[Hepatitis B screening in pregnant patients and their children--a Greifswald perinatal-pediatric study].

During a period of one year with systematic screening performed in 1,401 pregnant women 8 cases positive for HBs-antigen could be detected. For the investigation we used a counter electrophoresis method. In 500 serial specimens an additional examination was performed by aid of a RIA as well. With an incidence of 0.57% the number of HBs antigen positive pregnancies in the territory of Greifswald was found to be significantly higher than reported previously from other regions of the GDR. Our results claim for a mass screening for hepatitis B during pregnancy. Because of the possibility of virus excretion with human milk, HBs positive mothers should not be used as milk donors. Other results related to our investigations give evidence for the significance of obtaining specific hygienic regulations when medical care is given to asymptomatic HBsAg carriers. They also underline the necessity of interdisciplinary cooperation for teaching medical staff in health care centers and giving correct advice to those detected as hepatitis B carriers.

Importance of early diagnosis and follow-up in multiple endocrine neoplasia (MEN II B).

Three patients with a sporadic form of multiple endocrine neoplasia (MEN) IIb are presented. MEN IIb is the association of medullary carcinoma of the thyroid (MTC), pheochromocytoma and multiple mucosal neuromata. The age at diagnosis and primary therapy of MTC was relatively late (8, 16, 18 years) although thyroid enlargement (two cases), typical marfanoid habitus (two cases) or multiple mucosal neuromata (one case) had been evident since early childhood. The diagnosis was confirmed by measurements of tumour markers like calcitonin (CT) and carcinoembryonic antigen (CEA) for MTC, measurements of vanillylmandelic acid for pheochromocytoma, evaluation of the thyroid gland by sonography, scintigraphy and fine needle biopsy, and investigation of the adrenals by computer tomography and meta-(I-131) iodobenzylguanidine (I-131-MIBG) scintigraphy. After surgical treatment determinations of tumour markers confirmed relapses of MTC in all cases. Early diagnosis of MEN IIb, which could be made by recognition of the typical clinical appearance, is of special importance because of the poor prognosis of MTC.

[Therapy of primary pulmonary hypertension with phentolamine].

The effect of phentolamine was studied in 5 patients with pulmonary hypertension, of whom 3 had primary pulmonary hypertension and 2 had secondary pulmonary hypertension after pulmonary embolism. They received intravenous phentolamine in doses of 0.5 mg/min to 2 mg/min (i.e. up to a total dose of 27.5 mg in 25 minutes) during diagnostic cardiac catheterization. Under phentolamine infusion only 2 of the 3 patients with primary pulmonary hypertension showed a significant rise in cardiac index, from 1.4 to 2.0 and from 1.8 to 2.2 l/min/m2 respectively, a fall in pulmonary artery mean pressure from 100 to 93 and from 42 to 33 mm Hg respectively, and a decrease in pulmonary vascular resistance from 2950 to 1900 and from 1160 to 725 dyn X sec X cm-5 respectively. In 1 patient with primary, and 2 patients with secondary pulmonary hypertension, infusion of phentolamine had no effect. Interestingly, in the two patients with primary pulmonary hypertension who responded well to phentolamine the condition was very severe. It appears that even in cases with very advanced primary pulmonary hypertension there is a vasospastic component which can be influenced by vasodilators.

Correlation of field potentials with redox-states of cytochromes recorded from the olfactory cortical slice of guinea pig.

Field potentials and the reflection spectra of cytochromes were continuously recorded from the olfactory cortical slice of a guinea pig which was incubated under the normoxic and anoxic conditions. The reduction of the oxidated spectra of the cytochromes occurred before the decrease of field potential heights and the oxidation of the reduced spectra occurred before their recovery. In a 3 min anoxic experiment, a 90% recovery of the cytochromes aa3, b, and c, and the field potential amplitudes took place in this order after the restoration of oxygen.

[Minutio existimationis. On the question of therapy of psychologically traumatized, deformed patients].

Short case histories of twin sisters with progressive juvenile kyphoscoliosis who were incorrectly brought up and finally scorned as "witches" by their parents are used as examples to explain that Minutio existinationis (M.e.) can be regarded as a severe psychological trauma. The article seeks an approach for therapy. Rehabilitation of the two patients concerned proved, regrettably, unsuccessful. This again confirms that the life of sickly children after childhood is formed by the behaviour of the parents.

Long-term study of liver damage following subcutaneous injection of airborne particle extracts and polycyclic aromatic hydrocarbon fractions.

Female NMRI mice aged 9-12 weeks were each given a single subcutaneous injection of 0.5 ml of a suspension containing either the total extracts or the polycyclic aromatic hydrocarbon (PAH) fraction of airborne particles. Both the total extracts and PAH fractions contain 3 microgram benzopyrene. After about 15 months the livers were removed from the animals, which had by that time developed tumors at the injection site, and were subjected to electron-microscopical study. The essential alterations were observed in the nucleoli and the cell nuclei, which had greatly proliferated and exhibited irregular nuclear membranes. Advanced fibrosis was observed in central liver specimens of all groups. Marked alterations were also observed in the mitochondria and the mitochondrial cristae as well as in the bile canaliculi, Intracytoplasmic glycogen usually occurred densely clustered along the periphery of the cell. It may be concluded from the observations that both the total extract of atmospheric suspended particulate matter and the PAH fraction cause hematogenic damage to the liver following subcutaneous injection, a finding which cannot be interpreted as metastatic carcinoma.

Effect of silybin on hepatic cell membranes after damage by polycyclic aromatic hydrocarbons (PAH).

Young female NMRI mice were given a single intraperitoneal injection of a PAH mixture containing 3 micrometers benzo(a)pyrene in a suspension of 0.5 ml tricapryline. 3 and 4 days later a number of the mice received injections of 100 mg/kg body weight of Silybin in the tail vein. Thin sections of the animals' livers were studied under the electron microscope using the freeze fracture technique. Following intraperitoneal injection of the PAH suspension, and without treatment with Silybin, marked dilatation of the intercellular space and frequent occurrence of intracytoplasmic vacuoles was observed in addition to pronounced alterations in the hepatocytic nuclei and mitochondria. Following the administration of Silybin these alterations were no longer observed, though 50% of the cell membranes exhibited a marked band-like proliferation of the tight junctions. This observation may be interpreted as supporting the postulated membrane-stabilizing effects of Silybin.
