[Unstable stress profile--artefact or physiologic phenomenon?].

In 31 women with stable urethral closure pressure at rest the stress urinary profile (SUP) was found to be unstable. The reason was dislocation of the urethra during stress by between 1 mm and 8 mm. Of 14 women with stable SUPs, ten showed no dislocation whereas four had dislocation not exceeding 2 mm. A retrospective analysis revealed that dislocation had occurred in 40 of 114 patients (35.1%). SUPs with signs of dislocation should therefore be interpreted with care.

[Dilatation of the upper urinary tract in pregnancy--incidence, severity and follow-up. A sonographic study].

Incidence, onset and degree of the dilatation of upper urinary tract as well as the course throughout pregnancy was sonographically assessed in 310 pregnant women without symptoms from the urinary tract. 30 patients were regularly observed from the 16th week of gestation until delivery and within one week post partum. A grading system based on maximum calyceal diameter was used. 25% showed dilated calyces, primiparae more often than multiparae. The right kidney was dilated in 56, the left in 3 and both kidneys in 20 women. No decrease in parenchymal diameter was found during longer lasting dilatation of the pelvicalyceal system. Within one week after delivery the dilatation disappeared in nearly all cases. A gradual linear increase in incidence and severity of dilatation throughout pregnancy was found.

Comparison of two methods of islet preparation and transplantation in dogs.

Seventy-nine mongrel dogs underwent total pancreatectomy. Fifteen dogs served as apancreatic controls and died 7.0 +/- 4.2 days later (mean +/- SD). The pancreases of 44 dogs (group 1) were intraductally distended by manual injection of Hanks' balanced salt solution (HBSS). Thereafter each organ was mechanically disrupted and subjected to collagenase digestion as described by Mirkovitch et al. The pancreases of 20 dogs (group 2) were intraductally distended and subsequently perfused with collagenase by a roller pump. The organs were then mechanically disrupted and filtered through screens as described by Horaguchi et al. The resulting tissue suspensions were injected into the spleens of the dogs as autotransplants in both groups, by direct punction of the splenic capsule in group 1 and by retrograde infusion via a splenic vein tributary in group 2. The functional outcome was better in group 2 than in group 1, as assessed by the number of animals that became normoglycemic after transplantation [15/20 (75%) vs. 13/44 (30%); P = .0025]. The degree of islet purification, as measured by an increase in the tissue insulin/amylase ratio, was higher in group 2, and in both groups it was higher in normoglycemic than in hyperglycemic animals. The percent engraftment [i.e., amount of insulin recovered from spleen as percent of tissue transplanted (mean, 15.4% in group 1 and 14.5% in group 2) or as percent of original pancreas (mean, 4.9% in group 1 and 4.4% in group 2)] was low in both groups but again was higher in normoglycemic than in hyperglycemic animals within each group. In conclusion, both the degree of engraftment and purification and the route of implantation influenced the functional outcome after dispersed pancreatic islet autotransplantation to the spleen of totally pancreatectomized dogs, with purified tissue injected retrogradely functioning better than unpurified tissue injected directly.

[Leiomyoma of the small intestine].

Diagnosis and surgical treatment of a leiomyoma of the small bowel are described. The special findings in this case are the unusual location of this tumors and the fact that histological examination revealed a benign process, although the tumor had a diameter of more than 16 cm.

Reaction of bone marrow after cryo- and thermolesions on internal organs.

Focal thermo- and cryolesions were placed on one kidney and the liver of Wistar rats. The wound healing was investigated histologically and by tritiated thymidine autoradiography. Furthermore, the 3H labeling indices of neutrophilic and eosinophilic promyelocytes and myelocytes as well as that of erythroblasts were determined in bone marrow samples from femur and vertebra. Normal nonoperated rats and rats which underwent sham operation (simple laparotomy) served as controls. The cryonecrosis healed completely after 4 weeks, whereas thermonecrotic areas were still observed 12 weeks after operation. The maximum mesenchymal and epithelial proliferation occurred between the 2nd and 3rd postoperative days. In the bone marrow, the highest labeling indices were estimated in animals with thermonecrosis, with values remaining increased during the 2 weeks after operation. Significant differences between the bone marrow response after cryolesions and sham operation were not observed. Thermolesions of internal organs, for example, after electrocoagulation, evoke a persisting local granulomatous inflammation accompanied by a distinct bone marrow reaction probably because of the pure resorption of carbonized tissue components. Iatrogenic cryolesions, in contrast, are characterized by a quick and uncomplicated wound healing without a significantly increased proliferation of bone marrow cells, thus corresponding to the wound healing after sham operation.

Improvement in cyclosporine handling by anti-lymphocyte globulin in the early post-operative period.

The value of prophylactic anti-lymphocyte globulin (ALG) treatment after transplantation was examined in a prospectively randomised study. The control group was treated with azathioprine and steroids only. In a third group prophylactic ALG combined with azathioprine and steroids was given for 8.7 +/- 2.25 days after transplantation and was then replaced by cyclosporine and steroids. Renal graft survival rates could be improved if the conventional immunosuppression was accompanied by ALG prophylaxis. However, additional cyclosporine treatment proved to be desirable since in this way the number of early acute rejection episodes could be further reduced.

[Arterial circulation of the extrahepatic bile ducts].

The blood supply of the extrahepatic bile ducts was investigated in 15 human specimens. Human embryology explains the origin of the main blood supply in this area by the a. pancreaticoduodenalis posterior superior, the a. cystica and the a. hepatic propria. Other arteries are inconsistent and functionally unimportant. 2 or 3 marginal arteries are predominant in the arterial system surrounding the common bile duct. Surgery of the extrahepatic bile ducts should be performed with respect to this particular arterial network.

Evaluation of graft viability in heterotopic auxiliary liver transplantation in the rat.

In the rat model of heterotopic auxiliary liver transplantation, the coexistence of the engrafted liver and the recipient's native liver makes it difficult to evaluate the posttransplant graft viability. In this study, auxiliary liver transplantation was performed in Wistar rats, in which the recipient's native liver was handicapped with a 68% partial hepatectomy and a common bile duct ligation. Serum biochemistry of the liver was analyzed and compared with that of the selected control group. The surgical handicap of the liver showed severe damaging effects: the handicapped native livers appeared atrophic at autopsy, and no long-term animal survival could be achieved without an auxiliary liver transplantation. As the engrafted liver corrected the cholestasis of the handicapped native liver, significant differences of serum biochemistry were found between the transplanted group and the control group: for bilirubin concentration and gamma glutamyl transferase activity from postoperative day 3 to 28 (p < .05); for alkaline phosphatase on days 3, 7, 14, and 28 (p < .05); for alanine aminotransferase activity on days 3 and 14 (p < .05); and for aspartate aminotransferase activity on day 14 (p < .05). The efficiency to induce hepatic failure and to hamper its regeneration capacity in the native liver makes animal survival and liver biology as reliable parameters to evaluate the posttransplant graft viability in this rat model.
