[Heel necrosis: a complication of geriatric diseases (author's transl)].

12 heel necroses were seen in a group of 596 geriatric patients. The patients average age was 73 years. 3 tentative causes for the development of heel necrosis were investigated. 1. Imobility, 2. Arterial occlusive disease, and 3. Diabetes. Immobility was demonstrable in all cases. Additionally, arterial occlusions of the leg were present in 7 and diabetes in 6 patients. The percentage of arterial occlusion and diabetes was significantly higher in the heel necrosis group than in the overall hospital cohort. The two diseases can be looked upon as risk factors for the development of heel necrosis.

[Xanthofibrogranulomatosis, pontine glioma, multiple nevocytic nevi and albinism (authors transl)].

A case of retroperitoneal fibrosis with ureter compression is reported. Clinical picture and course were determined by the presence of an additional tumor (astrospongoblastoma) of the pons. Extraretroperitoneal tissue changes found at post mortem examination were shown histologically to be foreign tissues of the same type deposited in the retroperitoneal space (disseminated xanthofibrogranuloma). Possible connections between the disseminated xanthofibrogranuloma, the pontine tumor and an albinism also present and multiple nevocytic nevi of the skin are discussed.

Epithelially derived endothelin and nitric oxide in asthma.

Immunohistochemical staining of endobronchial biopsies has identified increased expression of the 21-amino-acid peptide endothelin (ET) and the inducible form of the enzyme nitric oxide synthase (iNOS) within the airway epithelium in asthma. Elevated concentrations of ET are also recovered in bronchoalveolar lavage fluid from asthma patients. iNOS generates the gas nitric oxide from L-arginine, and elevated levels of NO in exhaled air have been described in asthma. ET is a potent bronchoconstrictor and levels of ET in lavage and resting airflow obstruction are correlated. The effects of ET on bronchomotor tone may be modified by NO as this is a bronchodilator. The relative balance between ET and NOS may thus contribute to resting bronchomotor tone. ET also stimulates fibroblast proliferation, collagen gene expression and through its inhibitory actions on collagenase will promote airway wall collagen deposition and contributes to airway wall thickening which underlies bronchial hyperresponsiveness. The regulation of these epithelial events may thus be important to the control of asthma.

[Normal pressure hydrocephalus in geriatric patients--a treatable form of dementia].

Six geriatric patients are presented who were treated because of normal pressure hydrocephalus. A tentative diagnosis was made if dementia, gait disturbance, and incontinence together with a typical CAT-scan were present. In these cases an operation was carried out. After ventriculo-peritoneal shunting clinical improvements were seen in four patients. Postoperative complications were hygromas in three patients and a shunt-dislocation in another patient. A chronic subdural bleeding was recorded also in one of these patients with only a temporary success in this case. Two patients showed no clinical improvement at any time. However, in these cases additional neurologic symptoms together with typical CAT-scan changes were present. Before making a decision for an operation vascular processes and Alzheimer's disease should be ruled out.

Influence of hepatic and renal failure on pharmacokinetic properties of the novel recombinant plasminogen activator BM 06.022 in rats.

BM 06.022 is a novel recombinant, unglycosylated plasminogen activator comprising only the kringle 2 and protease domains of human tissue-type plasminogen activator (t-PA), and it has a longer half-life than t-PA. Because t-PA is mainly cleared by the liver, rat models of hepatic and renal insufficiency were used to identify the main catabolic organ of BM 06.022, compared with alteplase (recombinant t-PA). Hepatic insufficiency in rats was chemically induced by pretreatment with carbon tetrachloride for 1 day; renal insufficiency was achieved by acute bilateral, surgical nephrectomy. Plasma concentration of functionally active BM 06.022 or alteplase was measured by an indirect spectrophotometric assay. Intravenous administration of 200 kU/kg of BM 06.022 or alteplase over 15 sec to rats with hepatic failure or olive oil pretreatment as control did not significantly alter the total plasma clearance (CL) of BM 06.022 vs. control (4.9 +/- 0.5 vs. 5.7 +/- 0.5 ml.min-1 x kg-1, NS) in contrast to alteplase (32.1 +/- 6.5 vs. 82.3 +/- 12.9 ml.min-1 x kg-1, p < 0.05). Renal insufficiency increased the CL of BM 06.022 vs. sham surgery (3.1 +/- 0.4 vs. 6.3 +/- 0.5 ml.min-1 x kg-1, p < 0.05) in contrast to alteplase (33.2 +/- 5.2 vs. 37.2 +/- 7.2 ml.min-1 x kg-1, NS). In vitro incubation of 2000 U/ml BM 06.022 or alteplase in citrate blood and plasma demonstrated a decrease of the plasma concentration with a shorter (p < 0.001) half-life for BM 06.022 than for alteplase in blood (71.9 +/- 3.1 vs. 130 +/- 3.3 min) and in plasma (62.9 +/- 1.2 vs. 129.9 +/- 5.3 min).(ABSTRACT TRUNCATED AT 250 WORDS)
