Collagenolytic activity in keratoconus.

Corneal buttons were obtained from eight young adult patients with advanced keratoconus and were incubated in an organ culture system. The medium was assayed for collagenolytic activity using 14C-labeled reconstituted native collagen fibers as substrate. The assay was performed with and without prior collagenase activation with trypsin. The results indicated that the medium obtained from keratoconic corneas showed higher levels of collagenolytic activity compared with normal control corneas. We suggest that the level of collagenase activation may be a pathogenic factor in the development of keratoconus.

Corneal transplantation following prolonged storage in M-K medium.

A successful penetrating keratoplasty was performed on a 25-year-old keratoconus patient inadvertently using a 79-year-old donor cornea which had been stored for 32 days at 4 degrees C in M-K medium. A thin, clear graft ten months postoperatively suggested either recovery of the original donor endothelium or possible host endothelial cell migration. The clinical advantages of prolonged M-K storage beyond seven days versus the increased risks of endothelial decompensation and secondary infection are reviewed.

Piloplex, a new long-acting pilocarpine polymer salt. B: Comparative study of the visual effects of pilocarpine and Piloplex eye drops.

Induced accommodation and changes in vision (distance and near) were measured monocularly and binocularly in 9 young healthy volunteers in a double blind study after administering to them pilocarpine hydrochloride 4%, Piloplex 3.4, and saline eye drop instillations. Piloplex 3.4, a new long-acting pilocarpine polymer salt, and pilocarpine hydrochloride 4% (both contain equal amounts of pilocarpine-3.4%) induced changes in vision and accommodation. These changes were greater with pilocarpine hydrochloride than with Piloplex. The maximum changes occurred half an hour after instillation and the effect vanished after an additional period up to 3 hours. The changes were greater when measured monocularly than binocularly. Piloplex initiates a prolonged hypotensive effect which lasts for 12 hours. Patients with glaucoma are thus able to use Piloplex on a twice-daily schedule. Consequently, visual disturbances occur only once a day in contrast to pilocarpine hydrochloride given 4 times a day, which induces 3 visual disturbances during the day.

Computerized corneal topography of furrow corneal degeneration.

PURPOSE: To analyze the corneal topography features of furrow corneal degeneration. SETTING: Western Galilee-Nahariya Medical Center, Nahariya, Israel. METHODS: Computer-assisted corneal topography analysis of two patients with furrow corneal degeneration was obtained and studied. RESULTS: The typical appearance of circumferential peripheral thinning in the lucid interval between the limbus and corneal arcus indicated a clinical diagnosis of furrow corneal degeneration. The corneal topography maps showed central flattening and excentric localized steepening. A step of 1.50 diopters was seen central to the peripheral furrow. CONCLUSION: Keratometry and noncomputerized keratoscopy are insufficient methods of evaluating the topographic features of furrow corneal degeneration. Computer-assisted corneal topography showed steepening and central flattening that may be attributed to the thinned peripheral circumference.
