We report on 14 cases of unplanned out of hospital deliveries which occurring over a three year period. The incidence was 0.31%. All th women were booked and multi-parous. 11 (79%) of 14 deliveries occurred between the hours of 20.00 and 0800, suggesting some difficulty in getting to the hospital or a reluctance to attend the hospital during these hours. Family members were responsible for the deliveries in 7 (50%) of cases, and there was no maternal or fetal mortality. Morbidity in the babies was mainly hypothermia which occurred in 7 (50%) of the babies, 2 of whom (14%) required admission to the special care baby unit while 2 (14%) of the mothers suffered second degree tears. We conclude that women and their partners should be educated about contingency planning regarding how to respond in situations of unplanned out of hospital deliveries, to minimize maternal and neonatal morbidity.
Porphyria cutanea tarda (PCT) is the most common disorder of porphyria metabolism in Europe and North America. The incidence in women appears to be increasing, perhaps as a result of widespread use of hormonal contraception. Treatment in pregnancy is largely supportive, but screening of the pregnant patient for antinuclear antibodies, diabetes, human immunodeficiency virus (HIV) and hepatitis should be performed, as there is an association of PCT with these conditions. The baby should also be screened postpartum for PCT. Patients with PCT cope well with normal and operative delivery; however, an anaesthetic plan of management for labour and delivery is advisable.

Britain has the highest rate of teenage pregnancy in the European Union. In the younger teenager inadequate sex education, poor knowledge of and use of contraception, resulting from poor home circumstances and a high school drop-out rate appear to be important aetiological factors. A dedicated teenage clinic can enhance pregnancy outcome in this group of women, and results better than those from a standard antenatal clinic are achievable. For this group of women, other sources of sex education apart from the home and schools need to be explored.
Twin reversed arterial perfusion (TRAP) syndrome is an extremely rare manifestation of feto-fetal transfusion syndrome. This condition is seen in monozygotic twin pregnancies, where the affected twin is perfused retrogradely by the healthy twin. The affected twin presents with bizarre malformations such as acardia and acephalus. We present such a case, which was diagnosed prenatally. The pregnancy progressed uneventfully to term.

Intestinal obstruction in pregnancy is rare, volvulus being the commonest precipitating factor. It is a difficult diagnosis to make, because its symptoms mimic various pregnancy symptoms, but it commonly occurs in the third trimester or puerperium. A high index of suspicion is needed, and investigations for intestinal obstruction should be performed, especially when abdominal pain and vomiting persist. Careful evaluation of the abdominal cavity is also essential where a caesarean section is performed, particularly if there are unusual findings.

Thirty-two cases of emergency obstetric hysterectomy performed in a Nigerian Teaching Hospital were reviewed. The incidence was 1 in 575 births. Associated maternal mortality was 12.5%, morbidity was 62.5% and fetal loss was 78.1%. These rates are related to the indications which were spontaneous uterine rupture in 62.5% of the cases. The majority (68.7%) of the patients had poor antenatal care. Good antenatal care and delivery supervision would reduce the need for this operation.

A fatal case of tetanus arising from premature rupture of membranes in a retained dead fetus is presented. The condition followed the vaginal insertion of some herbs by a local herbalist. The case illustrates the danger of anaerobic infection in the presence of intrauterine fetal death particularly when it is complicated by premature rupture of membranes. Undue vaginal manipulation as happened in this case is an additional hazard and should be avoided.
