On the basis of guinea pig experiments and clinical findings related to 103 patients of whom 51 were operated on, the authors propose a diagnostic set which allows determination of pulmonary tuberculosis phase. The set informative value is 93.5%.

A combined ++clinico-roentgenological+ examination of 367 patients with a pleurisy syndrome was aimed at the study of a potential establishment of the etiologic diagnosis of the disease. Examination comprised the routine tests and findings of various types of biopsy, including pleural biopsy performed within the first two months from disease onset. The etiologic diagnosis of tuberculous pleurisy was most difficult in 96% of the patients who exhibited prolonged and stable maintenance of exudation or, on the contrary, very rapid regression of the pleural exudate without residual changes. Simultaneous morphologic and bacteriological examination of a pleural biopsy specimen contributed to the most efficacious and reliable diagnosis of tuberculous etiology of pleurisy.
The outbreak of cholera in the Pushkino District of the Azerbaijan SSR, caused by the penetration of Vibrio cholerae into the water of the irrigation system, is described. Altogether 2 cholera patients and 39 Vibrio carriers were detected. The etiological agent of this infection was V. cholerae eltor, serovar Ogawa, with typical phenotype characteristics. From all patients and 37 carriers virulent strains and from 2 carriers faintly virulent strains were isolated. In this outbreak family foci were clearly observed, but the transmission of infection through everyday contacts was practically of no importance. The foci with multiple cases were formed due to the action of one transmission factor: contaminated water.
The authors, having studied the long-term results of the operation of creating the renoportal venous anastomoses in patients with severe and malignant course of arterial hypertension, revealed anastomotic thrombosis in presence of hypotensive effect. Developed in the experiment and employed in 7 patients was the technique of dosaged narrowing the central left adrenal vein with the aim of achieving its thrombosis, inclusion of the collateral routes of the left adrenal blood outflow into the system of the portal vein and inactivation of aldosterone in the liver.
The analysis of observations on 36 patients is presented. Anuria complicated by acute pyelonephritis is considered as an acute surgical pathology requiring emergency operative intervention. The suggested tactics for complex examination and treatment of the patients contributed to decrease in the postoperative lethality down to 5.9%.
A technique of a dynamic cavernoscopy permitting one to conduct diagnostic and therapeutic manipulations for several months has been suggested. The given procedure was conducted in 36 patients with disseminated fibrous-cavernous pulmonary tuberculosis. An important role in defining management tactics belongs to the direct bacteriological and histological examination of the cavern contents. Local treatment by means of antituberculous drug solution instillation was administered in 29 patients. A positive effect was registered in 26 cases. The above technique can be used for the process prognosis and the treatment modification.
The author presents the method of monitoring the blood circulation in hands of seafarers examined, and infra-red radiation registration as an objective way of evaluating their fitness for work on ships which sail in the cold climate of the North of the USSR. An infra-red thermograph was used for this study.
Possibilities of roentgenologic method of examination with the use of roentgenography and tomography in the diagnosis of current forms of silicotuberculosis and tuberculosis in workers of the industries highly exposed to silicosis are analysed. ++Clinico-roentgenological+ pictures of 305 patients were studied in their dynamics for the period of 3 to 10 years. Being characterized by a benign course, silicotuberculosis is most often diagnosed in persons who left their work. A follow-up at least at 6-month intervals is of great importance in assessing its activity.
The authors have determined principal directions of a complex and successive action on the wound process taking into account its phasic character and new ideas as the dynamics of zonal disturbances of microcirculation around the gunshot wound. The use of the systemic treatment of 44 patients with gunshot wounds of soft tissues allowed the period of treatment to become 1.5 times shorter and the amount of suppurations was 4 times less.
The authors conducted experimental (130 rabbits) and clinical studies (249 persons) in gunshot wounds of the soft tissues. It was found that in 29% of cases, primary surgical debridement of the gunshot wound failed to be a radical intervention because zones of secondary necrosis of the tissues developed on days 3-4. In view of this, gunshot wounds must be examined without fail on days 3-5 to determine the indications for repeated surgical debridement. The indications for repeated surgical debridement are as follows: foci of necrotic tissues, unopened pockets, the presence of foreign bodies. Repeated surgical debridement of a gunshot wound, when indicated, reduces the number of purulent complications by 4 times and the duration of treatment by 1.5 times.
The captopril test was used to make a differential diagnosis of various types of primary aldosteronism. After captopril, there was no change in the activity of the renin-angiotensin-aldosterone system only in the group of patients with aldosterone-producing adenomas in a histological variant of "adenoma and atrophy". The findings suggest that aldosterone secretion regulation is autonomic in the adenoma unassociated with the function of the renin-angiotensin-aldosterone system only in the case of isolated adenoma with a histological variant of adenoma and atrophy. In patients with aldosterone-producing adenomas in the presence of the variant "adenoma and hyperplasia", aldosterone secretion retains sensitivity to the renin-angiotensin-aldosterone system as in patients with idiopathic hyperplasia and hypertensive disease, which indicates that it is possible to differentiate isolated "adenoma and atrophy" from hypertensive disease and idiopathic hyperplasia, despite its combination with tumor by using the captopril test.
An examination was carried out of microcirculation disorders as the main link of the pathogenetic process in the tissues surrounding a gunshot wound. Microcirculatory disorders were assessed in rabbits (924) with the help of radionuclide method (tissue radiometry and scannography) and the method of vital contact microscopy in gunshot wounds of posterior extremity soft tissues. After the injury the formation of four zones of tissue damage was revealed, with different character of microcirculatory changes in wound process dynamics. The obtained data may serve as the basis for working out zonal disorder classification and local treatment of gunshot wounds.
