Hemoconcentration and pre-eclampsia.

We measured erythrocyte aggregation and hematocrit in 155 uncomplicated pregnancies and in 55 pregnant women with pre-eclampsia. In the later group we found increased erythrocyte aggregation and a higher hematocrit than in normal patients of comparable gestational age. Erythrocyte aggregation correlated well with plasma fibrinogen levels (r = 0.74) but other high molecular weight proteins such as alpha-2-macroglobulin, IgM and lipoproteins were not measured. Hematocrit showed correlation with mean arterial blood pressure (r = 0.69) and peripheral vascular resistance (r = 0.73). The possible effects of changes in hematocrit and erythrocyte aggregation on blood flow through the intervillous circulation are discussed as are the clinical implications of our findings.

[Flow properties of blood in women on oral contraceptives (author's transl)].

Erythrocyte deformability, whole-blood viscosity, haematocrit as well as fibrinogen and plasminogen concentrations were checked in 39 clinically intact women of whom 22 had been on oral contraceptives for upto five years. Also determined were several serum proteins, such as transferrin, coeruloplasmin, alpha-2 macroglobulin, alpha-1-trypsin, beta-lipoprotein, and inactivated complement component C3. -Seventeen women with intra-uterine devices were chosen to form a control group. Proteins with sensitivity to oestrogen as well as whole-blood viscosity were found to be increased whereas erythrocyte deformability was decreased. Yet, no haemodynamic manifestation took place ot those rheological parameters, since haematocrit declined or remained constant, and no change occurred to fibrinogen and plasminogen concentrations.

[High molecular weight albumins in normal and pathological pregnancies and under tocolytic therapy. Serum concentrations of beta-1-SP-1, IgM and alpha-2-macroglobulin].

Pregnancy protein beta-1-glycoprotein levels were determined in serum obtained from 100 women with undisturbed pregnancy and 35 women with high risk pregnancy. Concentrations lower than normal, i.e. below the 1s range, were found only in preeclampsia and in pregnancies with intrauterine growth retardation. Antepartum CTG scoring showed reasonable correlation with the SP-1 values. Due to its long half life time and its relation to the trophoblast, the diagnostic value of SP-1 in cases of placental insufficiency is limited.

[Paroxysmal nocturnal haemoglobinuria (PNH) and pregnancy (author's transl)].

A case report concerning paroxysmal nocturnal hemoglobinuria is presented. The role of hemorheologic factors in perfusion of the placenta is clearly demonstrated. Fetal monitoring and the application of hypervolemic hemodilution enabled prolongation of the pregnancy until a healthy neonate could be delivered by Ceasarean section at the 34th week of gestation.

[Comparative examinations of high-molecular maternal serum protein bodies during pregnancy--a contribution to biochemical pregnancy control (author's transl)].

The maternal Serum concentrations of beta 1 SP 1, IgM and alpha 2-macroglobulin were determined in relation to the gestation age, using the simple radial immunodiffusion method in 102 non-pathologic pregnancies and 35 pregnancies involving risk factors. To assess the clinical relevance of these determinations, we examined to what extent the changes in concentration of beta 1 SP 1 IgM and alpha 2-macroglobulin would permit a prognostically useful conclusion on the placenta function and foetal condition. All patients with lowered beta 1 SP 1-serum concentrations were examined for their antepartua CTG-evaluation, as well as the type of termination of parturition. The normal distribution for beta 1 SP 1 showed a continuous rise in serum concentration up to the 37th pregnancy week. During the last 3 weeks, beta 1 SP 1 remained almost constant. For the cases with EPH-gestosis and placenta insufficiency, a beta 1 SP 1-concentration below the normal distribution level was found in the large majority of all cases. In diabetes mellitus during gravidity, twin gravidity and MHN, the determination of beta 1 SP 1 is not of any decisive prognostic significance. The maternal serum levels of IgM showed no significant differences when comparing normal pregnancy and risk pregnancy. The serum concentration of alpha 2-macroglobulin increased in both groups of patients with increasing gestation age. Of the three examined protein bodies, we consider beta 1 SP 1 to be a good, additional parameter for the assessment of the trophoblast function.

Contribution to the standardization of influenza neuraminidase inhibition tests, based on enzyme- antienzyme kinetic studies.

1. From enzyme-antienzyme kinetic studies a method for the determination of anti-n2-neuraminidase antibodies has been developed. This procedure is applicable in routine diagnosis. 2. Considering the present conditions, the reversibility of the virus neuraminidase antigen-antineuraminidase antibody complex has been proved. The inhibition must be described as a non-competitive one. 3. As a reference value for comparative immunity estimations the relative antibody concentration T150 is defined as the point of reciprocal serum dilution, where under a given substrate concentration the inhibited enzyme activity vi is half of the non-inhibited enzyme activity vo. 4. For the calculation of T150 a simple formula is derived from the Michaelis-Menten kinetic. Under present conditions the value of T150 is in a range of 1.5 log 2 dilution steps plus or minus the specific serum dilution T150 uninfluenced by possible variable parameters such as enzyme activity, substrate concentration and inhibitor concentration.

Different mechanisms underlying reduced beta 2-adrenoceptor responsiveness in lymphocytes from neonates and old subjects.

To investigate the mechanism underlying age-dependent changes in beta-adrenoceptor function we have determined beta 2-adrenoceptor density (by (+/-)-125iodocyanopindolol (ICYP) binding) and beta 2-responsiveness (cyclic AMP responses to isoprenaline stimulation) in lymphocytes derived from 20 neonates, 54 young adults (19-30 years) and 15 old subjects (60-86 years). In young adults the mean number of lymphocyte beta 2-adrenoceptors amounted to 862 +/- 36 (range 500-1560) ICYP binding sites/cell (N = 54); it was slightly higher in old subjects with 1230 +/- 94 (698-1980) ICYP binding sites/cell (N = 15). In contrast, lymphocytes derived from neonatal blood had a significantly lower mean beta 2-adrenoceptor number (385 +/- 35 (130-608) ICYP binding sites/cell, N = 20, P less than 0.01). (-)-Isoprenaline (0.01-100 microM)-induced increases in lymphocyte cyclic AMP content were significantly lower in neonates and old subjects than in young adults. While for neonates and young adults significant positive correlations between beta 2-adrenoceptor density and 10 microM (-)-isoprenaline-induced cyclic AMP increases exist, in old subjects cyclic AMP increases were much lower than could be expected from the beta 2-adrenoceptor number. It is concluded that the mechanism underlying reduced beta 2-adrenoceptor responsiveness in neonates and old subjects is different: while in neonates it seems to be due to the reduced beta-adrenoceptor number, in old subjects it is caused by a post-receptor defect--presumably by a decreased activity of the adenylate cyclase.

[Chorioamniotic infections following diagnostic amniocentesis in the 2d trimester].

Between 1978 and 1984 we performed 3 122 amniocentesis for prenatal diagnosis in second trimester. Based on a four week follow-up in more than 96% of all amniocentesis the percentage of fetal loss was 0.74% (n = 23). In 13 cases abortion was accompanied by chorioamnionitis. These cases were examined for various technical details and it could be concluded, that the incidence of febrile abortion is related to the early gestational age for amniocentesis. On the other hand, multiple insertions or the localisation of placenta both show no relation to the short-time febrile complications. To investigate the maternal-fetal transfer of mezlocillin, a single dose of 5 g mezlocillin was administered intravenously in 25 patients. In amniotic fluid mezlocillin levels exceeded 2 micrograms/ml nearly 120 minutes after administration. These pharmacokinetic data suggest, that a prophylactic regimen with antibiotics in the course of amniocentesis seems not to be beneficial for the prevention of chorio-amniotic infections.

[Hemodynamic changes during tocolysis with hexoprenaline and fenoterol].

The authors examined two groups of 50 patients each who had been given an average dose of 0.24 microgram/min Hexoprenaline or respectively 1.8 micrograms/min Fenoterol as beta-mimetics for premature labor. The patients who received Fenoterol were also given Verapamil as additional medication in a ratio of 1 : 40. In addition to blood pressure, heart rate, urine elimination and serum or urine osmolarity, the hemodynamic parameters stroke volume, cardiac output and total peripheral resistance were investigated. Stroke volume was measured by means of impedance cardiography. Giving equipotent doses, Hexoprenaline/Fenoterol dose ratio of 1 : 8, there was a slower and smaller increase in heart rate over 24 hours with Hexoprenaline, with identical suppression of uterine contractility (external tocography). This was also reflected in the cardiac output, while there were no differences in the stroke volumes. Inhibition of diuresis was lower with Hexoprenaline. This might be due to the fact that since there is no organ specificity for beta-1 or respectively beta-2 receptors and the patterns of distribution differ, Hexoprenaline has a somewhat less pronounced action on beta-1 receptors than Fenoterol, though the same beta-2 action.
