[Organization of hospital care for cancer patients].

Due to errors in the planning and development of cancer hospital system there is a gap between the present-day cancer morbidity levels and the number and profile of oncological hospital beds. Fresh approaches to cancer control planning are required. Improvement in treatment strategies is an important factor of cancer in--patient service development.

Results of the removal of malignant colonic polyps.

Two-hundred-and-twenty-six patients, aged 26 to 80, had 238 malignant polyps removed. According to the WHO classification, only those lesions in which invasive growth is detected, are termed malignant polyps. Immediately after receiving the histological report, eight patients were operated on. Later, over periods of up to 18 months following the removal of polyps, eight more patients were operated upon for recurrences. The results of follow-up over periods of up to 10 years indicate the efficiency of endoscopic removal of malignant polyps irrespective of the grade of the invasive growth. Taking their experience as a basis, the authors suggest that the plan of approach to further stages of treatment should be based not on the pathological findings in the removed polyps, but on the results of regular follow-up of the patients.

[Results of removal of malignant polyps of the large intestine].

As a result of endoscopic removal of 238 malignant polyps of the large bowel, malignant tumors in other parts of this organ were found in 6 out of 142 cases (67.7%) followed-up during terms ranging 6 months--10 years. More polyps in different parts of the bowel were found during operation in 27 cases. A procedure for follow-up of cases of malignant polyps is discussed. Endoscopic removal of malignant polyps of the large bowel should be used as a method of choice, particularly, in aged patients suffering concomitant grave diseases.

[Therapeutic tactics in colonic diverticulosis].

Based on an analysis of results of treatment of 556 patients the authors think operative measures to be indicated in cases with the perforation of diverticula, chronic peridiverticular infiltration and failure of the conservative therapy. In noncomplicated diverticulosis resection of the altered segment of the intestine should be added by serosomyotomy by an original authors' method.

[Selection of the treatment method in rectal cancer].

From 1956 to 1982, 4126 patients suffering from rectal cancer underwent surgery, 3229 of them had radical operations. The total postoperative mortality rate came up to 7.1% (n = 295) in the radical operated group it was 6.6% (n = 214). Between 1956 to 1982 continence preserving surgical procedures were constantly on the increase 1956: 46.4%, 1982: 66.8%). In 160 patients who underwent exstirpation of the rectum with final colostomy a closing magnetic device was implanted around the colostomy. 40 patients who underwent a rectal pull through had a myoplastic operation to substitute a new anal sphincter using a portion of the adductor longus femoris muscle. Preoperative X-ray therapy (2000-3000 rad in 5 days) in 217 patients with rectal carcinoma yielded no significant improvement of the 5 years survival rate.
