[Specifics of diagnosis and treatment of gastroduodenal diseases associated with Helicobacter pylori infection in surgical clinic].

Complex of diagnostic measures in patients with gastroduodenal ulcer disease includes conduction of investigations for revealing of helicobacteriosis, dysbacteriosis, helminthosis; esophagogastroduodenoscopy with optical biopsy and biopsy for histological and cytological investigation; computeric analysis of the blood neutrophils and buccal epithelium. Conservative treatment is conducted under endoscopic control, curative endoscopy and investigation for Helicobacter pylori are done additionally if necessary. In the absence of the conservative treatment efficacy various kinds of organpreserving surgical interventions are applied.

[Antibacterial therapy of patients with inflammatory necrotic pancreatitis].

The infectionized necrotic pancreatitis (NP) course, complicated by localized and diffusive peritonitis, abscess and retroperitoneal phlegmon in 86 patients, was analyzed. Severity of state of patients according to the APACHE II scale was estimated. Recommendations for application of empirical and purposeful antibacterial therapy was elaborated. Total mortality for infectionized NP with complicated course was 25.5%.

[Modern trends in the treatment of the obturated colonic ileus].

The results of treatment of 365 patients with obturated colonic ileus, caused by tumor, for the period from 1979 to 2000 years were analyzed. Till 1987 year in clinic were applied proximal colostomy, excision of tumor, restorative stage--in 3-4 months. From 1987 year using the method of intraoperative colonic lavage we performed simultaneous intervention with formation of primary anastomosis.

[Selective proximal vagotomy in duodenal ulcer complicated by decompensated pyloroduodenal stenosis].

The result of surgical treatment of 281 patient with duodenal ulcer disease, complicated by decompensated pyloroduodenal stenosis, was presented. Analysis of compensational possibilities of stomach motility and application of elaborated method of diagnosis and preoperative preparation have permitted to perform in 243 (86.6%) of patients the organ-preserving operation. Operative interventions of resectional type are done in 38 (13.4%) of the patients. Late follow-up result of treatment was studied up in 172 patients in terms from 1 year to 5 years. Postresectional syndrome have occurred in 15 (8.5%), recurrency--in 10 (6%) of patients. Mortality was 1.2%.

[The administration of sandostatin in the combined treatment of acute pancreatitis and its complications].

Experience of application of the somatostatin synthetic analog sandostatin ("Novartis" firm) in treatment of 212 patients, including 84--with an acute pancreatitis (AP), 41--with postoperative AP (APP), 15--with external pancreatic fistula and 72--with high risk of the APP occurrence for prophylaxis was summarized. The significant inhibiting action of preparation on endocrine pancreatic function was noted. High efficacy of sandostatin in patients with AP and its complications was determined.

[The administration of peflocine (pefloxacin) in the treatment and prevention of surgical infections].

The results of treatment of 23 patients with pefloxacin, administered because of the infectional complications in surgery development (1 group), and of 11 patients --for the infection development prophylaxis (2 group) were discussed. In the first group the recovery was noted in 83.3%, and significant improvement of condition--in 16.7% of patients. In the second group the infectional complications in postoperative period were absent.
