[Typical CT findings with the intrathecal infusion test in post-traumatic disorders of cerebrospinal fluid dynamics].

Simultaneous computer tomographic and cerebrospinal fluid dynamics examinations in patients with high-pressure hydrocephalus, normal pressure hydrocephalus and cerebral atrophic processes are presented and discussed. Exclusive CT and MR examinations do not allow a differentiation of patients with normal pressure hydrocephalus and patients with cerebral atrophy. In the simultaneous examinations the morphological substrate during the intrathecal infusion test is represented.

[Indications for the intrathecal infusion test in disorders of cerebrospinal fluid dynamics].

Indications for and results of the intrathecal infusion test including 50 patients with chronic disturbances of the circulation of the cerebrospinal fluid are presented and discussed. The computer-aided infusion test permits in 93 per cent a reliable differentiation between patients with a normal-pressure hydrocephalus and those with cerebro-atrophic processes. Functional checking of the valve systems is a further indication for the intrathecal infusion test. In patients with a classical pressure hydrocephalus or subdural hygromas, however, we do not see an indication for this invasive examination procedure.

[Axial osteosynthesis of dens axis fracture as a routine procedure in neurotraumatology. Diagnosis--Surgical-technique--Complications--Follow-up].

A survey on 34 cases of odontoid process fractures treated by means of axial osteosynthesis since 1981. In cases of acute fractures rate of bony consolidation was 100%, in cases of pseudoarthrosis 50%. 6 complications until the first 20 weeks were successfully treated. No later complications. Removal of implanted material is usually not necessary. Operative treatment of non union by means of bony grafting, axial osteosynthesis and atlanto-axial arthrodesis seems to be helpful. Investigations for the best implantation model must be continued.

[Results of surgical treatment of 170 injuries of the middle and lower cervical spine].

The article presents the results of examination of 170 patients with a cervical spine injury, who had been operated by ventral interbody fusion. It presents operative procedures and discusses the results concerning to neurological deficit before treatment. Cloward-Crock's and Bailey-Badgley's technique together with anterior plate fixation yields immediate stabilization and provides the best functional outcome.

[The need for spongiosa grafting in thoracolumbar vertebral body fractures].

Report on examination of 31 patients with fractures of spine in thoracolumbar region. All patients were treated by external fixation without accomplishing a plastic of spongiosa. The permanent SI-Index after operation is coming to 0.85. The outcome by use of external fixation is attesting our opinion, that with this method a plastic of spongiosa isn't necessary.

[Experimental studies of chemonucleolysis with aprotinin].

We have attend operatively exstirpated tissue of intervertebral disks to solution with 250.000 I. E. and 500.000 I.E. aprotinin. After preparation and colouring of the tissue we examinate by microscope. We couldn't find out neither a macroscopic nor microscopic evident effect of chemonucleolysis of aprotinin. Our opinion is, that an application of aprotinin to reduction of intradiscal pressure or to chemonucleolysis isn't effectively.

[The classification of neurons in the corpus geniculatum laterale pars ventralis of the albino rat: a Golgi-Kopsch study].

Neurons in the ventral lateral geniculate nucleus of 120 rats of a wistar strain were investigated with the Golgi Kopsch technique. Various features, such as size of neurons, their dendritic fine structure and their localization inside the nucleus were used to find new aspects for the classification of vLGN neurons except geniculo tectal relay neurons (GTR neurons). The results of our detailed investigation are in accordance with the classification given by Brauer et al. (1984). The dendrites show different morphological surface specializations, as there are smooth and varicose dendrites besides the spiny ones. Medium-sized neurons are most variable in their dendritic structure. It can be assumed that they represent a heterogenous group. The density of spines in the neurons investigated is much lower than in GTR neurons.

[Fixateur externe as a definitive treatment method of unstable thoracolumbar spinal fractures in emergency conditions--technical modification and clinical experiences].

A survey on 31 injuries of thoracolumbar spine, 27 were accompanied with neurological deficiency. Treatment was exclusively by means of percutan attached f.e. until bony consolidation. An average treatment time of 152 days took place. Mean rate of correction was about 0.24 (Beck's SI-Index). The former applied frame construction was substituted by self developed spindle fixateur system, adapted to the requirements of traumatology of the spine in emergency situations.

[Dorsal ganglion of the wrist joint: a trivial, minor surgery intervention?].

A small operation which is thought to be trivial with a recurrence rate of 30 to 40 percent. The dorsal ganglion of the hand arises from the typical predilection site in the dorsal portion of the scapholunate ligament. By excising the capsular attachments over the scapholunate ligament we can significantly reduce the recurrence rate to only a few percent. But the absence of recurrence doesn't guarantee for certain that the patients are painfree. In spite of the operation one third of the patients complain of persisting pain which is often due to an irritation of the posterior interosseous nerve (PIN) by the ganglion. A selective dissection and resection of the PIN proximal to the radiocarpal unit as a partial dorsal wrist denervation is proposed. This shows that the indication for the operation must be evaluated thoroughly.

[Computerized tomography and electroencephalography findings in post-traumatic brain edema].

The reduction in size of the ventricular areas in the computer tomogram as a morphological substrate of a generalised cerebral edema in 43 patients correlated better with the vigilance than with the EEG findings. This fact as well as the representation possibility of additional intracranial haemorrhages favour the computer tomography in the posttraumatic acute phase as compared to the electroencephalography, which should be employed in the subacute phase.

[The relative importance of the brain stem acoustic evoked potential (BAEP) in patients with brain stem tumors].

The brain stem acoustic evoked potential is a non-invasive procedure at the disposal of neurological diagnostics that permits to obtain information about the functional condition of the brain stem. As regards the question of the topodiagnostic possibilities of this examination method in patients with brain stem tumours the BAEP parameters were compared with the clinical symptoms existing at the time of the examination as well as with computer tomographic findings. There is a good agreement of the severity of the clinical symptoms and the distinction of the BAEP changes. In case of unilateral BAEP changes or BAEP changes with a pronounced accentuation on one side indications with respect to the lateral localisation of the brain stem tumour in connection with the clinical findings can be derived. An exact topodiagnosis of brain stem tumours is exclusively possible by the correlation of clinical symptoms, results of picture-yielding examination methods and BAEP examination.

[The value of auditory evoked brain stem potentials in the diagnosis of circulatory disorders in the area of vertebrobasilar circulation].

The acoustically-evoked brain-stem potential in patients with vertebrobasilar insufficiency has enjoyed a varied diagnostic evaluation in the literature. Our own examinations reveal an advantage in subdividing AEBP changes into normal and marginal, slight and pronounced, within a group of patients with blood supply disorders in the vertebrobasilar region. AEBP permits inferences to be made on the extent of the damage. However, one should be chary of referring AEBP change of vascular origin to any particular section on the basis of these deductions alone. Only the comprehensive consideration of clinical symptoms, AEBP findings, and neuroradiological examination leads to a valid opinion on extent and location.

[Computer-assisted model trials of cerebrospinal fluid dynamics of the craniospinal system].

Model-experiments demonstrated that the introduced computed method for simultaneous purpose of the parameter compliance C(p) and resistance to outflow of CSF R(p) by intrathecal infusion test produce equal results as conventional methods. By the calculation of the resistance to outflow of CSF R(p) no pressure plateau is necessary so that the injury of the patient is slightly. Conclusions of a standardized method for the intrathecal infusion test in clinical practice was made.

[Pathophysiologic aspects of conservative therapy of post-traumatic cerebrovascular disorders of the microcirculation].

In order to avoid or diminish secondary cerebral damage in the treatment of the craniocerebral trauma the removal of disturbances of the microcirculation is of decisive importance to the course of the disease and the prognosis. The microcirculation is essentially determined by rheological factors such as haematocrit, plasma viscosity, aggregation and fluidity of erythrocytes. Rheological aspects should be included to a greater extent than has been done until now in the basic therapy of traumatic cerebral blood supply disturbances. If this is not done, an important therapeutic reserve is not made use of. In our opinion isovolaemic haemodilution is a practical method. The application of cerebroselective calcium antagonists appears to be reasonable as well.

[Complications of Forestier disease affecting the cervical vertebrae as seen from a neurosurgical point of view].

In the presence of Spondylosis hyperostotica an isolated spondylophytary ventral ossification leads to the stiffening of the vertebral column with a considerable loss of elasticity. Patients with a rarely occurring participation of the cervical vertebral column are particularly endangered by traumata. Even a trivial trauma may lead to the fracture or a spinal canal diameter in the lower normal range lead to neurological symptoms. The degree of the instability and the neurological symptoms determine the therapeutical approach. The strategy of the treatment is discussed on the basis of the example of six patients from two neurosurgical clinics.

Identification of fragments of proximal and distal tubular cells in the urine of patients under cytostatic treatment by immunoelectron microscopy with monoclonal antibodies.

In an attempt to identify the origin of cellular fragments released in the urine of patients treated with potentially nephrotoxic drugs such as cytostatics, two monoclonal antibodies were applied: monoclonal antibody PM II 9 C2, directed against an antigen in distal tubular cells; and monoclonal antibody PM II 39 H11 specific for an antigen in proximal tubular cells. The specificities of both monoclonal antibodies were elaborated in the indirect as well as in the direct immunofluorescence technique. Both antibodies were then used to identify cellular fragments obtained from the urine of patients treated with cytostatic drugs by ultracentrifugation. By application of the indirect immunogold method, it was shown that material of proximal as well as distal tubular origin was shed by the damaged cells. Whereas the proximal tubular antigenic epitope recognized by PM II 39 H11 was always found in large irregular complexes of debris-containing vesicles, the distal tubular antigenic epitope recognized by PM II 9 C2 was always found associated with filament-like regular structures. This is the first report in which excretion of components of distal tubular cells is demonstrated as a consequence of the nephrotoxic side effects of cytostatic treatment. With the help of monoclonal antibodies, it has now become possible to identify and to investigate the damage inflicted on the distal part of the tubule system in addition to the well-documented proximal tubular damage.
