Tourniquet infusion chemotherapy in extremities with malignant lesions.

Tourniquet infusion chemotherapy involves the direct injection of a chemotherapeutic drug into the main artery of an extremity with prior application of an external tourniquet proximally on this extremity set at above the level of systolic pressure for ten minutes. Thus, the drug is not diluted and pushed by the blood into the venous circulation before diffusion into the tissues has occurred. This technique, applied in seven patients for a total of 40 instances, proved to be safe. Skin erythema and blisters, which are reversible, occur in the treated area. Complete clinical regression was achieved in all six patients with evaluable tumor, with high percentages of tumor necrosis. It was possible to avoid amputation in five of the seven patients treated. This technique appears superior to perfusion, but the long term permanence of regression has not yet been ascertained.

Mesenteric fibromatosis in Gardner's syndrome.

Two patients with mesenteric fibromatosis in Gardner's syndrome were treated by us. These tumors are slow-growing and may remain quiescent for long periods. Review of similar cases in the literature discloses that less than half of these tumors have been removed, pointing to the difficulty of their resection. Dissection of the infraduodenal portion of superior mesenteric vessels may help define whether these tumors are resectable. Radiation therapy or treatment with antiestrogens may be helpful in the treatment of unresectable tumors.

Carcinoid tumour of possible thymic origin: case report.

A case of carcinoid tumour of possible thymic origin in a 43-year-old man is presented. Carcinoid tumour arising in the anterior mediastinum (thymus) is a rare condition and only 26 cases have been reported in the past. A review of the literature showed that three-quarters of the reported cases were asymptomatic but the remainder of patients presented with various endocrine symptoms. Ultrastructurally the thymic carcinoid is similar to those found in other organs and also appears to be derived from Kultschitsky cells of the thymus gland.

Relationship between ethnicity and sleep patterns in normal controls: implications for psychopathology and treatment.

The purpose of the study was to examine ethnic influences on sleep regulation. Seventy-three normal volunteers from four ethnic groups (17 African-Americans, 10 Asians, 30 Caucasians and 16 Hispanics) were studied for two consecutive nights with sleep polysomnography recordings in the laboratory. The subjects were in good physical and psychological health, and were asymptomatic with respect to sleep/wake complaints or sleep disorders. With the exception of minor differences, sleep continuity, sleep architecture and rapid eye movement (REM) sleep patterns were comparable among the four groups. African-Americans had evidence of more stages 1 and 2 and diminished stage 4 sleep, whereas the Hispanics had higher REM density. These preliminary findings suggest that sleep patterns are remarkably similar across cultures. There are, however, important cross-ethnic differences, specifically in the depth of sleep and in phasic REM measures. Because sleep disturbances are common symptoms of emotional disorders and since many psychoactive agents affect sleep, cross-ethnic differences in sleep patterns may have potential implications for the treatment and prevention of psychiatric disorders.

The cytology of metastatic balloon cell melanoma.

We describe the cytologic presentation in fine needle aspirates of two examples of metastatic balloon cell melanoma. These hypervacuolated cells may be misinterpreted as adenocarcinoma or liposarcoma. Intermediate forms between the classic nonvacuolated round melanoma cell and the ballooned melanoma cell are described.

Diagnosis of chronic lymphocytic thyroiditis (nodular presentation) by needle aspiration.

Needle aspiration was performed on 40 patients with a diagnosis of chronic lymphocytic thyroiditis, and the cytologic findings are described. Clinically, 32 patients had a nodular lesion or lesions, and 8 had diffuse enlargement of the thyroid. A 20-ml glass syringe and an 18-gauge needle were used to aspirate the lesions, and smears were stained according to the Wright-Giemsa technique. Diagnostic material was obtained in 36 patients, and in 4 the aspirate was unsatisfactory. There was good correlation between cytologic findings and antibody tests. Thirteen patients underwent surgical excision of the lesions, and the diagnosis was confirmed histologically. In one patient, the histopathologic diagnosis was chronic lymphocytic thyroiditis and thyroid adenoma. The latter component was not recognized initially on the cytologic specimen. The abundant cellular material obtained in the early nodular forms may correspond to a cellular phase of the disease as opposed to a later fibrotic phase. The evaluation of the epithelial-lymphoid ratio is stressed, and substantial alteration of this ratio may suggest an association of chronic lymphocytic thyroiditis with other thyroid lesions, benign or malignant.
