[Morphometric picture of hypertrophic gastritis with consideration of the extent of folds of the fundal mucous membrane].

The results of histological and morphometric examinations of the fundal gastric mucosa of 93 resected stomachs (74 in peptic ulcer, 19 in gastric carcinoma) are presented. A special morphometric method based on the altered Hammar principle was suggested and used to develop the criteria of hypertrophy of the gastric mucosa with due account to the extent of its folds. The following histologic forms of mucous membrane hypertrophy were found: glandular hyperplasia without gastritis (4 cases), glandular hyperplasia with superficial gastritis (glandular hypertrophic superficial gastritis) (11 cases); foveolar hyperthrophic gastritis with glandular atrophy (1 case). The diagnosis may be established correctly only when the results of roentgen (or gastroscopic) data (the extent of the fold of the mucous membrane) and those of gastrobiopsy are combined.

[Gastric mucosa in esophageal cancer].

The fundal mucous membrane was examined histologically and morphometrically in 92 patients with oesophageal cancer and 5 patients with cancer of the cardial part of the stomach greatly extended into the oesophagus; the mucosa was obtained by operational biopsy. In 4.1% of the observations the mucosa was unchanged, in 30.9 per cent superficial gastritis, in 26.8% moderately marked atrophic and in 38.2% marked atrophic gastritis were found. In most patients gastritis appeared to preceed the main disease. A certain correlation was observed between the degree of atrophy of the gastric mucosa and the reduction of the secretory function, but this was observed in some of the patients only. In half of the patients with superficial gastritis achlorhydria was demonstrated.

Adenocarcinoma↗

[Unusual pseudosarcoid granulomatosis of a prostatic adenoma].

In the reported observation, numerous granulomas of the sarcoid type in the absence of sarcoidosis were found under the capsule of the removed prostate adenoma in a man of 57 who was practically normal for 8 1/2 years after the operation. Some of these granulomas were formed around adenoma glands. Also, large periglandular lymphoidcell infiltrates were found, sometimes with foci of cells similar to epithelioid ones. It is assumed that these infiltrates may precede granuloma formation. In the same sections there were rounded foci of sclerotic changes which apparently may be due to scarring of granulomas. An infectious-allergic genesis of the disease is suggested.

[New method of abdominal wall incision in autopsy].

The incision of the anterior abdominal wall proposed by the author is begun (after dissection of thoracic tissues on the median line) from processus xyphoideus sterni and led toward left regio iliacae (on the straight line or first on the left regio hypogastricae to its middle, then on liniae pararectalis) approximately till the middle of ligamentum Pouparti. Then from the lower end of this incision a transversal incision of superpubica is made at an angle, about 4--6 cm above simphysis ossium pubis, in adults 15--20 cm long. This incision provides a wide access to the organs of the small pelvis, demonstration of their changes on the spot, goes aside from median wounds or scars, and permits to penetrate from the transversal incision along the subcutaneous cellular tissue to the organs and tissues of regio subpubica.

[Changes in the intermediary glands of the gastric mucosa in peptic ulcer of different localization in morphometric interpretation].

Histologically and morphometrically (by the method of V.A. Samsonov). 80 stomachs resected for peptic ulcers were studied. Intermediary glands without atrophy were found in 16.3% of cases, atrophic changes in 51.2% of cases; in 32.5% of cases they disappeared completely and were substituted mainly by fields of intestinal metaplasia they disappeared completely and were substituted mainly by fields of intestinal metaplasia and pseudopyloric glands. In the atrophic process the number of cells increased in the gastric fossas and decreased in the glands; the gland-fossa epithelial cell index and the ratio of chief and parietal cells decreased. In the epithelial formula of intermediary gland the percentage of secondary and undifferentiated cells increased and that of parietal and particularly chief cells decreased. When the ulcers were localized in the stomach the glands were involved more frequently and extensively than in duodenal ulcers.

[Method of performing an autopsy without cutting into the thorax or dissecting the ribs].

Two variants of autopsy of cadavers without cutting of the thorax and dissection of the ribs are described. The method is principally based on the transversal cutting of the abdominal wall alongside both hypochondria, severing of the diaphragm and exposure of a complex of organs (after preliminary removal of the intestine and spleen). In the first variant of autopsy, the upper part of the complex is crossed in the thorax at the level of the upper aperture of the thoracic cavity, leaving the organs of the neck without autopsy. In the second variant, no such crossing is done and the complex is removed together with the organs of the neck and the tongue (that is, the autopsy is complete). To separate the latter, an additional transversal supraclavicular cut about 10 cm in length is made. The advantages of these variants lie in the cosmetic effect.

[Morphological characteristics of angiomyolipoma of the kidneys].

Eight cases of angiomyolipoma of the kidneys were studied. A variant of the tumour in which its vascular component consisted mostly of numerous primitive vascular fissures penetrating the smooth muscle tissue is described. In two cases, perivascular outgrowths of undifferentiated cells of apparently adventitial type were found. In most cases the mature fatty tissue of the tumour is found to have large multinucleate cells assume to be a manifestation of atypical differentiation of precursors of fatty cells. The smooth muscle tissue can be of a various degrees of maturity, and variants of angiomyolipoma may be distinguished in which the muscular component is represented either by highly a differentiated tissue, or by a immature tissue or combination of both.

[Case of pseudotuberculosis with lesions of the mesenteric lymph nodes].

The author reports for the first time on a case of pseudotuberculosis in a boy aged 9 years in the Karelian ASSR. The disease was diagnosed in the course of a histological investigation of the bioptic specimen of the mesenteric lymphatic node obtained during the operation performed in connection with suspected appendicitis. Histologically there were revealed granulomas with microabscesses therein and around them--very small foci of necrosis and necrobiosis with a positive reaction to fibrin. Granulomas, in contrast to those described in the literature, consisted predominantly of epithelioid, but not of reticular, cells. Their nuclei were poor in DNA, whereas cytoplasm--in RNA and Schick-positive material. Along the periphery of many granulomas there was revealed a zone of reticular cells with markedly pyroninophil cytoplasma (with plasmatization). In the cytoplasma of leukocytes a considerable amount of glycogen was identified.

[Epithelial formula and relationship between the chief and parietal cells of fundal gastric glands in gastric and duodenal ulcer].

The fundic glands of 143 stomachs resected in connection with peptic ulcer, including those localized in the stomach (70 observations) and in the duodenum (73 observations) were studied. The number of epithelial cells in the longitudinal section of the fundic gland in ulcer of the stomach was most often in the range of 100 to 149 (in 37 cases of 70), in duodenal ulcer--150 to 199 (in 36 observations of 73). In patients with duodenal ulcer, as distinct from those with gastric ulcer, the epithelial formula of the fundic gland was characterized by a lower content of non-differentiated and mucous neck cells and by a greater number of chief cells, the hyperpepsinogenic cellular shift (a rise of the content of chief cells up to 50% and more) in them was observed 4-5 times more often. The ratio index of chief and parietal cells in gastric ulcer (in 75.7% of cases) was usually in the range of 1--1.9 (tentative norm) and only in some cases (in 18.6% of observations) it was higher. In duodenal ulcer an elevated index was observed in 54.8% of cases.
