[Vascular mechanisms of disorders of rheological properties of the blood in patients with myocardial infarction].

The contribution made by vascular factors to impairment of blood rheological properties was demonstrated on the basis of examination of 65 apparently healthy persons by using a vascular occlusion test. The vascular endothelium showed a decrease in fibrinolytic, anticoagulant, antiaggregatory activities. The new term "hemorheological tolerance to vascular occlusion" was proposed to use. A number of quantitative and qualitative indices for normal hemorheological tolerance were identified. The paper provides data on the frequency of particular mechanisms responsible for its decrease. It also emphasizes that the knowledge of the mechanisms of vascular impairments makes it possible to choose a combined drug therapy for myocardial infarction in a differential fashion.

[Changes in thrombocyte-vascular hemostasis and cyclic nucleotide levels after anti-arrhythmia therapy in patients with ischemic heart disease].

Platelet-vascular hemostasis and cyclic nucleotide levels as well as the effects of mexitil, ritmilen and prolecofen on those were examined in 58 coronary patients with various heart rhythm disorders. The patients showed considerably increased platelet activity, low prostacyclin level and high thromboxane and cyclic nucleotides levels, particularly in patients with high-grade ventricular extrasystoles. Mexitil did not essentially affect the parameters in question. Ritmilen produced a significant increase in 6-keto-PGF1 alpha and a decrease of platelet activity. In prolecofen-treated patients, platelet activity decreased significantly, cAMP and cGMP dropped while the cAMP/cGMP ratio went up; prostacyclin and thromboxane changes were not significant. This may be a mechanism of prolecofen's antiarrhythmic action.

[Clinical effectiveness and a change in the tolerance for physical loading in hypertension patients with a moderate limitation of table salt consumption].

Salt sensitivity was measured in 38 patients with first- and second-stage essential hypertension. Patients with high salt sensitivity were identified, in whom moderately limited (to 6 or 5 g daily) dietary salt consumption produced a hypotensive effect. A combination of low-sodium diet and hypotensive drugs in salt-sensitive patients allows diminished dosage of medication. Moderately-limited dietary salt uptake was shown to increase exercise tolerance in patients with essential hypertension.

[Clinico-experimental substantiation of the use of physical exercise in patients with ischemic heart disease for the prevention of arrhythmia].

In experimental conditions, exercise increased electrical stability of the heart in trained rats, promoted an improvement of heart electrical stability in hypoxic hypoxia and under the effect of dextran, which is known to worsen blood rheologic properties, and had no effect on heart electrical stability in curantyl-treated rats. In 50 postmyocardial-infarction coronary patients, exercise was conductive to a drop in erythrocyte cholesterol/phospholipid ratio and total plasma cholesterol, and a rise in alpha-cholesterol level. In 10 untrained control coronary patients, erythrocyte cholesterol/phospholipid ratio remained unchanged.

[Role of erythrocyte membrane cholesterol in disorders of their structuro-functional status in patients with ischemic heart disease].

Increased molar cholesterol/phospholipid ratios, significantly increased membrane microviscosity, reduced filterability through a 3 micron-pore filter, increased aggregation and longer disaggregation time, and occasional abnormally large cells were found in erythrocyte assays of 80 coronary patients, as compared to normal subjects. It is suggested that erythrocyte microrheologic disorders are related to excessive cellular cholesterol content in coronary patients.

[Total and segmental left ventricular ejection fraction of acute myocardial infarct patients during serial determination using 2-dimensional echocardiography].

Ultrasonic scanning of the heart was carried out on days 1, 10, 20 and 30 of uncomplicated myocardial infarction (MI) in 46 patients whose total and segmental ejection fraction (EF) (5 segments by a two-chamber projection from the apex) were then calculated. Total left-ventricular EF was moderately reduced on day 1 (48.6 +/- 1.8%) and increased significantly by day 30 (55.4 +/- 2.3%, p less than 0.01). All patients showed impaired segmental contractility in infarcted areas. Segmental EF was essentially reduced in segments 3, 4 and 5 in anterior MI, and in segments 1 and 2 in posterior MI. Recovery was associated with increasing EF in asynergic segments, that nevertheless remained diminished on day 30, as compared to unaffected areas. The rise in left-ventricular total EF is attributed to better contractility of asynergic segments.

[Potentiation of the hypotensive effect of labetalol in hypertensive patients treated to maintain sodium balance in the body].

Hypotensive activity of labetalol therapy alone or in combination with the diuretic agent oxodolin or with low-sodium diet (about 110 mmol/day) and potassium cooking salt substitute "Sanasol" (60% sodium chloride, ammonium chloride, calcium gluconate, magnesium asparaginate, etc.) was studied in 67 patients with second- or third-stage essential hypertension and 14 patients with nephrogenic hypertension. Central hemodynamic changes were assessed by means of radiocardiography and tetrapolar rheography. Combined treatment had better hypotensive effect in moderate and severe hypertension (diastolic arterial BP above 110 mm Hg). The diuretic-labetalol combination made possible a 25-45% reduction in the adrenoblocker dose. The effect of combined hypotensive treatment was similar in patients with different central hemodynamic types. Low-sodium diet with potassium substitute was well tolerated by the patients.

[Role of the cholesterol of platelet membranes in disorders of their structuro-functional condition in angina pectoris patients].

A study of 80 anginal patients showed their platelet molar cholesterol/phospholipid ratios, platelet membrane microviscosity, membrane release of calcium and fatty acids, and platelet aggregation capacity to be elevated. It is assumed that excessive platelet cholesterol has a significant effect on platelet aggregation, that is mediated by the intraplatelet processes examined, in anginal patients.

[Use of prazosin in patients with hypertension].

Clinical trials of the alpha-adreno-blocker prazosin (Pratsiol as produced by "Orion", Finland) were carried out in 42 patients with essential hypertension, stages 2-3 by WHO classification, and 3 patients with nephrogenic hypertension. Daily doses of the drug varied between 3 and 20 mg. Good hypotensive effect was noted in 51% of patients whose cardiac index had not originally exceeded 4.5 r/min/m2 and showed no basic rise under the effect of treatment, total peripheral resistance was never below 1200 din X sec X cm-5, and heart rate kept below 80 beats per minute. In cases of higher cardiac index and heart rate values, prazosin had little or no effect. A daily dose of 8-10 mg may be considered an optimum regimen. Sinus tachycardia was the most common of side-effects. The drug was particularly effective where hypertension was combined with bradycardia.
