Direct approach to the difficult anterior neck region.

This article presents an alternative method to correct the sagging neck with a local dermolipectomy and median platysma duplication. The technique is recommended mainly for the older patient with redundant skin and fat in that region and who, for some reason, does not want to undergo a facelift.

Team approach to total auricular reconstruction.

The surgery of microtia is a procedure that demands surgical and artistic skills, as well as precise planning of interventions. In cases where a functioning inner ear is present, functional and esthetic reconstruction is performed jointly with ear, nose, and throat surgeons. The middle ear plasty is performed through a transmastoidial approach at the time of the second stage of the outer ear reconstruction. We demonstrate the special requirements of this team approach and also pay a tribute to Burt Brent, who has set the standards of modern ear reconstruction.

Rejuvenation of the lips.

One of the primary causes of labial wrinkling is soft-tissue volume loss. I suggest combining the various ancillary procedures with substantial tissue augmentation using autologous dermis grafts.

[Esthetic surgery of the lower third of the face].

Various techniques for the correction of the aging perioral region are discussed. The authors emphasize the fact that one of the reasons of presenile lipwrinkling is soft tissue loss. As a remedy dermis grafting is recommended and is described in detail. The procedure can be combined with the standard techniques of perioral aesthetic surgery.

[Aspiration lipectomy, late results, unsatisfactory results and their correction].

When new techniques become available and subsequently are adopted by a large number of surgeons, improvements occur and an average complication rate becomes apparent. It is the analysis of the unfavourable results and the complications that stimulates improvement. After nine years of experience in suction lipectomy this technique has become in our hands a very safe procedure. If the empiric laws of surgery are observed, complications are extremely rare. Good indication and choice of the right patient are the key to success and it is important to realize that the amount of volume reduction is dictated by the limited retractile elasticity of the skin.

Liposuction is a procedure that permits elimination of localized fat deposits through a small incision that leaves an inconspicuous scar. The principal indications are fat deposits in the gluteocrural area, the hip, and the abdomen. In selected patients, submental and cervical fat pads are also an indication. For the obese patient, the technique should only be employed in combination with dermolipectomy. The author presents his original technique and discusses indications, drawbacks, and unfavorable results.

[Aspiration lipectomy, a status determination].

The method of suction lipectomy, which was introduced by us in 1976, has proved to be successful as an alternative and supplementary form of therapy in cases of therapy-resistant localised fat deposits. Critical selection of suitable patients and a meticulous surgical technique are essential for this procedure, whilst the shape of the suction tube is of secondary importance. Opinions vary as to whether it is better to perform a continuous or layered fat resection or to use a "tunnelling" procedure. For the reasons already mentioned we prefer to use the procedure of layered fat resection; developments in the future will establish whether different indications exist for both these techniques. We will also be in a position to judge the efficacity of the fibrinogen adhesives which we have been using in certain selected cases for the past two years.

Adipose Tissue

Regional fat aspiration for body contouring.

Our experience with the original suction lipectomy is presented. The fat removal, done with a specially designed suction tube, is performed layerwise, creating a continuous, regular cavity that is transformed into a virtual space by compressive dressings. The main indication being excessive peritrochanteric fat pads, the operation is done under either epidural or general anesthesia. No antibiotics or anticoagulants are administered, and the patients are mobilized as soon as fully awake. Practicing suction-assisted lipectomy for more than seven years, we have never observed an infection, thrombosis, or fat embolism. In experienced hands, good results can be expected in about 80 percent of patients. Complications are almost always of an aesthetic nature as a result of bad indications. The method is not a surgical treatment for severe obesity.

[Functional plastic surgery of the nose].

The authors emphasize the importance of the extramucosal approach to the septum and the cartilaginous and bony vault in the correction of the crooked nose, for elongation of the pig-snout nose with retracted tip, and for closure of septal perforation. Furthermore, they present their own procedure for surgical treatment of stenoses of the nares as well as for huge tissues losses of the septum.
