[A formalized patient history--a new form of medical documentation].

In a work, the characteristic of a new form of medical documentation--formalized case record (FCR), which permits to obtain the detailed information about the state of a patient at all the stages of treatment, is given. Because of the more and more wide introduction of a computer into the clinical practice, FCR can be considered as a necessary form of medical documentation. The universal character of a FCR permits to use it at different in-patient departments of surgical profile.

[The role of Treitz' ligament in regulating the evacuation of solid food from the duodenum].

In the chronic experiments on dogs by means of modified method of multiple drainage of the intestinal fistulas, the effect of the Treitz ligament transection on dynamics of solid carbohydrate food evacuation from the duodenum was studied. It was established that in the norm, evacuation from the duodenum was submitted to exponential law and depended on absolute quantity of food evacuated from the stomach. After the Treitz ligament transection, the evacuation through intestine had a non-exponential character. The operation led to disturbance in co-ordination between evacuatory function of the stomach and that of duodenum, and also had the depressive effect on intestinal emptying. The conclusion is made of the functional role of the Treitz ligament as active regulator of solid food evacuation from the duodenum.

[The role of the duodenojejunal flexure in regulating the motor evacuatory function of the duodenum].

The authors studied the macro- and microstructure of the Treitz ligament and duodenojejunal flexure in 92 cadavers. The variants of the Treitz ligament diversion from the duodenojejunal flexure, its topographic anatomy are described. The effect of the Treitz ligament transection on evacuatory function of the duodenum in 3 dogs was studied. The authors consider the Treitz ligament and duodenojejunal flexure as a valve, which regulates the duodenal content emptying. The possible mechanisms of the duodenojejunal valve functioning are discussed.

[Recurrent ulcers following selective proximal vagotomy (10 years' results)].

The results of selective proximal vagotomy (SPV) in 1266 patients were studied. In 778 (61.5%) SPV alone was performed, in 488 (38.5%)--SPV with drainage operation: Finney pyloroplasty--in 249 (51%), Heineke-Mikulicz pyloroplasty--in 77 (15.8%), Jaboulay gastroduodenostomy--in 75 (15.4%), anterior hemipylorectomy--in 49 (10%), other drainage operations--in 38 (7.8%). After isolated SPV, excellent and good results were noted in 81.5% of the patients, satisfactory--in 11.1%, unsatisfactory--in 7.4%; after SPV with drainage operation--in 87.7%, 9.6% and 2.7%, respectively. An ulcer recurrence after SPV was revealed in 58 (7.4%), after SPV with drainage operation--in 13 (2.7%) of the patients examined. In 90% of cases, an ulcer recurrence developed at the pyloroduodenal zone, in 10%--in the stomach. The incidence of ulcer recurrence was 8.7% after operations performed by surgeons, who had not enough experience with the SPV performance, and 4.4%--by experienced surgeons. In 46.5% of cases, in recurrent ulcer, the conservative treatment was performed, and the remaining patients underwent surgery (antrumectomy, revagotomy etc.).

[The Roux syndrome and its clinical significance].

On the basis of the analysis of the use of the Roux-en-Y jejunal interposition in surgical treatment of 147 patients with diseases of the stomach and duodenum, a conclusion is made of the expediency to distinguish the special type of functional disorders developing after the operation, which are called the Roux syndrome. Clinical manifestations and methods for the treatment of the mentioned syndrome are elucidated in detail.
