[Respiratory system reactions to organic aerosol exposure].

The effects of organic aerosols (coffee, teas, spices, confectionery, soy, animal feed) on the respiratory system of workers occupationally exposed to these aerosols are presented. Described are possible mechanisms responsible for the development of respiratory impairment. Our studies indicate the possibility of development of respiratory impairment in exposed workers. The prevalence of chronic respiratory symptoms and diseases varied from 2.7 to 56.3% and of acute symptoms from 9.9 to 78.9% of exposed workers. Statistically significant acute reductions of ventilatory capacity (FVC, FEV1, FEF50, FEF25) were recorded during the work shift. Described are some immunological changes which can develop in exposure to organic aerosols. Experimental studies with water organic dust extracts on isolated guinea pig trachea suggest that bronchoconstrictive changes in humans may be reproduced in non-sensitized guinea pigs. The need for taking preventive medical examinations in order to prevent development of respiratory and/or immunological impairment is stressed.

[Respiratory symptoms, diseases and ventilatory function in shoe industry workers].

The respiratory health status was studied in 376 female shoe workers. The mean age of workers was 32 years and the mean exposure 12 years. A significantly higher prevalence of all chronic respiratory symptoms and diseases was found in exposed compared to control workers. This was particularly pronounced for chest tightness (exposed: 44.7%; control: 0%), dyspnoea (exposed: 42.6%; control: 2.0%) and rhinitis (exposed: 46.3%; control: 2.4%). In exposed workers there was also a high prevalence of acute symptoms which developed during work shift, particularly irritation of the throat (56.6%), dryness of the throat (61.4%), dryness of the nose (56.4%), and eye irritation (63.6%). The prevalence of acute and chronic respiratory symptoms and diseases increased with duration of exposure. There were also statistically significant acute reductions in all ventilatory capacity tests (FVC, FEV1, FEF50, FEF25) during work shift. The measured ventilatory capacity values in exposed workers were significantly lower in comparison with the predicted values (P < 0.01). Lung function severity increased with duration of employment. The data suggest that occupational exposure to atmospheric pollution in the shoe manufacturing industry may be responsible for the development of acute and chronic respiratory impairment.

[Insurance for work-related accidents and occupational diseases].

According to Article 51 of the Health Insurance Law, a new class of compulsory insurance - employer's compulsory health insurance - should be used in practice. In applying this article of the Law, the employer is bound to provide sources for all medical and other care needed by the insured in the case of an accident at work or an occupational disease. The employer is obliged to reinsure that class of risk with the insurance companies that idemnity the costs of medical treatment if these costs incur as a consequence of the insured risk (accident at work or occupational disease). The Croatian Institute for the Health Insurance is intermediary between various medical care units, employers and insurance companies. On the examples of four different cases the paper aims at demonstrating the need for quick and efficacious collection of both insurance and medical documentation, the need for professional assessment of that documentation, and the need for closer cooperation between medical experts working with insurance companies and medical supervisors working with the Croatian Institute for Health Insurance.

[Respiratory findings in workers with no occupational exposure to air pollutants in the workplace].

The prevalence of acute and chronic respiratory symptoms and diseases as well as ventilatory capacity were studied in 806 workers without occupational exposure to air pollutants. The established prevalence of chronic respiratory symptoms was similar to that found in the general pollution. It was also higher in smokers than in non-smokers. Relatively a very small number of workers (only smokers) complained of acute symptoms during work shift. The measured ventilatory capacity values were not significantly different from the predicted values. There was a significant increase in all ventilatory capacity values during work shift (FVC, FEV1, FEF50, FEF25) which varied from +1.9 to 9.8% of the preshift values. In older workers (>40 years of age) as well as in those with longer exposure (<40 years) and those with shorter employment (<10 years). Smoking habit appears to be the major factor responsible for the development of lung impairment in workers not exposed to atmospheric pollution.

[Ventilatory function and immunologic reactions in female workers processing dried fruits and teas].

Ventilatory capacity and immunological reactions were studied in 54 female workers employed in processing dried fruits and teas, and in 40 control workers. There was higher prevalence of all chronic respiratory symptoms in the exposed compared to control workers, although the differences were statistically significant only for dyspnea and nasal catarrh (p < 0.01). The exposed workers with positive skin tests had significantly higher prevalence of dyspnea (p < 0.05) and nasal catarrh (p < 0.01) than those with negative skin tests. In exposed workers there were statistically significant acute reductions of ventilatory capacity tests during the shift, particularly in flow rates at 50% and the last 25% of vital capacity on maximum expiratory flow-volume (MEFV) curves (FEF50, FEF25). In comparison to predicted normal values, the mean FEF25 was significantly lower than normal (p < 0.01). Significantly larger number of exposed workers compared to controls reacted with positive skin tests to at least one of the occupational allergens (p < 0.01). However, only the prevalence of positive skin test to sage (60.0%) was significantly higher than in control workers (20.0%) (p < 0.01). Our data indicate that exposure to aerosols of dried fruits and teas may in some workers cause the development of respiratory symptoms and increased skin reactivity to work-related allergens, with an increase of immunoglobulin IgE.
