Cryptococcal meningitis in pregnancy: a case report.

We report a pregnant woman with cryptococcal meningitis. She experienced symptoms of meningitis before gestation which worsened during pregnancy. Combined treatment of amphotericin B and flucytosine, including ventriculo-peritoneal shunt gave a favorable outcome for both the patient and her child. We suggest that pregnant women with cryptococcal meningitis should continue their pregnancy and prefer to use combination of both drugs in standard doses because of the effectiveness, shorter duration of treatment and less toxicity than amphotericin B alone.

Tuberculous brain abscess.

We report a case of tuberculous brain abscess. He developed neurological symptoms inspite of 6 months treatment of pulmonary tuberculosis. His condition improved after surgical excision of abscess and short-course chemotherapy. Routine staining of pus is simple and a very useful technique to detect this entity.

Treatment of tuberculous meningitis with 6-month course of chemotherapy.

The results of a 6-month course of chemotherapy for treatment of 29 patients with tuberculous meningitis were analyzed. There were 7 patients in stage 1, 12 patients in stage 2 and 10 patients in stage 3. The result was recovery of 20 patients, death of 4 patients, while 4 patients were lost to the study and 1 patients dropped out from serious side effects. Three patients had residual neurological deficits.

Specificity of antibodies in cerebrospinal fluid of human cerebral gnathostomiasis cases.

Specificity of antibodies in cerebrospinal fluid (CSF) of human cerebral gnathostomiasis cases were examined by indirect fluorescent antibody technique against paraffin sections of Gnathostoma spinigerum larva. Specific greenish fluorescence was observed at cuticle, esophagus, muscle cells, intestinal cell cytoplasm and microvilli. CSF of confirmed cerebral cysticercosis cases gave fluorescence mostly at the cuticle. It is suggested that parasite-specific antigen may be present on intestinal cell microvilli and CSF would be a good source of antibodies in studying specificity of antibodies to gnathostome infections.

Bilateral facial nerve paresis in eosinophilic meningitis.

A 29-year-old man presented with headache, nuchal rigidity and bilateral facial paralysis. Lumbar puncture revealed eosinophilic CSF pleocytosis. Facial diplegia was improved by supportive treatment. Thus, eosinophilic meningitis is one of the etiologies of bilateral facial nerve paresis.

Neurogenic pulmonary edema associated with meningitis.

A 17-year-old man presented with acute febrile, obtundation, nuchal rigidity and CSF pleocytosis with polymorphonuclear cell predominate. He developed acute pulmonary edema which could not be explained by other mechanisms. Meningitis appears to be another cause of neurogenic pulmonary edema.

Group A beta-hemolytic streptococcal meningitis in adults.

Two cases of Group A beta-hemolytic streptococcal meningitis are reported. One patient presented with fever, coma, seizure and was associated with skull defect whilst the other had meningitis after surgical removal of an epidural cyst. The first died but the latter completely recovered. This organism should be considered in patients with skin wounds and bacterial meningitis due to Gram-positive cocci.

Movement disorder in hypothyroidism: a case report.

A case of primary hypothyroidism with hemichoreoathetoid movement was presented. After thyroid hormone replacement therapy for a few weeks, the movement disorder decreased dramatically. This abnormal movement may be one of the neurological manifestations of hypothyroidism.
