[Comparative assessment of resting myocardial perfusion in patients with postinfarct cardiosclerosis by electron-beam tomography and 99mTc-MIBI myocardial single-photon tomography].

The purpose of the study was to compare myocardial perfusion assessed by electron beam computed tomography (EBCT) with that obtained by 99mTc-sestamibi single photon emission computed tomography (SPECT) in patients with old myocardial infarction and control subjects at rest. A total of 42 patients with suspected and known ischaemic heart disease (IHD) were included in the study. 20 pts had a history of Q-wave myocardial infarction (MI), 12 pts had an old non-Q-wave MI and 10 served as controls (without perfusion defects on SPECT images at rest). Assessment of the myocardial perfusion by EBCT was performed using the short axis view and multislice mode (MSM) during injection of 50 ml of the nonionic contrast medium at 4 ml/s via cubital vein. Perfusion defects were localized by SPECT according to 6-segment model of the LV (septal, anterior, lateral, posterior, inferior and apical). Overall concordance between EBCT and SPECT was 67% for normal versus abnormal perfusion. Agreement between the 2 methods for each of the 6 segments was 81% (K = 0.62) for the anterior segment, 71% (K = 0.42) for the septal segment, 71% (K = 0.43) for the apical, 69% (K = 0.3) for the lateral segment, 48% (K = 0.13) for the posterior segment and 60% (K = -0.13) for the inferior segment. Discrepancies between the two of techniques were most notable in the posterior region. Beam hardening during passage of the contrast medium through the heart chambers and descending aorta is possible explanation of the artifacts on EBCT images. This study demonstrates that nowadays EBCT is not yet alternative to SPECT in the assessment of the myocardial perfusion in patients and further improvements of scanning techniques are necessary.

[Radionuclide diagnosis of right ventricular myocardial infarction].

The diagnostic capacities of 99mTc-pyrophosphate plane myocardial scintigraphy versus 99mTc-pyrophosphate single photon emission computed tomography (SPECT) were compared. Recording right precordial ECG leads showed that 26 patients had right ventricular myocardial infarction (MI)-typical changes as ST-segment evaluation, followed by abnormal Q wave. Plane scintigraphy indicated a characteristic inclusion of 99mTc-pyrophosphate into the right ventricular myocardium in 18.8% of the patients with acute lower MI and in one of 38 patients with acute MI of the anterior left ventricular wall. SPECT revealed a characteristic inclusion of 99mTc-pyrophosphate into the right ventricular myocardium much more frequently than did plane myocardial scintigraphy--in 34% of cases. Right ventricular myocardial inclusion of 99mTc-pyrophosphate was found in 50% of the patients with acute lower MI, including all 9 patients with positive 99mTc-pyrophosphate myocardial scintigraphy. Thus, the sensitivity of SPECT in the diagnosis of right ventricular MI is somewhat higher than that of precordial ECG and more than thrice higher than that of plane scintigraphy.

[Physical working capacity and myocardial perfusion in patients with diabetes mellitus].

A study was made of the interrelationship of myocardial perfusion with physical working capacity in different types of DM (with the duration of disease from 4 to 8 years without clinical signs of circulatory insufficiency). Rated physical exercise testing in 12 patients with insulin dependent diabetes and 27 patients with noninsulin dependent diabetes as well as in 40 healthy subjects has shown a GTT decrease in diabetes mellitus irrespective of patient's age, sex and body mass to be more marked in noninsulin dependent type and to be closely related to disturbed oxygen supply of the heart and a lowered myocardial reserve. Bicycle ergometric testing combined with 201Tl scintigraphy in 20 patients has shown disorder of perfusion in all the patients irrespective of a diabetes type and duration of disease. Insulin dependent diabetes mellitus was characterized by stable perfusion defects resulting from metabolic derangements, and noninsulin dependent diabetes mellitus was characterized by a decrease in the level of a maximum Tl uptake by the myocardium and transient perfusion defects of ischemic type.

[An intensive regimen of supersonic-frequency-current therapy in chronic inflammation of the adnexa uteri].

As shown by clinical, electrophysiological and hormonal examinations of 34 patients with chronic inflammation of the uterine appendages and control subjects, the disease treatment with intensive low-frequency current has no negative effects on cardiovascular and endocrine systems, improves function of the reproductive system. Intensive low-frequency currents are believed to be a useful modality in management of chronic genital inflammation.
