 28 days. Both treatments were very effective in alleviating spontaneous pain, pain on movement, morning stiffness and joint mobility. Nimesulide was better tolerated than naproxen with fewer and less serious side-effects being reported. The results of this study show nimesulide to be effective and well tolerated in the treatment of elderly patients with osteoarthritis.

Salivary gland echography in primary and secondary Sjögren's syndrome.

A series of different ultrasonographic abnormalities detected by salivary gland echography (SGE) were investigated for their discriminant power for Sjögren's syndrome (SS) in 53 patients with either primary SS (n = 27) or secondary SS (n = 26), as well as in 90 controls. Among the controls, 26 suffered from dry mouth and/or recurrent or persistent swelling of at least one parotid or submandibular gland due to other selected disorders, while 64 were healthy, asymptomatic subjects. Mild, evident or gross inhomogeneous parenchymal patterns were the only variables selected by stepwise discriminant analysis, when comparing patients to controls. However, a mild submandibular inhomogeneity did not prove useful for such a discrimination. Based on these data, a simplified evaluation and standardised quantification of salivary involvement, as detected by SGE, is proposed using an echographic score (range 0 to 6) which assigns points to the different degrees of glandular inhomogeneity. Score values above 0 showed a sensitivity of 88.8% in primary SS and of 53.8% in secondary SS, as well as a specificity of 84.6% and of 92.2% with respect to either symptomatic or healthy controls. The lower sensitivity of SGE for patients with secondary SS presumably was a result of their milder salivary involvement.
