[Tumor form of lymphostatic papillomatosis of the skin].

The case of a patient with two plantar papillomatous tumours that had had a relapsing course over 10 years is presented. The tumours were located on the right forefoot and the left great toe. Several operations were performed for different diagnoses, i.e. plantar verrucae, verrucous carcinoma and pyoderma vegetans. Histologically, the papillomas proved to be pseudocarcinomatous hyperplasias with multiple ectatic dermal lymph plexuses. Indirect lymphography revealed a circumscribed lymphatic dysplasia with cystic-ectatic cutaneous vessels within the tumour area. Draining lymphatic collectors were conspicuously reduced in number. Isotope lymphography showed a normal lymph transport rate from the foot to the groin. It is concluded that the circumscribed disturbance of lymphatic drainage is the decisive cause of the reactive papillomatous hyperplasia. Thus, these papillomas clinically resembling verrucous carcinoma represent the tumour variant of a papillomatosis cutis lymphostatica.

[Pagetoid reticulosis of the Ketron-Goodman type].

The case of a 79-year-old woman with widespread pagetoid reticulosis (Ketron-Goodman type) is presented. The nature of the epidermal cellular infiltrate in pagetoid reticulosis of the Ketron-Goodman type was examined by immunohistochemical methods and by electron microscopy. On ultrastructural examination the infiltrating cells proved to be medium-sized and large Sézary-type and blastoid cells. Immunohistochemically, the epidermal infiltrate was composed of approximately 80% immature T cells (OKT 16) and 30% proliferating cells (Ki 67). Mature T cells (CD 5) were found to account for only 15-30% of all cells in the infiltrate. These results suggest that pagetoid reticulosis (Ketron-Goodman type) is a malignant T-cell lymphoma, which is characterized, in addition to the epidermotropic behavior common to all T-cell lymphomas, by innidiation and in-situ spread within the epidermis.

[Isotype lymphography findings in sclerotherapy, varicose exeresis and excision of autologous venous transplants for arterial reconstructions].

In an isotope lymphography study on 24 patients (38 legs), the influence of complete sclerosation therapy of trunk varicosis of the great and small saphenous veins according to Tournay is examined before and, on average, four weeks after treatment. The isotope lymphography parameter was defined as the ratio of the outflow from the foot to the influx to the groin area (quotient Q). The results showed no significant change in Q after treatment compared with the starting values. Thus, sclerosation therapy did not result in any impairment of lymphatic transport. In contrast, of the 9 patients (12 legs) treated by varicose exeresis who were referred for isotope lymphography there were 8 cases (i.e. 9 legs) of impaired lymphatic transport. However, this was a pre-selected patient group. Three further patients were examined by isotope lymphography after resection of a saphenous segment for arterial reconstruction. These revealed excessive lymphatic transport disturbances which were quantified for the first time. Vascular surgeons should include this complication in their explanatory talks with patients.

[Plaque-formed cutaneous mucinosis with telangiectasis].

We report the case of a 71-year-old man with a combination of circumscribed euthyroid myxodermia and clinically visible telangiectasias. We excluded the possibility of a sympathetic lesion being the cause of the telangiectasias by using a new method, transcutaneous PO2 measurement. Different possible reasons for the simultaneous occurrence of mucinosis and telangiectasias are discussed.

[Controlled study of increasing venous tone in primary varicose veins by oral administration of Ruscus aculeatus and trimethylhespiridinchalcone].

In a randomized double blind study, 50 patients suffering from trunk or branch varicosis were orally treated with either a commercial preparation of Ruscus extract, trimethyl hesperidine chalcone and ascorbic acid (Phlebodril), or a placebo over 2 weeks. Changes of the venous tonus were measured by means of venous occlusion plethysmography in rest position (venous capacity, venous distensibility) as well as during (active) exercise (expelled blood volume). All parameters showed a tendency towards improvement in the verum group. The changes were partly significant (p less than 0.5). The course of the study suggests that the period of treatment possibly was too short to obtain the full pharmacologic effect.

Diagnostic assessment of diabetic microangiopathy by tcPO2 stimulation tests.

Transcutaneous PO2 measurements at 37 degrees C can recognize and quantify diabetic microangiopathy if suitable stimulation tests are used. The following parameters are of practical importance: resting PO2, postocclusive PO2, rubefacient-induced PO2 and 45 degrees C heating PO2. Patients with diabetic microangiopathy show diminished PO2 values in rubefacient-induced PO2 and 45 degrees C heating PO2. The former ist probably related to endothelial-cell function disturbances and the letter to organic vessel wall changes. Thus, a quantitative and differentiated picture of microangiopathy can be obtained in each individual patient.

Metabolism of [14C]arachidonic acid by polymorphonuclear leukocytes in patients with psoriasis.

The formation of LTB4 and its omega-oxidation products 20-hydroxy- and 20-carboxy-LTB4 from exogenous [14C]arachidonic acid (AA) by neutrophils from 12 psoriatic patients and 10 healthy controls was investigated. Only a slight difference was detected in the mean amount of [14C]LTB4 produced. In contrast, the amounts of [14C]omega-oxidation products obtained from psoriatic PMN were 2.4-fold higher than the amounts from PMN of healthy controls. We conclude that in vitro, psoriatic PMN synthesize more LTB4 from exogenous AA than do PMN of healthy individuals and due to an efficient omega-oxidation system, the net release of LTB4 in both groups appears to be similar.

[Results of studies in chronic urticaria with special reference to nutritional factors].

In a clinical study 90 patients with chronic or chronic relapsing urticaria underwent a standardized diagnostic programme. In 61% of the cases no causing factors were to be found. In the remaining 39% the most common causes were represented by intolerance reactions (16%), infection causes (16%) and physical causes (13%). To some extent multiple causes were present. In the case of substances intolerance causing, it was a matter of medicine (acetylsalicylic acid and other analgetics) in three quarters (12%) and of food additives (benzoate, sorbic acid, tartrate yellow and sunset yellow) in one quarter (4%). There was not a case in which food itself was a cause, neither in the sense of intolerance reaction nor in the sense of a true allergy. In 13% of the patients, who became symptom-free during the elimination diet, no causes whatsoever could be found neither in oral provocation tests nor in provocation diets. At present we are still in the dark if impurities in foodstuffs (antibiotics, yeasts, moulds etc.) could play a role in these cases.

[A case of skin myiasis caused by Dermatobia hominis].

A case of erysipelas-like symptoms in a 26-year-old female student of medicine having returned from Nicaragua to Germany is reported. On the removal of the scurf covering the supposed entrance of the erysipelas, a larva of Dermatobia hominis, the human bot fly, was extracted from the head skin, and the inflammation completely disappeared within a short period of time.

[Biological behavior of epithelioma cuniculatum].

E. cuniculatum is a low-grade squamous cell carcinoma belonging to the group of Ackerman's verrucous carcinoma. The papillomatous, cauliflower-like tumor slowly grows over many years without forming metastases. Histology reveals deeply invaginated proliferations with organoid epidermal organization and central parakeratotic horny crypts. Only the basal and suprabasal cell layers show histological features of malignancy; the basal membrane, however, remains intact. Radiation therapy as well as repeated operations may provoke anaplastic transformation into metastases. We discuss the therapy and differential diagnosis of these tumors and report on two own cases.

[Ackerman carcinoma of the lower extremity--course, pathology, therapy].

Two cases of verrucous carcinoma (Ackerman) of the lower extremity are reported. This is a low-grade malignant and slow-growing variant of squamous cell carcinoma. One tumor existed for at least 15 years, the other for 3 years, both without metastasizing. These cauliflower-like lesions are more exophytic than endophytic and are covered with foul-smelling keratinous material, which can also be pressed out of the crypts. On histological examination, deep invaginated epidermal proliferations can be seen, with a high degree of keratinization leading to central parakeratotic horn crypts. Only the basal and suprabasal cell layers show nuclear atypicality and loss of polarity of the basal cells, but the basal membrane is intact. The therapy of choice is local excision. The tumor must be totally removed on the first attempt. Radiation therapy and repeated operations favor anaplastic transformation and may provoke iatrogenic metastasizing.

[Paradoxical constellations in Doppler sonographic reflux diagnosis].

What we call a paradoxical phenomenon means the constellation of negative Valsalva proximally and positive Valsalva distally in the course of a single venous stem or two successive veins. The explanation of this phenomenon is of fundamental methodological importance, because the existence of true paradoxical refluxes would call into question the reliability of the Doppler technique. We have found five paradoxical phenomena in certain regions of the leg. By means of systematic, superficial and deep Doppler examination, it is possible to explain these phenomena by eight different reflux types. The existence of refluxes coming from extrapelvic branches of the internal iliac vein has been unknown till now, because they are not recognizable by way of routine venographies. Our findings emphasize the high value of the Doppler technique regarding clarification and explanation of the connections between functionally disturbed veins.

[Current therapeutic guidelines and differential diagnosis of venous leg ulcers].

In the majority of patients with venous ulcers, there is intrafascial venous insufficiency as well as varicosis. That is why it is important to determine the value of both factors to treat the ulcer. It is recommended that the patients be classified into four therapeutic groups, which is based on the progress that has been made in noninvasive angiological diagnostics. Refluxes in the deep leg veins can be detected by Doppler ultrasound. Determination of venous pressure and light reflection rheography show whether it is possible to improve chronic venous insufficiency by the elimination of varicosis. The differential treatment of varicosis, compression therapy, topical external medication and the manner in which patients must live are explained in detail. Most of the methods are doubtful or even impossible if there is simultaneous arterial occlusive disease. Special guidelines for the management of mixed venous arterial ulcers are explained.
