This article addresses key concepts in cost accounting. It is designed to provide dermatology nurses with a foundation and theoretical basis for understanding how costs are calculated. It addresses the primary methodology for doing cost/benefit analysis when determining wound treatment costs.
The financial costs of inpatient pressure ulcers to an acute care facility.
The variable costs for treatment of pressure ulcers in an acute care facility that is part of a health maintenance organization were examined in a three-month, retrospective study. The average age of these patients was 75.61 and the average cost of treatment per day was $80.42. The average total variable cost per patient was $1,300.37. The average variable cost for treatment of a patient admitted for an ulcer was $3,746.03, while the average variable cost for treatment of patients admitted for other reasons was $621.02.

The guidelines for treating pressure ulcers are based on understanding anatomy and physiology, type of skin injury, process of repair, factors affecting healing and stages of ulcers. All these aspects contribute to appropriate patient care and enhance healing of pressure ulcers.

Pressure ulcers are one of the most common iatrogenic illnesses in health care that affect a specific patient population--the elderly and the immobile. While the pathophysiology and etiology of these lesions are seemingly well understood, treatment modalities are still often based upon supposition. This article is divided into two major sections: Assessment and Treatment. Assessment will cover the etiology of pressure ulcers, physiology of the skin, wound healing, pathophysiology of unhealthy wounds, and the critical role assessment must play in determining treatment. The section on treatment will discuss the major categories of dressings, as well as other treatment modalities, in terms of the goals of therapy.
