[Blood loss in dialysis in repeatedly used capillary dialysators].

The blood losses increase to the twofold to threefold by capillary dialysators C-DAK 4 and C-DAK 5 are about 6.8 ml and 26 ml, respectively, in the first use. In repeated use of the C-DAK 4 the blood losses by the C-DAK 5, however, are so high that it is not to be advised to use them manifold under our conditions of dialysis. By an increase of the reperfusion quantity from 150 to 250 ml of electrolyte solution for one C-DAK 4 blood losses may considerably be reduced. In a parallel switching of two C-DAK 4 each exemplar should be perfused individually.

[Blood coagulation disorders in liver cirrhosis in relation to the degree of portal hypertension].

Investigated were the haemostasis of 45 patients with histologically confirmed liver cirrhosis. The patients were subdivided into three groups according to the extent of their porto caval collateral circulation as proved by laparoscopy, gastroscopy and radiology: I = no porto caval shunts (n = 10); II = moderate porto caval shunts (n = 13); III = distinct porto caval shunts (n = 14). A 4th group consisted of 8 patients with bleeding from oesophageal varices. The results indicated a significant decrease in the stages I-III of the coagulation factors produced in the liver (incl. factor XIII and AT III) and the thrombocytes. Unchanged remained the concentration of factor VIII, whereas the factor VIII associated antigen showed an increased activity depending on the severity of the disease (stages I-III). In patients with bleeding from oesophageal varices, values of about 300% of normal could be demonstrated. Depending on the stage of the porto caval collateral circulation, the concentration of fibrin(ogen) split products were also increased. For comparison, patients with pre- and posthepatic blockage were investigated, whose portal hypertension was not caused by liver cirrhosis. Besides a mild thrombopenia they only showed a secondary hyperfibrinolysis. The results, above all in the cases of liver cirrhosis, can be explained by pathophysiological mechanism: a decreased synthesis of clotting factors-a disturbed portal microcirculation with fibrin deposition-an impaired function of the liver RES.

[Basic disturbances and extra-renal complications - prognosis determining factors in acute renal failure].

The author adopts a definite attitude to the prognosis of acute renal failure, based on the experiences of 410 peritoneal dialyses and 168 haemodialyses in 165 patients. In a total lethality of 51% there is no relation between the choice of the technique of dialysis and lethality. The changes of survival are mainly determined by the causal disease and its extrarenal complications. The worst prognosis have patients with a breakdown of other vital functions besides renal failure. The possibility of the alternative treatment haemodialysis/peritoneal dialysis should always be given.

[Clinical chemistry parameters in continuous peritoneal dialysis].

The continuous peritoneal dialysis (CAPD) is with four or five change of 1 1/2-2 l dialysis solution a day a 24 hours/day and 7 days/week dialysis. In 6 patients with terminal renal insufficiency the behaviour of biochemical parameters which serve as acknowledged indicators for the efficiency of a dialysis technique, were pursued under this method more than 4 weeks. At the end of the treatment period the following average values came up: creatinine 9.0 mg/dl, urea-N 65 mg/dl, anorganic phosphate 4.8 mg/dl (without Al-hydroxide), potassium 4.3 mval/l, calcium 4.1 mval/l and a pH-value of 7.35. The results allow the conclusion that with the CAPD a sufficient elimination of retention products is possible and values in the suburaemic region may be achieved similar to the conditions in the intermittent dialytic technique. Conclusions concerning a broad application of the technique in the terminal renal insufficiency may, however, not be made alone from these observations, since the problems of relapsing peritonitis and of the chronic loss of protein known from literature did not yet play a role in our patients in the short observation period.

Aldosterone in vitro restores nephron Na-K-ATPase of distal segments from adrenalectomized rabbits.

Aldosterone, when added in vitro to defined nephron segments, dissected from adrenalectomized rabbits, restores the depressed Na+-K+-ATPase activity in the thick ascending limb of the loop of Henle and in the cortical collecting tubule within 1 h of incubation. The direct effect is specific and, as demonstrated for the cortical collecting tubule, dose-dependent. The time course of aldosterone-mediated Na+-K+-ATPase activation is compatible with the conjecture of changes in the enzyme lipid environment.

Age-related changes in the auditory evoked brainstem potentials of albino and pigmented guinea pigs.

The auditory evoked brainstem responses of guniea pigs in two age groups were recorded and examined for evidence of age-dependent changes at peripheral stations in the auditory pathway. Because pigmented guinea pigs have been found to be less sensitive to sounds than albinos, both groups were included in this study. Old and young animals did not differ in response latency or in the conduction times associated with the individual potentials. By contrast, the amplitudes of the brainstem responses to high-frequency stimuli were distinctly reduced in old guinea pigs, with no difference in the dynamic of the amplitude between the two age groups. Within each age group, albino and pigmented animals resembled one another in all parameters studied. The effect of the aging process at the peripheral stations of the auditory pathway is discussed in the light of these results.

Age-dependence of the neural auditory thresholds of albino and pigmented guinea pigs.

Short-latency brain-stem responses of albino and pigmented guinea pigs were examined for evidence of age-dependence of the auditory threshold. Both groups exhibited a loss of hearing correlated with age. Moreover, albinos had significantly lower auditory thresholds than pigmented guinea pigs of the corresponding age group. A further difference between the two groups lay in the degree of which sensitivity declined with age; hearing loss in albinos was only about half that in pigmented animals.

[Results of treatment on 56 sarcomas of the uterus between 1953 and 1977 (author's transl)].

At the Frauenklinik Finkenau Hamburg a total of 56 women were treated for sarcoma of the uterus between 1953 and 1977. Accounting for 1.2% of all malignancies, sarcoma is a rare tumor. The different histological types are analysed with respect to age distribution, endocrine status, preceding pregnancies, etc. Treatment results over a 15 years period and a subsequent 10 years period were compared. Symptoms and local findings show great variability. Preoperative diagnostic procedures frequently did not result in final diagnosis. The mode of therapy changed over the long period of 25 years. Forty-nine patients were followed up over a period of more than 10 years. In Patients with sarcomas which had derived from preexisting myomas, the 10 year survival rate was 89% (16/18). The total 10 year survival rate was 57% (28/49). Effectiveness of different therapeutic approaches such as operation, irradiation, and cytostatic therapy is demonstrated. Local recurrences and distant metastases usually occurred within 2 years after primary treatment. One case with successful chemotherapy of pulmonary metastases of leiomyosarcoma is demonstrated showing complete remission over a period of 28 months.

Unchanged glycolytic capacity in rat kidney under conditions of stimulated gluconeogenesis. Determination of phosphofructokinase and pyruvate kinase in microdissected nephron segments of fasted and acidotic animals.

The glycolytic capacity in rat kidney was studied under conditions of stimulated gluconeogenesis like 48 h starvation and 48 h acidosis induced by force-feeding with a NH4Cl/glucose solution. Rats fed ad libitum and force-fed with a glucose solution served as controls. The specific activities of the key enzymes phosphofructokinase and pyruvate kinase were determined in single segments of the nephron. The determinations were performed by microdissection of lyophilized tissue sections followed by enzymatic analysis, including oil well technique, enzymatic amplification, and fluorometric measurement. In the distal portion of the nephron of both fed and force-fed animals a 10-fold higher specific activity of phosphofructokinase and pyruvate kinase was found compared with the proximal portion, known as the site of renal gluconeogenesis. Starvation and acidosis altered neither the distribution pattern nor the specific activities of these enzymes. From these results it is concluded that renal glycolysis does not undergo alterations at the level of enzymatic activities in favour of renal gluconeogenesis, probably because of the separate localization of both pathways within the nephron.

[Immunoprophylaxis against Marek's disease and its perspectives].

An account is given of the present international position in immunoprophylactic control of Marek's disease. An assessment is made of the method used for immunisation in the GDR. In that context, reference is made to problems for which solutions will have to be found in the long run.

[Studies of iron level, Fe binding capacity, copper and protein in the blood plasma as well as hemoglobin in the blood of sows during the day].

The average iron level in the blood plasma of six gilts, weighting between 120 kg and 130 kg, was 103. 3 micrograms/100 ml, prior to early-morning feeding. The iron fixation capacity, measured at the time, was 673 micrograms/100 ml. Average iron levels went up to 121.3 micrograms/100 ml after 1.5 to two hours and to 130.3 micrograms/100 ml in the course of 3.5 to four hours. An average level of 124.3 micrograms/100 ml had been reached after another 1.5 to two hours. The iron fixation capacities first declined and then went up again. The average iron level recorded from eight adult sows, weighing between 180 kg and 210 kg, was 75 micrograms/100 ml, when measured seven weeks from piglet weaning. The average iron fixation capacity was 488.3 micrograms/100 ml at the same time and for the same animals. The iron level did not change throughout testing. The average copper level was 230.9 micrograms/100 ml, at the beginning, but individual differences in behaviour occurred during the day. No difference was recorded, at the beginning, between gilts and adult sows for blood plasma protein, levels being 7.73 or 7.81 g/100 ml, and for haemoglobin in the blood, levels being 12.8 or 12.5 g/100 ml. Individual variations, however, with no clear-cut direction, occurred over the testing period.

[Iron, iron-binding capacity, copper and protein in blood plasma and hemoglobin in the blood of sows before and during pregnancy].

Blood tests were applied to five gilts and twelve adult sows, prior to insemination as well as in the sixth, ninth, twelfth, and 15th weeks of pregnancy. Average haemoglobin values in both groups dropped from 14.0 g/100 ml or 11.3 g/100 ml, at first measurement, to 9.5 g/100 ml in the ninth week of pregnancy. Then regrowth occurred to 12.6 g/100 ml or 12.2 g/100 ml up to the 15th week. The average blood plasma iron levels were 142 micrograms/100 ml or 117 micrograms/100 ml, at first measurement and went up in both groups, between the twelfth and 15th weeks, from 110 micrograms/100 ml to 161 micrograms/100 ml or from 105 micrograms/100 ml to 149 micrograms/100 ml. The original iron fixation capacity values for both groups were 673 micrograms/100 ml or 645 micrograms/100 ml and dropped to 524 micrograms/100 ml or 492 micrograms/100 ml by the 15th week of pregnancy. The copper levels grew from 221 micrograms/100 ml to 305 microgram/100 ml for gilts and from 200 micrograms/100 ml to 236 micrograms/100 ml for adult sows, from the twelfth to 15th weeks. The average blood plasma protein levels had been 7.7 g/100 ml or 8.1 g/100 ml, at the beginning, and went up to 8.5 g/100 ml in either group by the 15th week.

The clinical spectrum of Campylobacter fetus infections: report of five cases and review of the literature.

Five cases of extra-intestinal Campylobacter fetus infections are described and an additional 242 cases are reviewed from the literature. A variety of clinical syndromes are encountered including endocarditis; thrombophlebitis; meningitis; pneumonia and pleuritis; and infectious arthritis. Thirty-eight per cent of patients presented with bacteremia enteritis. Campylobacter fetus demonstrates a preference for endovascular surfaces. The majority of patients are male and have an underlying illness. Mortality is increased in patients infected with C. fetus intestinalis. Therapy is based on in vitro antibiotic susceptibility tests although the organism is usually sensitive to an aminoglycoside and chloramphenicol.

[Acute dialysis treatment of urologic patients].

51 urologic patients were dialyzed following acute indications during 1968-77. The 47 adults were 31-81 (average 59) years old. Due to the frequency of occurrence the predominant diseases were: Urolithiasis (combined with pyonephrosis, urosepsis, nephrocirrhosis), malignant tumors, and bladder neck adenomas. 18 patients were postoperative cases. 32 patients suffered from severe diseases or complications outside the urogenital tract. According to a differentiation of three risk groups, there were 33 patients belonging to the most severe group III. The patients' admission to the dialysis unit was late in most cases: 13 patients were already in coma or precoma, 18 patients overhydrated, 11 patients' serum potassium was more than 7 mval/1,28 patients' serum creatinine was more than 10 mg/dl. A prophylactic dialysis was possible in 11 cases only. We have accepted all 51 acute urologic patients, admitted to our clinic, for dialysis treatment. Peritoneal dialysis was performed in all 51 patients, only in 6 of them this treatment was followed by haemodialyses. The reasons for prefering peritoneal dialysis were haemorrhages or the danger of haemorrhages, a critical cardiovascular state, or an extreme acotaemia. In 143 peritoneal dialyses with 91 insertions of stilet catheters, one perforation of the small intestine occurred. The patient survived the resulting peritonitis. 13 of 15 patients with this indication got into an operable state in the course of dialysis treatment. Lethality of 61 per cent (31 of the 51 patients died) was related to the severity of the basic urologic disease. 4 of these latter patients could have been admitted to a regular dialysis treatment. In further 6 cases this would have been possible after a special urologic treatment.

[Primary carcinomas and sarcomas of the vagina - therapeutic results (author's transl)].

Treatment was given to 41 cases of primary carcinoma and nine cases of sarcoma of the vagina, over ten years at the Gynaecological Hospital of the Medical Academy of Erfurt. Combined therapeutic radiation has remained to be the optional method. Surgery has continued to be justified, depending on indication. Treatment of vaginal sarcoma has remained to be problematic. All therapeutic facilities available at appropriately equipped carcinoma centres should be used on such cases.
