[Initial clinical results on the problem of implantation of intraocular lenses in diabetes mellitus].

About 2% of the Austrian population suffer from diabetes mellitus, the incidence being higher in the 5th and 6th decades of life. As a consequence the number of diabetics requiring cataract surgery is relatively large. In recent years extracapsular cataract extraction with implantation of a posterior chamber lens has become the method of choice. The question under what circumstances implantation is indicated in diabetics and in which cases it is contraindicated is still under discussion. During the past 2 years extracapsular cataract surgery with implantation of a posterior chamber lens has been performed on 36 patients (42 eyes). The average period of observation ws 8.5 months. The patient material was analyzed in a retrospective study and the results of surgery are reported in this paper.

[Follow-up of central diabetic retinal changes following peripheral photocoagulation].

One eye of each of 27 patients suffering from preproliferative diabetic changes in the periphery of the retina was treated by panretinal photocoagulation; in the fellow eye photocoagulation was only performed in the peripheral zones of the fundus. Follow-up observations over an average period of 1.5 years showed a centripetal progression of capillary obliterations in only 5 eyes after peripheral photocoagulation and a less pronounced tendency to development of a diabetic maculopathy as compared to eyes treated by panretinal photocoagulation.

[Immunofluorescence studies of the human trabecular meshwork].

Accumulation of extracellular material in the trabecular meshwork is responsible for increased resistance to outflow in the human eye. By means of the indirect immunofluorescence technique the authors detected extracellular collagen type IV, fibronectin and laminin in the trabecular meshwork and the wall of Schlemm's canal of glaucoma patients and controls. Collagen type IV is the so-called "basement membrane collagen" present in the lens capsule, Bowman's and Descemet's membranes. Fibronectin is a glycoprotein with many biologic activities; it can promote cell attachment and forms complexes with collagen. Laminin was isolated in 1979 and is present in the lamina lucida of basement membranes. The increased production and pathological metabolism of these three substances might be of some importance in the pathogenesis of primary open-angle glaucoma.

[Healon as an internal tamponade in retinal detachment surgery].

In 14 selected cases of retinal detachment the vitreous cavity was filled with Healon (0.8-4.0 ml) during subretinal drainage. Follow-up periods ranged between 2 and 7 months. In 12 cases the retinal detachment was cured; in 2 eyes redetachment with massive periretinal proliferation occurred. In addition to transient Tyndall phenomena, which were frequently seen in the anterior chamber and vitreous cavity, the intraocular pressure increased to as much as 45 mm Hg postoperatively in 5 cases. Owing to failure of the antiglaucoma therapy in 3 cases the Healon filling had to be removed.

[True idiopathic exfoliation].

In contrast to pseudoexfoliation, true exfoliation is an extremely rare condition not associated with glaucoma. In true exfoliation the superficial layers of the anterior lens capsule are partially peeled off. This results in a fine homogenous water-clear transparent membrane floating in the anterior chamber. The etiology of this mostly bilaterally condition may be exposition to infrared rays, iridocyclitis, metallic intraocular foreign bodies and trauma. To our knowledge this is the first published report of a case of idiopathic true exfoliation of the lens capsule.

[Coagulation technic in diabetic maculopathy].

One hundred thirty-six eyes with diabetic maculopathy, which had been operated on by photocoagulation between 1976 and 1980, were followed up with regard to the morphology of the retinal changes and compared with 246 untreated eyes. Of the treated eyes, 77% showed no further progression of the microangiopathy, whereas in 80% of the untreated eyes the development of a diabetic maculopathy was observed.

[Postoperative intraocular pressure following iris-clip lens implantation].

Within the past three years intraocular Binkhorst lenses have been implanted in 152 consecutive cases. In 36% of the intracapsularly operated cases (123 eyes) and 45% of the extracapsularly (29 eyes), intraocular pressure increased to 40 mm Hg for three to four days postoperatively. Pseudophakic-pupillary block glaucoma was not observed in any of the cases. Fourteen days postoperatively no case required glaucoma treatment. As in normal aphakic eyes, intraocular pressure dropped to subnormal values 8 weeks postoperatively in 6% of the cases.

[Electron microscopical and chemical investigations of the pathological changes of the vitreous body].

By means of vitrectomy in vivo investigations of the pathologic changes of the vitreous body including histology and chemical analysis have become feasible. The electron microscopical examinations of the tissue particles gained by means of vitrectomy displayed collagen fibrils which were partly irregular, most of the mesh-work being split up. The periodicity of these fibrils was about 64 nm, similar changes had been observed in human vitreous following perforating injuries, as well as in rabbit vitreous after blood injections. All typical stages of the lysis of red cells and their phagocytosis by large macrophages could be demonstrated. In order to get an accurate chemical analysis of the material resulting from vitrectomy it was first to all necessary to find out a method which enabled us to compare the concentrations of different chemical substances in the material gained by vitrectomy with the content of these substances in the whole vitreous body. Of 16 pairs of human post mortem eyes, enucleated between 3 and 5 hours after death, one eye was submitted to vitrectomy under constant suction- and infusion-pressure and cutting rate, the duration of surgery being different (3-9 min.); in each fellow-eye the whole vitreous body was removed. The comparison of the different concentrations of chemical substances showed a typical correlation. This method was performed in 13 clinical cases suffering from different kinds of vitreal changes. The concentration of protein was found to have increased in cases suffering from intravitreal hemorrhages, in cases suffering from shrinkage of the vitreous body after spontaneous complete resorption of intravitreal bleedings and in one case suffering from intravitreal membranes after endophthalmitis; in cases suffering from subvitreal bleedings the protein content of the vitreous body was approximately normal. The pathology leakage of the retinal vessels as it can be found in cases suffering from proliferative diabetic retinopathy caused no significant increase of the intravitreal protein concentration. The concentrations of lactic acid and glycogen were examined in order to find out if anaerobic glycolysis could be proved in the vitreous body of eyes suffering from ischaemic retinopathy. Only one case showed an increased concentration of lactic acid, in 3 cases there was a reduced content of glycogen. In these 3 cases the possibility of the existence of anaerobic glycolysis could be excluded, because in rabbits a rapid excretion rate of exceeding concentrations of lactic acid via retinal vessels had been found.

[A method of standardizing the results of quantitative chemical investigations on material gained from vitrectomy (author's transl)].

In this study a method is described which allow conclusions to be drawn from the concentration of chemical substances in the material gained from vitrectomy in relation to the content of these substances in the whole vitreous body. In 16 pairs of human postmortem eyes enucleated 3-5 h after death one eye of each pair was subjected to vitrectomy under constant suction and infusion pressure and cutting rate; the duration of surgery varied (3-9 min) and in each fellow eye the whole vitreous body was removed. Comparison of the different concentrations of chemical substances in the samples showed that they could be correlated.

[Quantitative chemical analyses on the material gained by vitrectomy (author's transl)].

The material gained by vitrectomy from 13 eyes suffering from different pathological changes of the vitreous body was analysed in relation to the concentrations of protein, lactic acid and glycogen, and standardized by means of a special method. The concentration of protein was found to have increased in cases of intravitreal haemorrhage, shrinkage of the vitreous body after complete spontaneous resorption of intravitreal haemorrhage, and in a case of intravitreal membranes following endophthalmitis; in cases of subvitreal haemorrhage the protein content was approximately normal. The concentrations of lactic acid and glycogen were examined to find out if anaerobic glycolysis could be proved in cases of ischaemic retinopathy. Only one case showed an increased concentration of lactic acid while in three cases there was a reduced content of glycogen. In these three cases the possibility of anaerobic glycolysis could not be excluded, because in rabbits a rapid excretion rate of increased lactic acid concentrations via retinal vessels has been reported.

[Comparison of betamethasone and clobetasone butyrate eye drops and their effect on intraocular pressure (author's transl)].

The effect of clobetasone butyrate eye drops on ocular pressure was tested in 15 steroid responders suffering from different types of conjunctivitis and compared with betamethasone eye drops. Elevated intraocular pressure following application of 0.1% betamethasone eye drops decreased significantly within 2 weeks under medication with 0.1% clobetasone butyrate 4 times daily, whereas fellow eyes kept under treatment with betamethasone suffered further increases in IOP. The therapeutic effect and subjective compatibility of the two steroids were found to be approximately equal.

[Clinical experience with cryotherapy of lid tumors (author's transl)].

Eleven patients with lid tumors underwent cryotherapy. Of these, six suffered recurrence of the tumor following surgery or radiotherapy, respectively. The patients' average age was 66.1 years. The temperatures required for cryotherapy were obtained with fluid nitrogen. Two possible modes of treatment may be distinguished. In contact freezing (used in eight cases) the cryoapplicator is in direct contact with the surface of the tumor. In the second method, spray-freezing (used in three cases), fluid nitrogen is sprayed at a certain pressure directly on to the surface of the tumor. In all cases treatment was performed on outpatient basis. The indications for treatment, the technique applied and the results obtained hitherto are presented.

[Factors provoking the development of rubeosis iridis after vitrectomy in diabetics (author's transl)].

A retrospective study on 184 eyes of 167 diabetics which had been submitted to a vitrectomy because of various affections of the vitreous body and retina shows, that aphakic eyes, cases suffering from repeated bleeding into the vitreous cavity, and eyes with advanced stages of diabetic retinopathy are mostly preposed to the development of rubeosis iridis. Coincidence of two of these factors was found in more than two thirds of the eyes with post-operative rubeosis iridis. The stage of the diabetic retinopathy was preoperatively determined by means of ultrasonography.
