[Hartmann's procedure in the treatment of acute obstructive left-sided colonic cancer].

This retrospective study included all patients over a five-year period who underwent the Hartmann's procedure because of a malignant left-sided large bowel obstruction. Twenty-nine patients were treated with this procedure. The cumulative operative mortality was 14%, and the five-year survival was 31%. Intestinal continuity was restored in seven patients (24%). There were no anastomotic leakages. The treatment resulted in a permanent colostomy in 62% of the patients. The Hartmann's procedure can be employed as a treatment of malignant left-sided large bowel obstruction, but will cause a high frequency of permanent colostomy.

Acute Disease

Mandibular osteomas in sporadic colorectal carcinoma. A genetic marker.

Pantomography of the mandible was performed in 98 patients with sporadic colorectal adenocarcinoma. Twenty-eight patients (29%) had osteomas versus 5% in a control group (P < 0.001). Mandibular osteomas are found in most patients with the premalignant dominant syndrome familial adenomatous polyposis. Sporadic colorectal cancer examinations of married couples have shown that diet has only a moderate influence on the development of colorectal cancer, whereas pedigree studies indicate a genetic component. On this basis we conclude that mandibular osteomas are probably genetic markers of the development of sporadic colorectal carcinoma.

Adenocarcinoma

[3-layer anastomoses after resection of colorectal cancer. A prospective study of 1015 anastomoses].

This study included patients who underwent elective curative resection of adenocarcinoma of the large bowel with primary suture of the anastomosis. The overall incidence of clinical anastomotic leakage was 5%. The incidence of anastomotic leakage was significantly higher in men than in women after low anterior resection. The three-layer anastomosis has no advantage in large bowel surgery when compared with data from other sutured anastomoses reported in the literature.

Adenocarcinoma

[Retrograde colo-colic invagination].

A case of retrograde colo-colic intussusception, diagnosed preoperatively by straight films of the abdomen and confirmed by barium enema X-ray, is presented. The mechanisms behind intussusception are discussed briefly.

Aged

[The occurrence of late thromboembolic complications after elective abdominal surgery].

Forty-five consecutive patients who were submitted to elective major abdominal surgical interventions under optimal thromboembolic prophylaxis participated in an investigation as regards the occurrence of late thromboembolic complications. The patients were followed postoperatively with 125I-fibrinogen scanning every second day for a week or until discharge from hospital. If deep venous thrombosis was suspected, intravenous phlebography was carried out. Thirty-five patients completed the investigation. None of the patients developed deep venous thrombosis during thromboembolic prophylactic treatment. In two patients (6%) (95% confidence limits 1-19%), deep venous thrombosis was demonstrated after conclusion of prophylaxis, 13 and 28 days postoperatively, respectively. It is concluded that despite optimal thrombosis prophylaxis, late thromboembolic complications may occur. Prospective investigations are required to elucidate the clinical significance of late thromboembolic complications and to illustrate the value of prolonged thrombosis prophylaxis.

Adult

[Cecostomy can not be recommended as a routine method in the treatment of acute left-sided obstructive colon cancer].

During a period of 17 years, 135 patients with acute left-sided obstructive cancer of the colon were admitted to two hospitals in the County of Copenhagen. Primary decompression by means of coecostomy was performed. The plan was to perform resection of the tumour after some weeks and colocolic anastomosis and closure of the coecostomy either spontaneously or operatively after some months. The total mortality was 24%. A total of 84 patients (62%) completed the planned therapeutic course, 21 without complications. Eighteen patients survived with permanent coecostomies. The mortality in this material was similar to those in materials where other therapeutic regimes were employed. The prolonged therapeutic programme and considerable morbidity thus do not result in reduction in the mortality. The authors cannot therefore recommend primary decompressive coecostomy. This method can only be recommended as an emergency measure under local anaesthesia for patients in such poor general condition that other forms of treatment are not possible.

[Sclerotic and atrophic lichen of the genitals in boys].

Sclerotic and atrophic lichen (LSA) is a rare condition in boys. A retrospective analysis of boys who underwent circumcision for medical reasons, revealed that nine out of 59 boys had histologically verified LSA. The condition thus appears to be a relatively frequent cause of phimosis in children.

Adolescent

Appendicitis and enterobiasis in children.

The importance of Enterobius vermicularis in the pathogenesis of appendicitis is not known. The authors found Enterobius in 38 (12.5%) of 303 appendices removed from children. No evidence was obtained for a pathogenic effect of the parasite in appendicitis: Negative laparotomy was significantly more common in the infested group than in the other children. The findings support the theory that E. vermicularis can cause symptoms resembling true appendicitis viz. appendicopathia oxyurica.

Animals

Infravesical obstruction caused by a retrovesical malignant schwannoma.

A 77-year-old male presented himself with acute urinary retention, and a history of infravesical obstruction. Cystourethroscopy failed to show the obstruction. A malignant schwannoma (MS) causing impression in the bladder was removed through a laparotomy. Postoperatively there were no urologic symptoms, and the urodynamic investigations had changed to normal.

Aged

Function after lower limb amputation.

Functional ability and social dependence were investigated by personal interview of 107 lower limb amputees surviving 1-5 years postoperatively. Among eight independent variables studied by multiple regression analysis, increased age was associated unfavorably with physical ability and social dependence. Independence from social provisions preoperatively showed favorable relationships with functional capacity and postoperative dependence. Above-knee or bilateral amputation and postoperative pain were associated with reduced functional ability, but not with social dependence. No significant association was found with cause of operation or sex of the amputees. The importance of proper prosthetic fitting and pain control is emphasized.

Activities of Daily Living

Correction of respiratory acidosis and transient hypomagnesemia.

Four elderly patients with established chronic obstructive airways disease were admitted with a days' to weeks' history of increasing dyspnea. Acute respiratory acidosis was diagnosed and mechanical ventilation instituted. A few hours after initiation of treatment, arterial pH was normal in all patients, but serum magnesium concentrations decreased over the next 1-3 days. Thereafter, a spontaneous normalization was seen. It is suggested that the transient hypomagnesemia is due to correction of the acidosis per se.
