Treatment of acute lymphoblastic leukaemia in adults with modified BFM regimen.

Sixty-two adults with acute lymphoblastic leukaemia (ALL) were treated with a modified BFM regimen. Forty-two (70%) achieved complete remission (CR). Twenty-one percent of all evaluable patients and 32.4 percent of complete responders are in continuous complete remission (CCR) at a median follow-up of 41 months (range 24-81 months). Long-term survival was better in T- ALL (47.1%) when compared to precursor-B ALL (4.8%) (P < 0.04).

Exploratory data analysis of hyperlipidemia on the Macintosh: software tools for analysis of biochemical, clinical, and genetic variables in 1677 consecutive lipid clinic patients.

Exploratory data analysis (EDA) software facilitates unstructured, iterative open exploration of complex datasets with the aid of multiple linked graphical displays. We are investigating relationships between plasma lipoproteins and coronary artery disease by retrospective analysis of 1677 consecutive UCSF Lipid Clinic patients. Our preliminary experience is with Data Deck 3.0 although several additional software programs (JMP 2.0, Systat 5.1, Minitab 8.0, StatView 4.0) are mentioned. Lipid diagnosis (751 women and 925 men) was 22% primary hypercholesterolemia, 19% combined hyperlipidemia, 3% dysbetalipoproteinemia, 15% endogenous lipemia, 4% mixed lipemia, 5% elevated Lp(a) and 32% with no major lipid abnormality. We found the Macintosh platform (68030) to be flexible and powerful for analysis of moderate size (less than 1 Mb) clinical datasets. High resolution color monitors (1024 x 768 pixels), fast hard disks (< 18 msec) and moderate amounts of system memory (8 + Mb) facilitate exploratory analysis.

Possible role of streptokinase-induced delayed hypersensitivity and eosinophilia in pathogenesis of ankylosing spondylitis.

Present study relates to the results of investigations on 16 patients with Ankylosing spondylitis (AS). Age of the patients ranged from 16 to 44 (median 25) years. Male female ratio was 4.3:1. Sacroileitis was detected in all the patients. Seven patients each had stage 1 and 2 disease. Two other patients had stage 3 disease. Lower mean Hb per cent was detected among patients as compared to control groups. Twelve of 16 (75 per cent) patients had eosinophilia. HLA-B27 antigen was detected in 10 of 16 (62.5 per cent) patients. Streptokinase-induced delayed sensitivity was detected in 10 of 16 (62.5 per cent) patients. Anti-streptolysin 'O' antibodies were detected in 2 of 16 patients' sera. Moderate negative correlation was found between the size of induration of skin test and haemoglobin concentration (P < 0.05). Possibly, delayed hypersensitivity might induce both eosinophilia as well as anaemia among AS patients.

Impact of improved survival of very low birth weight infants on recent secular trends in the prevalence of cerebral palsy.

OBJECTIVE: To review recent secular trends in the prevalence of cerebral palsy in industrialized countries that have population-based cerebral palsy registries and to estimate such time-trends for the United States, where until recently such registries were absent. DATA SOURCES: Recent epidemiologic studies of cerebral palsy published in peer-reviewed journals in English, and US vital data bearing on the principal demographic determinants of cerebral palsy--birth rates, the birth weight distributions, birth weight-specific mortality risk, and cerebral palsy risk among survivors. RESULTS: Most epidemiologic studies from industrialized countries show a rise in the childhood prevalence of cerebral palsy in recent decades, largely because of the increasing contribution of children of low and very low birth weight to its prevalence. The only demographic determinant of cerebral palsy prevalence that is changing rapidly in the United States is survival of low birth weight and very low birth weight infants. Based on the magnitude of change in the survival of low and very low birth weight infants, it is estimated that childhood prevalence of cerebral palsy rose about 20% between 1960 and 1986 in the United States. CONCLUSION: An apparently unavoidable side effect of the increasing success of newborn intensive care is a moderate rise in the childhood prevalence of cerebral palsy.

The reliability of neonatal head circumference measurement.

Interobserver reliability in head circumference measurement was assessed in a cohort of 1105 low birthweight (less than or equal to 2000 g) infants enrolled in a study of brain hemorrhage. In 927 (83.9%) subjects, head circumference was measured both by a pediatric resident or admitting pediatrician, and by a trained ultrasound technologist. The Pearson correlation coefficient for these two sets of measurements was 0.934 (p less than 0.01), and the intraclass correlation coefficient was 0.933. By contrast, analysis of differences revealed that the limits of agreement were from -1.99 to 2.03 cm, indicating that 5% of measurements differed by 2 cm or more. Using clinicians' measurements as the gold standard, ultrasound technologists detected abnormal head circumferences with a sensitivity of 91.2%, a specificity of 97.2%, and a positive predictive value of 88.0%. For clinical purposes this level of reliability may be acceptable, but in research studies this degree of misclassification would lead to attenuation of the odds ratio.

Steroid response to adrenocorticotropin stimulation in children with human immunodeficiency virus infection.

To evaluate whether frank or subtle disorders of adrenal steroidogenesis exist in human immunodeficiency virus (HIV)-infected children, the adrenal steroid response to an iv bolus of ACTH-(1-24) (0.25 mg Cortrosyn) was determined. Ten children (six males and four females, aged 7 months to 7.5 yr) were studied. Five underwent repeat testing 3-5 months after initial assessment. Nine patients were classified as P2 or symptomatic according to the Center for Disease Control criteria for HIV infection in children. Eight had failure to thrive, six had opportunistic infections and neurological deficits, and two were receiving ketoconazole at the time of ACTH testing. Only one patient had a neonatally acquired transfusion-related HIV infection. Three of the children died 2-5 months after ACTH testing. All patients had normal or slightly elevated baseline and stimulated cortisol levels compared to the control population. The mean post-ACTH cortisol level was significantly higher than the mean post-ACTH level in the control population. No patient demonstrated an impaired aldosterone response to ACTH. The basal and ACTH-stimulated dehydroepiandrosterone levels were normal. Although individual deoxycorticosterone and corticosterone levels were variable, the mean stimulated deoxycorticosterone and corticosterone levels in the patients were suggestive of a selective defect of the 17-desoxy pathway in the adrenal fasciculata. No changes were noted in the patients' cortisol, dehydroepiandrosterone, and aldosterone responses on repeat ACTH testing. In HIV-infected children we have demonstrated that cortisol and aldosterone synthesis is intact. Thus, the chronic debilitation observed cannot be explained on the basis of adrenal insufficiency. However, a selective deficiency of 17-desoxysteroid hormone production from the adrenal fasciculata may be present.(ABSTRACT TRUNCATED AT 250 WORDS)

Silent nucleotide substitutions and G + C content of some mitochondrial and bacterial genes.

The G + C content of DNA varies widely in different organisms, especially microorganisms. This variation is accompanied by changes in the nucleotide composition of silent positions in codons. (Silent positions are defined and explained in the text). These changes are mostly neutral or near neutral, and appear to result from mutation pressure in the direction of increasing either A + T (AT pressure) or G + C (GC pressure) content. Variations in G + C content are also accompanied by substitutions at replacement positions in codons. These substitutions produce changes in the amino acid content of homologous proteins. The examples studied were genes for 13 mitochondrial proteins in five species, and A and B genes for bacterial tryptophan synthase in four species. In microorganisms, varying AT and GC mutational pressures, presumably resulting from shifts in the DNA polymerase system, exert strong effects on molecular evolution by changing the G + C content of DNA. These effects may be greater than those of random drift. The effects of GC pressure on silent substitutions in the systems examined are several times as great as the effects on replacement substitutions. GC pressure is exerted on noncoding as well as coding regions in mitochondrial DNA. This is shown by the close correlation (correlation coefficient, 0.99) of the G + C content of the noncoding D loop of mitochondria with the G + C content of silent positions in the corresponding mitochondrial genes.
