Cardiac biomarkers: past, present, and future.

Accurate recognition of signs and symptoms and prompt diagnosis of myocardial infarction are essential for preserving myocardial function and saving lives. However, measurements of cardiac enzymes such as creatine kinase, lactate dehydrogenase, and their isoenzymes do not always provide accurate clinical diagnosis, particularly in patients with other concomitant diseases. Recently, alternative biomarkers of cardiac disease have been described: creatine kinase mass, cardiac troponins, and myoglobin. All cardiac biomarkers have some clinical usefulness in diagnosing acute coronary syndrome and acute myocardial infarction. Indications for use vary for each biomarker, and each has advantages and disadvantages and can be used at various times. However, the following must be considered: (1) Serial testing is essential with any biomarker. (2) None of the current biomarkers can be used to detect myocardial ischemia. (3) The decision of which biomarker to use should be based on the capabilities of the healthcare facility and the signs and symptoms of the patient.

A surgical approach to treating emphysema: lung volume reduction surgery.

Chronic obstructive pulmonary disease (COPD) is the fourth leading cause of death in Americans. In addition, COPD significantly affects the quality of life of afflicted patients. Historically, the treatment for emphysema has been primarily medical and includes medications, smoking cessation, and pulmonary rehabilitation. Recently a surgical procedure, lung volume reduction surgery (LVRS) has been reintroduced as a treatment for end-stage lung disease, primarily emphysema. In LVRS, wedge resections are performed to reduce the size of the lungs and improve alveolar functioning. This article describes the evaluation of patients who undergo LVRS and the associated pathophysiology in addition to the surgical procedure, inclusion and exclusion criteria, pre- and postoperative care, potential complications, and preliminary data associated with the procedure at one facility. Although LVRS is not a procedure for all patients with emphysema, it may significantly improve the quality of life of selected patients.

Nurses and ionizing radiation: a study of two institutions.

The results of this study revealed that the nurses studied were exposed to ionizing radiation at levels defined as safe. However, since the actual exposure level that increases health risks is unknown, it is recommended that critical care nurses take as many precautions as possible to minimize exposure that, over the long run, could have deleterious effects.
