[Beclomethasone dipropionate seasonal allergic rhinitis treatment of children. A double blind cross over study (author's transl)].

In a double-blind cross over study on 25 children with seasonal allergic rhinitis treatment with Beclomethasone dipropionate (BDP)-Aerosol was found more effective than placebo. 17 children preferred BDP, two the placebo-aerosol, while 6 had no preferrence. During a period with very high pollen concentrations in the air the effect of BDP-therapy was no longer sufficient.

[Therapy of Childhood Asthma with Beclomethasone Dipropionate (author's transl)].

In addition to the usual mangement of asthma the introduction of Beclomethasone-dipropionate aerosol can be considered as a progress in the long-term treatment of asthma, because in most cases oral and parenteral application of corticosteroids is no longer necessary. According to the results of many authors the wellknown side effects of steriod therapy have so far not been observed. We report our experiences with Beclomethasone in the treatment of 19 chronically asthmatic children.

Isolation and characterization of the DNA fraction of rat liver chromatin which binds polylysine.

The structure of eukaryotic chromatin has been investigated by isolating and analyzing the "accessible" DNA fraction of rat liver chromatin. This DNA fraction has been isolated by titrating the chromatin with the protese-resistant D isomer of polylysine to bind the "accessible" DNA sites. After removal of chromosomal proteins by digestion with pronase, all DNA not protected from attack by bound polylysine was removed by digestion with DNase. Even after exhaustive treatment with pronase and DNase approximately 30% of the chromatin DNA remains resistant to nuclease attack. Analysis of the isolated DNA shows it to be mainly double-stranded with an average size of 200-250 base pairs. The DNA is slightly A-T rich and contains both repetitive and "single-copy" nuleotide sequences. The results suggest that there are extensive regions in chromatin where the DNA is not tightly complexed with protein. Furthermore, the DNA of these regions is similar in gross properties to the DNA of the total genome.

Time course of PCDD/PCDF/PCB concentrations in breast-feeding mothers and their infants.

PCDD/PCDF/PCB concentrations were measured in samples from four mothers (at delivery and during lactation) and their infants (at birth and the end of first year of life). For two of these mothers it was the second delivery and breast-feeding period, and additional data were available from first lactation period and the first-born infant at the age of 11 to 12 months. Five of the six infants were fully breast-fed for at least 17 weeks. In four of them a distinct PCDD/PCDF/PCB accumulation was observed at the end of the first year of life: concentrations in blood fat were 1.5 to 3.6 times higher than maternal levels measured at the same time. Due to decreasing maternal body burdens during lactation, PCDD/PCDF concentrations at 11 to 12 months of life were only about half as high in the second infant as in the first one at the same age. During second pregnancy, no important change of the concentrations was observed in maternal blood.

[The protective effect of theophylline in exercise-induced asthma in childhood].

In order to study the protective effect of theophylline on exercise-induced asthma in children, we investigated the lung function of 16 children with documented bronchial asthma. Exercise provocation was performed by running on a treadmill for 6 minutes before and after two dose regimens of theophylline (10 and 20 mg/kg body weight). While for FEV1 the low dose regimen only showed a trend (p < 0.1), there was a slight but significant improvement in the high dose regimen (p < 0.05). Concerning sRaw both groups showed a small but significant improvement compared with the baseline values (p < 0.05). We conclude that in addition to its bronchodilatory effect theophylline may modulate exercise induced bronchospasm in asthmatic children.

[Carrier-bound measurement of serum theophylline levels in children].

Long time treatment of asthma in children with theophylline requires therapeutic drug monitoring by determination of serum concentrations. Beside established assays a carrier-bound reagent test has recently been developed, which has been demonstrated to be accurate, precise and cheaper than other assays. It has the advantage to give rapid results within minutes, which makes it suitable for intensive care units as well as for outpatient care.

Role of the Th2 cytokines in the development of allergen-induced airway inflammation and hyperresponsiveness.

Increased production of interleukin (IL)-4 and IL-5 by T-helper cells may be pivotal for the induction and regulation of allergic diseases. We have studied the role of IL-4 and IL-5 in the development of eosinophilic airway inflammation (AI) and airway hyperresponsiveness (AHR) in a mouse model of allergen-induced bronchial asthma. Utilizing different modes of sensitization, we delineated the importance of IL-5-mediated eosinophilic airway infiltration for the development of in vitro and in vivo AHR and demonstrated the inhibition of airway inflammation and AHR by anti-IL-5 antibody treatment. Studies in IL-4- and IL-5 deficient mice revealed the importance of both cytokines for the induction of AI and AHR independently from the production of allergen-specific IgE, and indicated these cytokines as potential targets in novel approaches in the treatment of asthma.

IgE antibodies to tetanus toxoid in relation to atopy.

In approximately 25% of subjects receiving tetanus booster immunization (4 study groups A-D with n = 6, 25, 50 and 240, respectively; age 18-40 years for groups A-C, age 2 for group D), substantial IgE antibody levels were found. In groups A and B, sequential serum samples were available for analysis. The IgE response peaked before the IgG response and was often higher at week 1 than at week 2. The 6 subjects in group A (military recruits) received 2 booster injections. At the time of the second injection, 2 subjects had a very high IgE antibody titre (as well as a high IgG antibody titre), but no signs of an allergic reaction were noted. A significant association with atopy-related factors was found, but many apparently non-atopic subjects developed IgE antibodies.

Platelet-activating factor inhibits ciliary beat frequency of human bronchial epithelial cells.

The aim of our study was to investigate the effect of platelet-activating factor on human bronchial ciliary beat frequency (CBF) ex vivo. Brush biopsies were obtained from eight children (mean age 6.6 years) who underwent bronchoscopy for clinical reasons (i.e., foreign body aspiration, chronic cough, stridor). Immediate measurement of CBF by phase-contrast microscopy and photoelectric technique was performed before further treatment, and then on three subsamples: after adding platelet-activating factor (PAF) (10(-5) M); after adding the inhibitor WEB 2086 (10(-4) M); and after adding both PAF and WEB 2086. Addition of 10(-5) M PAF reduced mean CBF by 2.1 Hz to 9.2 Hz (p < 0.05). The inhibitory effect of PAF was completely reversed by the addition of the PAF-inhibitor WEB 2086, whereas WEB 2086 alone did not significantly influence CBF. Optimum PAF concentration had been determined in previous dose-response experiments. Our data indicates that PAF involved in airway inflammation contributes to a decrease in mucociliary clearance in humans.

A comparative study of the efficacy and safety of loratadine syrup and terfenadine suspension in the treatment of 3- to 6-year-old children with seasonal allergic rhinitis.

The efficacy and safety of loratadine and terfenadine in the treatment of 3- to 6-year-old children with seasonal allergic rhinitis were compared in a third-party-blind, randomized, parallel-group study. A total of 96 children were included in the efficacy analysis: 49 children received 5 or 10 mg of loratadine once daily, and 47 received 15 mg of terfenadine twice daily, for 14 days. The mean total score for both nasal and non-nasal symptoms was decreased significantly from baseline at days 3, 7, and 14 in both treatment groups. At endpoint, these scores had improved 73% in each group. There were no statistically significant differences between the two groups in the total symptom scores at any point during the study. Both treatments were effective in relieving individual nasal and nonnasal symptoms. Therapeutic response to treatment was good or excellent in 82% of loratadine-treated children and in 60% of terfenadine-treated children. Few adverse events were reported during the study; all were mild or moderate and were not significantly different between the two treatment groups. There were no reports of sedation or dry mouth in either group. Once-daily treatment with 5 or 10 mg of loratadine was as effective as twice-daily treatment with 15 mg of terfenadine in improving the symptoms of seasonal allergic rhinitis in children 3 to 6 years old. Both treatments were well tolerated.
