The results of an analysis of 13 cases showed that isolated intrathoracic metastases of synovial sarcoma may be treated surgically if there are no recurrences. Since response of metastases to preoperative chemotherapy was registered in 3 out 6 patients, it is suggested that research in this problem should be recommended. A similar approach should be used in postoperative antitumor medication of cases of newly-detected lung metastases.

A long-term experience with dissemination of tumor of different parts of the esophagus was evaluated using the results of resected material examination and post-operative follow-up. Schemes of treatment for different degree of metastatic spread of esophageal cancer are discussed. Diagnostic laparatomy is recommended prior to surgery, radiation or combination treatment of tumor of the median or lower thirds of the esophagus; laparatomy should remove paracardial lymph nodes, nodes of the lesser omentum as well as retroperitoneal nodes located along the left gastric artery, those in the vicinity of the bifurcation of gastric artery and along the upper boundary of the pancreas. If retroperitoneal lymph nodes are involved, resection of the esophagus is useless and radiation treatment may be recommended as palliation only. Cases of metastases in paracardial lymph nodes or those of the lesser omentum should be provisionally referred to a group of candidates for surgery, while the actual expediency of surgical, radiation or combined treatment of such cases is subject to further evaluation, as more data accumulate. In cases of cancer of the lower third of the esophagus, revision and surgery (radiation) should be carried out in the mediastinal lymph nodes located along the esophagus up to the bifurcation of the trachea; when the median third is involved--along the entire thoracic part of the esophagus; in cases of cancer of the upper third--along the esophagus up to the bifurcation of the trachea, including the cervicosubclavicular lymph nodes.
Cases of esophageal cancer are often admitted to oncological establishments at relatively late stages of tumor process: in 85%, the record of symptoms exceeds three months. The average duration of case histories before admission is 7.4 months. Diagnostic errors were established in the patients who had consulted the doctor during a period of less than 3 months after symptom development. This was due to the frequent appearance (41%) at initial stages of symptoms other than dysphagia which was misleading. However, even if dysphagia was apparent, primary X-ray examination failed to detect esophageal tumors in 20.1% of cases. Therefore, an endoscopic examination must be performed in cases of dysphagia. Whenever dysphagia is a consequence of esophageal burns, patients should undergo examinations at specialized oncological establishments for some years.
Extensive metastatic proliferation is rather characteristic of cancer of the lower esophagus. In persons who died due to progression of the disease metastases are revealed in 3 of 4 cases, lymphogenic metastases prevailing over hematogenic ones in the ratio of 3 : 2. Clinical investigations indicate early metastases in cancer of the lower esophagus, intraoperatively these are detected in a half of all the cases, in radical surgery--in 41.3%, in tentative surgery--in 72.8%. Unresectable metastases as frequently as tumor invasion of the adjacent organs are the cause why the radical operation was refused (1 : 1). Intraoperative diagnosis of metastatic dissemination of cancer of the lower esophagus is far from being perfect, which is evidenced by not infrequent detection at autopsy of unremoved metastases in persons, died within the terms up to 3 months following radical surgery (16.8%). Among persons, died after the resection of distal esophageal segments with the construction of esophagastroanastomosis, metastases in the pleural cavity are detected in every fifth person (21.7%), among those died following the Torek surgery--twice as frequent. Among patients who died during the period of 3 months after radiation therapy for cancer of the lower esophagus metastases in lymph nodes and abdominal organs, i.e. beyond the zone of radiation effect, were found in a half of cases.
Not infrequently cancer of the middle and lower thirds of the esophagus metastasizes into the subfrenic lymph nodes, which justifies a revisional laparotomy for completing the diagnosis of the tumor process spreading. A peduncular gastrostomy is carried out at the revisional laparotomy. A one-stage operation of extirpation and plasty of the esophagus with a stomach tube, housed in the bed of ablated esophagus, involves a great risk and is followed by a high fatality rate.

Three schemes of nonstandard combination of irradiation and surgery for esophageal cancer are presented: individual approach (IA), dynamic planning (DP), and sequential scheme (SS). In IA preoperative fractionation is adjusted to the tumor size. In DP esophageal resection is performed only in nonresponders to small-fraction 40-45 Gy radiation. In SS surgery is performed after radiation with greater than 50 Gy for residual esophageal tumor or reappearance of the tumor.
In cardioesophageal cancer the 5-year survival was 52.5% for operations on patients without regional metastasis or involvement of the serous gastric membrane. When regional metastases were present, the 5-year-survival was 23.3%. When the whole wall was invaded with the tumor but there were no metastases to the regional lymph nodes, the 5-year survival increased to 38.2%. In cases involving both invasion and metastases, the percentage of 5-year survivors was 13.3%. These findings indicate the prognostic significance of metastatic spread to regional lymph nodes. When cardioesophageal tumor invasion spreads beyond the original tumor site, combined resection yields apparently similar results. The experience of this clinic permits us to conclude that potential progress in obtaining improved long-term results for esophageal cancer management is dependent on advances in lymph node dissection methods and the search for effective combinations of systemic treatment, involving chemotherapy or immunotherapy.
