[Acanthosis nigricans maligna--a gastrointestinal neoplasie. A review (author's transl)].

Acanthosis nigricans maligna (a. n. m.) is a well known paraneoplastic syndrome of the skin in 90 p. c. associated with tumours of the gastrointestinal tract (mostly gastric carcinomas) in contrast to pseudoacanthosis. Metastases are detected at the time of first diagnosis usually. Therefore, the a. n. m. may be the first sign of the tumour but does not represent an early symptom. The a. n. m. typically disappears with eradication of the responsible tumour and may reappear on its recurrence or metastasizing. In all cases an intensive search for gastrointestinal cancers is absolutely necessary. The etiopathology of the a. n. m. is still under advisement and the current state of knowledge is discussed in detail.

[Effect of laxative measures on the serum concentration of digoxin in the human].

Plasma digoxin levels were measured in patients on laxatives during the initial phase and during the steady state of digoxin intake. Plasma digoxin levels were significantly lower during intake of laxatives. Reduced intestinal resorption may be due to accelerated passage through the intestinal tract and to alteration of distribution volumes.

[Leukocyte-adherence-inhibition reactivity in patients with colorectal and pancreatic cancer (author's transl)].

The immunoreactivity of patients with colorectal or pancreatic cancer was investigated by a modified leukocyte adherence inhibition test (LAI). The microcapillary-LAI-test was easy to practise and measurements had a low intra-assay coefficient of variation. The number of false positive results was low, only 1 of 39 patients with benign disease and not one of 13 healthy volunteers reacted in the presence of colorectal carcinoma extracts. In 43 patients with colorectal cancer 17 of 25 (68%) with Dukes' B & C and 15 of 18 (83%) with Dukes' D cancer were LAI positive. Of the 19 patients with colon polyps 6 positive results were observed, exclusively in case of large villous adenomas, polyposis coli and one polyp with a focal carcinoma. 10 of 11 patients with pancreatic cancer reacted in the test. Leukocytes from patients with limited cancer of other source, when tested with colorectal or pancreatic carcinoma extracts, showed a negative LAI response. However, in case of metastatic diseases some crossreactions were observed, which resulted in false positive tests. Following our results, the LAI test could be of value in the diagnosis of gastrointestinal carcinoma.

[Expert judgement of pancreatic disease (author's transl)].

An expert judgement about patients with pancreatic disease has to be founded on a detailed description of the status. Diagnosis has to be established by reliable results of physical laboratory examination, which ought to be repeatable on control. Analysis of causal relationship has to be based upon scientifically well documented facts in regard to etiology, pathogenesis, course and prognosis of the disease. Objective judgement of pancreatic disease nevertheless is hampered by difficulties considerably in establishing diagnostic results and interpreting these. Significance of secondary pancreatitis as compared to primary pancreatitis is underestimated quite often nowadays. Excretory insufficiency of chronic pancreatitis and as a consequence insufficient resorption is quite often the determining factor for physical unfitness.

[Autonomous nervous system and bile ducts (author's transl)].

Storage, concentration and excretion of bile in the extrahepatic bile duct system are rather complicated processes being regulated by the autonomous nervous system. Groups of ganglionic cells and nerve fibers are located in the papilla, the neck of the gall bladder, and to a lesser degree in the wall of the gall bladder, that is to say in parts of the system which are important in regard to coordination of the excretory function. The number of ganglionic cells decreases in diseases which go along with functional disorders of the bile duct system. If the papilla Vateri and the duodenal musculature are disconnected anatomically, the papilla will retain autonomous muscular activity which however will not be coordinated any more with duodenal function, as the chymus passes by. Parasympathetic activity plays a major role in regulating the complicated mechanism of bile excretion. The autonomous nervous system is important for normal functioning not only of bile excretion but also of other organs; this explains the coincidence of dyskinesia of the bile ducts with other functional disorders being observed rather often. Autonomous regulation of bile excretion has to be taken into consideration as well, when it comes to outlining a therapeutic regimen.

[Arterial and venous blood-pressure in the mesenteric circulation of rabbits with combined infusion of secretin and norepinephrine (author's transl)].

Norepinephrine and secretin were infused into V. cava inf. and Art. mesent. sup. with a short intervall and after that immediately successive on rabbits, anesthetized by urethane-chloralose; simultaneously blood-pressure was measured in Aorta abdom., Art. mesent. sup. and V. mesent. sup. Secretin alone does not change blood-pressure; infusion of norepinephrine into V. cava inf. leads to a rise of blood-pressure of about 16% in Aorta abdom., of about 12% in Art. mesent. sup.; in V. mesent. sup. blood-pressure remained unchanged. Although secretin and norepinephrine have a contrary effect on the vascular tonus with cats, in rabbits combined infusion of secretin and norepinephrine caused an increase of blood-pressure not fewer than infusion of norepinephrine alone. Possible haemodynamic mechanisms were discussed.

[Investigation of the choleretic action of febuprol on healthy test subjects (author's transl)].

The choleretic effect of 3-butoxy-1-phenoxypropanol-(2), Febuprol (INN), was examined in healthy volunteer subjects. In a dose range from 50 to 200 mg, the total bile is dose-dependent and excretion is significantly increased. The excretion of the bile constituents bile acids, bilirubin, cholesterol and phospholipids is increased proportionally with the bile secretion. The lithogenic index remains unaltered.

[Combinations of routine laboratory tests in diagnosis of malignomas (author's transl)].

The question if combined routine-laboratory-tests could improve the search for malignomas in man was checked by comparison of 519 patients with carcinoma and 460 patients with other diseases. In order to do so a combination of four (erythrocyte sedimentation rate, alkaline phosphatase, red blood-picture, relative alpha-2-globuline-increase) and five (additionally alpha-1-globuline-increase) was put up. For either group the found constellations - consisting of normal and pathological items - and their sensibility and specificity were set up. To find out their diagnostical value the likelihood-ratio was determined. Those combinations with exclusively pathological results of all four respectively five laboratory reports and the linked symptoms "anemia - relative alpha-2-globuline-increase" showed to be of high differential diagnostical value. In these groups the probability of malignoma was found to be 4,6:1, 8:1, 3,2:1. The serum protein dispersion of a group of patients with benigne and maligne diseases of the digestive tract was additionally checked by means of the paper-electrophoresis. Those changes that showed a relative increase of all globuline-fractions combined with a decrease of serum albumins under 45% was almost exclusively found in patients with malignomas. All-together this paper shows that certain pathological laboratory-tests--if found together in one patient--give a grave indication for the presence of malignoma.

[The diagnostic importance of low-density lipoprotein (LP-X) for the diagnosis of cholestasis].

Based on the clinical, biochemical and morphological examination of a selected group of patients, the following may be said about low-density lipoprotein (LP-X): The LP-X test 1. proves to be a reliable method of examination for diagnosing cholestasis. It is a quick and simple test; 2. has a highly significant statistical relationship (x2 = 31,7) to the histological results and other parameters indicating cholestasis; 3. does not, however, distinguish between intra- and extrahepatic cholestasis and positive results do not allow any conclusion as to the etiology of the icterus; 4. makes it possible to diagnose cases of cholestasis with aP activities not clinically manifest; 5. in borderline cases conclusions may be drawn from a combination of LP-X and aP results and, most of all, from LP-X and gamma-GT, a highly sensitive, specific and simple method of diagnosis. 6. diagnostic findings of still greater significance resulting from applying laboratory data indicating or precluding cholestasis may be derived from a prospective study with a larger group of patients, using the same parameters and taking the likelihood ratio into consideration.

[Milk intolerance, lactose intolerance and lactase deficiency in partial resection of the stomach].

Of 40 patients with a partial gastrectomy (Billroth-II) 10 developed a milk intolerance and 11 had a lactase deficiency, the latter in 4 cases appearing together with a decrease in other disaccharidases. Only 2 of the 11 lactase-deficient patients complained of milk intolerance. The Ethanol lactose tolerance test (ELTT) was performed in 21 patients and was found to be abnormal in 6. Whereas cases of abnormal ELTT usually (4 of 6 cases) showed a lactase deficiency, only 2 patients with milk intolerance showed an abnormal ELTT and lactase deficiency. Milk intolerance can therefore only exceptionally be explained by lactase deficiency, and lack of lactase in the upper jejunum usually does not produce intolerance symptoms. Moreover, in comparable determinations from the afferent and efferent jejunal loop no differences in enzyme activities could be observed.
