[Undulating course of precocious puberty].

The normal course of precocious puberty is progredient resulting eventually in full sexual maturation. In few cases a transient form of the disease has been recorded, characterized by a temporary appearance of signs of sexual maturation. We report on a female patient with an undulating course of the disorder: after precocious initiation of puberty, a temporary interruption and subsequent resumption of sexual maturation occurred. Presently, the patient is successfully treated with an LHRH-analogon. Within the framework of this case problems in diagnosis and treatment of precocious puberty are being discussed.

Late changes in aorto-renal vein transplants.

The results of 152 renal artery reconstructions in 128 hypertensive patients were reviewed 1 to 15 years postoperatively in 89 of 98 survivors. Forty-seven (52.8%) patients were normotensive, 29 (32.6%) are improved, and 13 (14.6%) were not improved. Angiography was performed 1 to 11 years after the operation. Only 2 of the 31 vein implants showed an adequate lumen. Four vein grafts were occluded. In 2 out of 4 stenosed transplants stenosis was functionally significant, and the patients with recurrent hypertension underwent re-reconstruction. The causes of the stenoses were severe constriction by scar tissue and extensive subintimal cellular proliferation of the vein graft. Twelve of 31 vein bypass grafts were dilated by an average of 22%. The average increase in caliber of the remaining 9 vein grafts was 106%. The possible contributory factors were forceful distension, excessive adventitial dissection, inadequate storage of the vein, ischemic damage of the vein wall (especially in younger patients), and the increase flow rate through the transplant.

The aging human cerebral cortex: a stereological characterization of changes in the capillary net.

Capillaries in the cerebral cortex of six age groups, ranging between 19 and 94 years (= 34 human brains) were stereologically investigated. Capillary parameters such as diameter, volume fraction, specific surface area, mean intercapillary distances and total length per unit cortex volume in patients older than 75 years were similar to those in young ones, 19 to 44 years old. Aged subjects between 64 and 74 years revealed increased capillary diameter, volume and total length per unit cortex volume, as well as decreased specific surface area. Frequency distributions in the same age indicate a real increase of capillary diameter and volume, as also an increase of these parameters on the level of blood vessels with diamteres greater than 8 micrometer, probably arterioles. The results of this study argue against the wide-spread assumption of a decreased blood supply in the cerebral cortex during old age. On the contrary, the capillary net is able to respond to changed metabolism and blood pressure.

[Local recurrences in keratoid squamous cell carcinomas of the oral cavity: incidence, causes and therapy after a curative primary treatment (author's transl)].

The frequency of local recurrences in keratoid squamous cell carcinomas of the oral cavity is analyzed in view of the following criteria: Irradiation technique, volume of the primary tumor, irradiation parameters considering the factor of dose and time. The values of the dose, measured by ret, were calculated using the Ellis-formula modified by Gabriel-Jürgens et al. A focal dose of 1,700 ret is recommended for Stage T1 carcinomas of the tongue, a dose of 1,900 ret for the stages T2 and T3. The importance of the concept of nominal standard dose is emphasized with regard to its clinical application. Surgery seems to be superior to the renewal of radiation therapy in the treatment of recurrences.

[Three dimensional optimization method for the interstitional afterloading therapy (author's transl)].

The authors present a new method for the interstitial therapy working with an Ir-192 source which is displaced step by step. After having been fed with the tumor parameters, the computer calculates for every stopping point the duration of stay of the source which provides a threedimensional optimization. The practical application of the method is discussed and the optimization procedure is compared with other similar methods.

Sensitization of leukocytes of cancer patients against fetal antigens: leukocyte migration studies.

Hypertonic extracts from human fetuses (10--22 wk of gestation) were used to test the sensitizaton of leukocytes from cancer patients against fetal antigens in a direct, microcapillary tube assay system. Leukocytes were simultaneously exposed to a panel of allogeneic tumor extracts and a panel of fetal extracts. Leukocytes from 24 gastric cancer patients, 43 colorectal cancer patients, and 13 lung cancer patients were assayed with extracts obtained from gastric, colorectal, and oat cell carcinomas, respectively, and these extracts were also used with leukocytes from 41 patients bearing tumors of various other organs. Significant migration inhibition by tumor extracts was observed in 81.6% of the tests with gastric cancer, 67.4% of the tests with colorectal cancer, 69.0% of the tests with lung cancer, and 51.2% of the tests with other types of cancer. With fetal extracts, significant migration inhibition occurred in 58.3, 58.7, 59.6, and 54.9% of the tests, respectively. Reactivity against fetal extracts did not depend on the gestation age of the fetuses used for extraction. The conclusion was reached that the leukocytes of most of the cancer patients were sensitized against substances contained in fetal extracts irrespective of the type of tumor of the leukocyte donor. The cross-reactivity pattern suggested that 3-M KCl extracts of whole human fetuses contained a complex mixture of specificities related to the various fetal organs and tissues, which may have represented counterparts to most of the tumor-associated specificities.

Relationship between curative radiation therapy of paravertebral tumors and the incidence of radiation myelitis.

The literature about radiation damage to the spinal cord is reviewed and 4 of own cases are presented. It is pointed out that in curative radiation therapy the risk of myelitis that accompanies higher spinal cord doses must be weighed against the therapeutic gains. However, in view of the lack of information about radiation damage to the spinal cord, this may be quite difficult.

[Stereological investigation in the cerebral cortex of aging subjects (author's transl)].

Involving cortical regions, capillaries of the human cerebrum of two 19 and 27 years old men, a 69 years old woman and a 72 years old man were stereologically investigated by optical-electronic image-analysis. The cortical capillary net work was demonstrated by the alkaline phosphatase activity. Each cortex region comprised a determination of the stereological parameters diameter, projected area, specific surface area, capillary distances in linear direction of TV-lines and total length per unit cortex volume. A comparison between different cortex regions revealed a good correlation between increased values of the diameter and the projected area, a decreased specific surface area and diminished capillary distances, which entail a shortened distance of oxygen diffusion through the cortical tissue. During aging a diminished capillary surface area, which results from increased values of diameter and projected area is compensated by shortened capillary distances. Presumably an augmented capillary length is due to a condensation of the capillary net per unit cortex tissue. The behaviour of the registered stereological parameters seems to be an accommodation of the capillary net in the human cerebrum to metabolic and circulatory changes during aging.

[Determination of the severity of mitral stenosis by hemodynamic and echocardiographic parameters].

M-Mode and two-dimensional echocardiographic examinations were performed in 70 patients with pure or prevailing mitral stenosis. Mitral valve excursion, mitral valve opening area (MVOAe) and diastolic E-F slope were determined and compared with the gradient across the valve and the opening area obtained during cardiac catheterization. Mitral valve excursion and E-F slope showed mean values of 1.71 cm and 1.98 cm/sec. respectively and were indicative of a stenosed mitral valve. Correlation between E-F slope and gradient with MVOAe was poor. The correlation coefficient was r = +0.56 and r = 0.34 resp. MVOAe compared favorably to the mitral valve area determined at cardiac catheterization (r = + 0.96) and the gradient across the mitral valve (r = 0.90)9 We conclude: 1. Determination of the mitral valve opening area by means of two-dimensional echocardiography represents a valuable addition in the assessment of the severity of mitral stenosis. 2. M Mode echocardiography indicates the presence, but not the severity of mitral stenosis. 3. Computerized planimetry is superior to the manually planimetered opening area and represents a reproducible, exact and time-saving procedure.

[Leukocyte migration test and determination of carcinoembryonic antigens in the postoperative follow-up care of colon and rectum carcinoma].

Preoperative and serial postoperative CEA determinations have become widely accepted tools to aid the early detection of recurrent colorectal cancer. The panel modification of the direct leukocyte migration test (LMT) has proved to be an excellent monitor in these tumor patients. In this study LMT and CEA-RIA were compared. Peripheral blood leukocytes were pulsed with a high dose (2.5 and 0.5 mg/ml) of 3 m KCl extracts of five different colorectal tumors as well as with one 3m KCL extract of normal colonic mucosa. Patients showing a pathologic migration index (less than 0.80 and greater than 1.17) of 3 or more out of five tumor extracts were considered to be "positives." With this test mode 51 of 59 (86%) patients with resectable tumors were reactive, while only 24 of 69 (41%) yielded elevated CEA values. Both LMT and CEA-RIA were positive in 12 of the 13 patients with nonresectable tumors. After curative surgery most patients' leukocyte migration and CEA values were in tne normal range. Of the nine patients with localized tumor recurrence, eight were LMT reactive and five showed elevated CEA values. Of five patients with distant metastases, four demonstrated a positive reaction in the LMT and five a rise in CEA levels. The combination of the LMT with the CEA-RIA promises to improve the detection of recurrent colorectal cancer before the appearance of clinical evidence of recurrence.
