[Urologic manifestations of AIDS].

The frequency of urological manifestations of AIDS appears to be increasing at the present time: between 1985 and 1988, 38 HIV1 seropositive patients were examined in the Urology department. In seven cases, the positive serology was discovered during investigation of a urogenital infection and 21 patients suffered from declared AIDS. The reasons for admission to hospital were: urogenital infection: 14 cases, dominated by epididymitis (7) and prostatitis (5), characterized by their frequent progression towards abscess formation; tumours: 6 cases, including 5 Kaposi sarcomas of the penis; and, lastly, neurological manifestations, chronic renal failure and urological diseases unrelated to AIDS. This study indicates that serology should be performed routinely in cases with a known risk factor and/or with an atypical urogenital condition, particularly infectious. The adoption of aseptic techniques and rigorous protection of the entire surgical team is now absolutely necessary.

Analysis of hamster lymphomas for the presence of hamster papovavirus DNA.

The hamster papovavirus (HaPV) is a polyomavirus isolated from skin epitheliomas arising spontaneously in young Syrian hamsters. It can induce lymphomas and leukaemias in newborn hamsters. Although no virus particles are detectable by electron microscopy, high amounts of monomeric and oligomeric forms of extrachromosomal HaPV DNA molecules are found in the lymphoma cells. These molecules display deletions of about 300 nucleotides in length. Their role in the lymphoma induction is discussed.

[Transrenal percutaneous treatment of urinary fistula. Apropos of 19 cases].

Percutaneous nephrostomy (PCN) was used to treat 16 ureteral fistulas, two ileal fistulas following ileo-cystoplasty, and one pelvic fistula. Discharge resolved in all cases. PCN alone achieved complete recovery of the ileal and pelvic fistulas. Insertion of a wire-guide through the fistula into the bladder and stenting of the ureter for 5 to 20 days with a 8 to 10 F multi-side-hole catheter was possible in 12 of the ureteral fistulas and ensured complete recovery in every case. Because of complete stenosis, this procedure failed in the four other cases of ureteral fistula, and surgery was therefore required. Transrenal percutaneous treatment of urinary fistulas is a simple, effective procedure requiring only local anesthesia, and can be recommended in recently operated patients, and when retrograde catheterization is inadvisable (ileo-cystoplasty).

[An abdominal approach above the liver to the inferior vena cava].

The inferior vena cava (IVC) can be approached abdominally above the liver, either below the diaphragm, or in the fibrous pericardium, or in the serous pericardium. These techniques are described and discussed, as is the venous clamping upstream, always essential. Although rarely used, these procedures should be known by all surgeons.

[Correction of deviations of the penis by the Nesbit operation].

The Nesbit procedure is proposed in the treatment of the penile curvature. Our technique is a longitudinal incision followed by a transversal suture. From 1981 to 1986, 25 patients were operated: 19 fair results, 4 good, 2 unsatisfying. The shortening of the phallus is the major pitfall of the technique.

The anatomy of erection.

Erection is due to a neurovascular mechanism: an inflow of arterial blood and an obstruction of venous return. The direction of this mechanism is of neurologic origin: (1) control of the smooth muscle tissue of the corpora cavernosa by the autonomic system controlled by reflex centers in the cord or cortical centers accessible to multiple (visual) stimuli (2) control of the perineal muscles by the somatic system. Tumescence is due to a reduction in the alpha-sympathetic tonus of the cavernous tissue permitting influx of arterial blood, and to decreased venous flow from compression of the subalbugineal venous network against the tunica albuginea of the corpus cavernosum. Once this obstruction to the venous return has been achieved, the arterial flow in the corpora cavernosa decreases but persists; the arterial blood in the corpora cavernosa is renewed and not shut off. Rigidity is due to an increase in intracavernous arterial pressure simultaneous with contraction of the perineal muscles (ischiocavernosus) under the somatic control of the pudendal nerve.

Induction of lymphomas by the hamster papovavirus correlates with massive replication of nonrandomly deleted extrachromosomal viral genomes.

The hamster papovavirus isolated from skin epithelioma can induce lymphomas and leukemias after subcutaneous inoculation into newborn hamsters. The lymphoma cells are virus free but contain large amounts of extrachromosomal hamster papovavirus DNA. We have cloned and partly sequenced some of these DNA molecules from independent tumors. These genomes displayed overlapping deletions consistently sharing a common end within the noncoding regulatory sequences; the other end was variable but always extended into the sequence coding for the N-terminal part of the viral capsid VP2. This unique in vivo interaction between a polyomavirus and its cellular host, the genesis of these variant molecules, and their role in the lymphoma formation are discussed.

[Complications of the treatment of renal calculi by extracorporeal lithotripsy. Prospective evaluation of 80 treated kidneys].

The acute effects of extracorporeal shock wave lithotripsy (ESWL) on morphology and function of the kidney were prospectively evaluated by abdominal radiography, ultrasonography and contrast-enhanced computed tomography, in 80 treated kidneys. Two types of complications were demonstrated: 1. Obstruction of the ureter by stone fragments in 25% of cases. Relief procedures became necessary in 5 patients (6% of the 80 cases). 2. Limited injuries of the kidney and perirenal spaces in 44% of cases: parenchymatous abnormalities 17.5%; subcapsular hematoma 12.5%; perirenal fluid collection 24%. No treatment was necessary and morphologic abnormalities disappeared in one or two months post-ESWL. Severe ureteral obstruction after ESWL seems to be related to stone size. No definitive evidence that kidney's injury was related to the number of shock waves applied to the kidney was found.

[Percutaneous treatment of fistulas and stenoses after anastomosis of the uretero-digestive tract].

14 ureteral stenoses and 5 fistulae following an uro-intestinal anastomosis (UIA) were managed by a transrenal percutaneous approach. The stenoses (12 uretero-ileal and 1 uretero-colic anastomoses) were dilated with an angioplasty balloon and stented for several weeks. After withdrawal of the stent, performed on 12 out of 14 patients, the dilatation was successful in 8/12 cases (66%), with a follow up of 3 to 36 months. All the fistulae were dried. In 2 cases, complete recovery was achieved after placement of a bilateral nephrostomy tube. In 2 other cases, the ureter was stented at the site of the fistula which dried without sequelae after withdrawal of the stent. In the last case (uretero-colic diversion) surgery was performed after the fistula dried for the cure of a complete stenosis associated with the fistulae. The use of percutaneous nephrostomy is highly recommended for the management of post-operative stenoses and fistulae before considering surgical correction.

The hamster papovavirus: evolutionary relationships with other polyomaviruses.

The hamster papovavirus (HaPV) is a polyoma virus with a restricted tumor spectrum. It is actively replicated in hair follicle tumors arising spontaneously in young Syrian hamsters. It can also induce lymphomas and leukemias in newborn hamsters. The complete nucleotide sequence of a cloned HaPV has been established recently. This report presents a comparison of this sequence with other polyomavirus genomes (polyoma, SV40, BKV, LPV) by matrix dot analysis and electron microscopy heteroduplex mapping. The results demonstrate a close relationship between the HaPV and the murine polyoma virus and designate the LPV as the closest relative among the primate polyomaviruses.

[Benign ureteral stenoses. Dilatation with an angioplasty balloon].

An angioplasty balloon was used to dilate 23 benign ureteral stenoses in 21 patients. The anterograde approach through percutaneous puncture of the kidney was used in 19 cases. Following dilatation, an 8 F to 24 F catheter was left in the ureter. Dilatation was successful in 13 out of 23 cases (56%) followed up for 1 month to 2 years. It failed in 8 cases, and 2 patients are undergoing ureteral remodelling. Thus, dilatation of benign ureteral stenoses after percutaneous renal puncture was effective in about 60% of the cases. The follow-up period is still too short for us to determine the indications of this technique according to the cause and duration of the stenosis.

[Urinary tract obstruction by Candida bezoars, or fungus balls].

Two patients admitted to an intensive care unit for severe bacterial infection develops Candida albicans superinfection with obstruction of the urinary canals by "fungus balls". The development of medico-surgical techniques using antibiotics and immunosuppressive drugs accounts for the increasing incidence of disseminated candidiasis. The kidneys are particularly sensitive to Candida infections. Fungus balls are conglomerates of mycelium which may result in obstruction of the urinary tract. Their clinical, radiological and ultrasonic features, as well as the microbiological and serological diagnostic problems they raise are described. Ultrasonically guided percutaneous nephrostomy is a very interesting technique for the diagnosis of the condition and its local treatment by drainage and lavage with amphotericin B. This antifungal, combined with flucytosine is still used for the systemic treatment of candidiasis.
