Small arterial anastomoses: experimental models applicable to microneurosurgical practice.

Four main models of vascular microsurgical anastomoses in rabbits are described with reference to revascularization procedures in man. Some technical expedients designed to improve the patency rate, are outlined. This preliminary report emphasizes the value of laboratory training to achieve successful results with microneurosurgical treatment of cerebral ischemia.

[Voluntary ingestion of packages containing narcotic drugs. Diagnosis and treatment in a clinical case].

A case of intracorporeal concealment of narcotics is reported. Cocaine mixed with marihuana were the ingested drugs. Surgical treatment is, in selected cases, the safest method to remove such packages from the gastrointestinal tract also in subjects without mechanical obstruction. In fact, rupture of a packet threatens the patient's life, because of cocaine's high toxicity and lack of antidote.

Aqueductal stenosis as isolated localization involving the central nervous system in children affected by von Recklinghausen disease.

The Authors report three cases of non tumoral aqueductal stenosis associated with von Recklinghausen disease in children. Moreover, 16 similar cases collected from the literature are illustrated. The clinical features are evaluated in light of literature's data. Among all 19 cases, the median age was 19 years (range 6-46 years) and 9 patients were under the age of 13 years. In this latter group, the most represented clinical symptoms were headache and gait disturbances. As regards the neuroradiological procedures, CT scan represented the best tool in the evaluation of the ventricular size. Our own three cases underwent to ventriculo-atrial shunt procedures, followed by clinical recovery (follow-up: 3 months-5 years). In conclusion it is felt that, among all the pathological events usually described in patients affected by neurofibromatosis, aqueductal stenosis seems to belong to the natural history of this disease.

Bilateral calcified chronic subdural hematoma. Further pathogenetic and clinical consideration on the so-called "armored brain".

Calcified chronic subdural hematomas after shunting procedures for treatment of hydrocephalus have been rarely described. When the calcified wall is so large to extensively cover the surface of the cerebral hemispheres, this instance has been defined "Armored Brain". The Authors report the case of an eleven-year-old patient affected by triventricular hydrocephalus due to aqueductal stenosis, that was treated by a ventriculoatrial shunt. The post-operative course was uneventful and the patient recovered completely from the preexisting neurological deficits. Nevertheless, a CT scan performed 2 and 1/2 years later, revealed the development of a bilateral calcified chronic subdural hematoma, to such extent to configure a so-called "Armored Brain". Clinical course, pathogenesis and treatment of such kind of pathology are discussed in light of the data reported from the literature.

Spinal ossifying arachnoiditis. Case report.

A case of low dorsal spinal cord compression, due to ossifying arachnoiditis, is reported. Its relationship with a venous malformation overlying the dorsal surface of the spinal cord is mentioned. The literature is reviewed and possible etiopathogenetic factors favouring this rare condition are discussed in the light of few similar reports. The value of surgical treatment is stressed.

[Hemorrhagic pseudocysts of the spleen].

After a short introduction regarding the physiopathology of secondary spleen cysts, the authors describe a personally observed case of voluminous hemorrhagic pseudocyst. They underline the rarity of the orientation towards radical surgery, having in mind the post-splenectomy risks determined by deficiency of important spleen functions. The authors, with respect to this, underline the importance of reimplanting splenic tissue fragment into omentum, promoting to use, in selected cases, this treatment that needs, however, further clinical findings.
