[Neurofibromatosis of the breast in a patient with Morbus von Recklinghausen].

Neurofibromatosis is a rare autosomal dominant disorder with several subtypes. Common is the appearance of specific skin alterations. Neurofibromas occurring in the breast are very rare, and in such cases they are most common in the areolar area. A case of a 46-year-old woman with von Recklinghausen's disease of the breast is reported, and the literature will be discussed.

[Guideline for the diagnosis of postmenopausal bleeding. PMPB Working Group of the SGGG].

2.1. History and clinical-gynecological investigation including a Pap smear are the first step in the clarification. The history should make sure if there is in fact bleeding from the genital and not from the urological or the intestinal region. Drug intake should be recorded, and risk factors for the development of endometrial carcinoma should be considered. This will not affect further investigation. The clinical-gynecological investigation should prove the source of postmenopausal bleeding according to the anatomical site--uterine, infra-, or suprauterine. The causes of infrauterine bleeding may easily be diagnosed by means of inspection of the external genitalia and further by using a speculum. The causes of uterine bleeding are of major importance. Cytology and colposcopy, supported by bimanual investigation, exclude cervical carcinoma as a cause of bleeding. Atypical endometrial cells on the cytological smear arouse suspicion of endometrial carcinoma. 2.2. Transvaginal sonography (TVS) is the next step if the above-mentioned investigations are negative. Both adnexa should always be investigated and the findings sonographically documented, so that solid cystic masses in the adnexal area can be better identified as suprauterine causes of postmenopausal bleeding. Then the uterus should be investigated. Further procedures are decided from the results of measurement of the longitudinal section of the endometrium at the level of maximum endometrial thickness. If the endometrial thickness is _<4 mm, an observant attitude can be assumed. After 3 months the patient should be controlled against using TVS. If bleeding recurs or the endometrial thickness is >4 mm on TVS, the procedure given in subparagraph 2.3 should be followed. In case the endometrial thickness is >4mm or not measurable, a histomorphological investigation according to subparagraph 2.3 should be performed. In such cases, saline infusion sonohysterography(SIS) is useful as a simple method to supplement TVS. It can aid in the decision making as to which further, more invasive measures should be taken (endometrial biopsy/hysteroscopic resection). Computerized tomography or magnetic resonance imaging are, as a rule, not indicated in patients with postmenopausal bleeding. 2.3. A definite diagnosis is possible only on the basis of a histological investigation. If TVS or SS show evidence of a polypoid state, removal under hysteroscopic control is the diagnostic method of choice. In cases of symmetrical or asymmetrical thickening of the endometrium on SIS, a less invasive biopsy may be sufficient. If the biopsy specimen does not yield representative diagnostic material, one should proceed as described above. A fractionated curettage should as a rule not be performed solely, but in combination with hysteroscopy.

[Does the increasing cesarean section rate reflect a shift in indications or a decline in quality?].

The cesarean section rate is continuously increasing at our department, from 13.8 to 17.2% between 1999 and 2000. The mother's wishes regarding the mode of delivery weigh increasingly on our obstetric decisions. Patient acceptance of elective cesarean section is high, and the increasing cesarean section rate is influenced by paramedical factors. Obstetrics appears to be developing into a mostly surgical discipline. The effect this has on quality (and costs) remains to be seen.

Primary carcinoma of the fallopian tube. A report of 19 cases with literature review.

Primary carcinoma of the fallopian tube is the rarest cancer of the female genital tract with an incidence of 0.5% of all gynecologic tumors. Since the first report in 1847 about 1,500 cases have been published. Due to similarity of the clinical presentation the staging and therapeutic management have been adapted to that of ovarian cancer. We retrospectively evaluated all the 19 patients who had been diagnosed with primary carcinoma of the fallopian tube at the Department of Obstetrics and Gynecology of the University of Zurich between 1977 and 1998. All lesions were staged according to the rules of FIGO adopted in 1991. At the time of diagnosis the median age was 62 (46-87) years. Twelve (63%) women revealed FIGO stage III-IV, whereas four (21%) and three (16%) patients were diagnosed in stage I and stage II, respectively. Eight (42%) women were nullipara. Histology showed serous-papillary carcinoma, in ten (53%) cases. The 5-year survival rate was 22% for all FIGO stages and 80% for stage I. None of the patients with stage III and IV survived 5 years. Ovarian cancer and primary carcinoma of the fallopian tube are similar in many aspects. Both carcinomas have a similar age distribution, show an increase among nulliparous women, are often of serous papillary histology, have a poor prognosis with stage and residual tumor size as important prognostic factors, and respond initially well to platinum-based chemotherapy. Nevertheless, there appears to be a difference between the two diseases: primary carcinoma of the fallopian tube is more often diagnosed in an earlier stage. This many be due to lower abdominal pain resulting from tubal dilatation and to abnormal bloody-watery discharge.

[Possible future of the project for quality assurance of indications and outcome in interventional cardiology and gynecology].

In the last few years the basis for national guidelines for the indications of coronary angiography, coronary revascularisation and hysterectomy has been established. The guidelines have been published and are accessible to medical doctors and patients on the Internet using a database system called "Second Opinion System": http:/(/)sos.inf.ethz.ch Dissemination, implementation and validation of the guidelines will be of major importance in the near future. With reference to validation, the question remains whether the established guidelines are correct for the everyday treatment of patients. A suitable method of answering this important question is comparison between the appropriateness and necessity rates of the various indication groups, combined with outcome measurements. The Internet-based second opinion system (SOS) may be used for data collection in order to verify hypotheses. Because of the database architecture, only relevant information about the patients is collected via Internet, independent of time and place. In addition, the system allows users to evaluate their individual data, and special attention is given to data protection. The discussion about priorities in health care (rationing) will be increasingly important in the near future. The present project may offer a way of maintaining adequate access to health care services for patients. Therefore, the participation of as many institutions as possible in the project "quality assurance of indication and outcome in interventional cardiology and in gynaecology" is of great importance.

[Gynecological infections in general practice].

Although infections of the lower female genital tract are common, they only rarely pose diagnostic and therapeutic problems for the experienced clinician. If there is no response to primary therapy, however, or in cases of recurrence further steps are indicated. Sexually transmitted diseases are characterized by common involvement of the upper genital tract by ascending infection. Appropriate therapeutic measures including treatment of the partner are important in order to prevent severe acute and chronic diseases.

Photodynamic therapy for the hypercalcemic type of the small cell carcinoma of the ovary in a mouse xenograft model.

OBJECTIVE: The hypercalcemic type of the small cell carcinoma of the ovary (HTSCCO) is a rapidly fatal ovarian tumor in young women. Photodynamic therapy (PDT) induces selective necrosis to malignant tissues. The aim of this study was to determine the sensitivity of the HTSCCO to PDT in a mouse xenograft model. METHODS: Tumors were obtained from a patient with a HTSCCO and were transplanted into nude mice. Following photosensitization with m-THPC, either superficial or interstitial laser light was administered to the tumors. Necroses were measured by morphometry. Serum calcium levels were determined prior to and after PDT. RESULTS: Superficial irradiation of m-THPC sensitized tumors showed over three times more necrosis than control tumors (P = 0.037). Interstitially irradiated tumors showed over seven times more necrosis than control tumors (P = 0.0012). All animals showed a highly significant hypercalcemia prior to PDT (P < 0.0001). PDT induced a significant decrease in serum calcium levels (P = 0.0297). CONCLUSION: This study suggests that PDT may be of therapeutic value for the HTSCCO.

Swiss consensus guidelines for hysterectomy. Swiss Society of Gynecology and Obstetrics, Switzerland.

OBJECTIVE: The quality of the indication for hysterectomy is widely discussed at present. In early 1996, the committee for quality assurance of the Swiss Society of Gynecology and Obstetrics decided to set up nationally accepted guidelines for the indication of hysterectomy. METHODS: A modified Delphi approach was used. In a first step, general guidelines and actions prior to hysterectomy were defined. An expert panel of 17 Swiss gynecologists rated 74 frequent indications, twice for appropriateness (more benefits than risks for the patient), once for necessity (n = 34; procedure has to be offered or discussed with the patient), and outlined suggestions to be performed prior to hysterectomy. RESULTS: In a home rating round before the first panel met, there was an agreement rate of 48%. In 45% we observed neither agreement nor disagreement; in 7% we found disagreement. After the panel discussion 89% of experts agreed, 11% were indeterminate, and there was no disagreement. The necessity ratings showed agreement in 68% while 32% were indeterminate. The average median rating on a 1-9 point scale (1 = extremely inappropriate, 9 = extremely appropriate or necessary) was 5.4 over all single indications for appropriateness and 7.8 in single indications for necessity. After a second panel for consensus all panelists agreed on both appropriateness and necessity. CONCLUSION: The results of the appropriateness and necessity consensus presented in this paper reflect the findings of a 17 member Swiss panel. This joint effort by a medical society may be a step towards the direction of a peer controlled healthcare system.

[Chemotherapy of endometrial cancer revisited].

The current status and future directions of chemotherapy in the management of endometrial cancer are reviewed. For patients with advanced or recurrent endometrial carcinoma the most active single drugs are doxorubicin, epirubicin, cisplatin, carboplatin, paclitaxel, ifosfamide, 5-fluorouracil and vincristine with response rates ranging from 18 to 36%. Data at the present time support the conclusion that if chemotherapy is indicated a combination of doxorubicin + cisplatin is the standard chemotherapy for patients with advanced or recurrent endometrial carcinoma and yields a response rate of 47-60%. A first trial using a combination of these drugs with paclitaxel promises an increase in response rate to 73%, but data regarding prolongation of survival are not yet available. Up to now the benefit of neither chemotherapy nor endocrine therapy could be established in the adjuvant setting.

[How much preoperative information? - How can it be documented?].

Preoperative information for patients and documentation of this information are becoming increasingly significant. Opinions diverge especially on how to document the information provided, the information protocol being only an ancillary element. The patient must be in a position to make an informed decision without suffering any prejudice. Information protocols relating to treatment, that is to the operation, aim at ensuring the quality of the treatment and at documenting the information provided. A scheme of the operation is an important element of this information. Twenty-one presurgical information protocols have been tested in a pilot phase and are still being evaluated by a patients' association. Information and documentation concepts need to be set up by professional societies before insurance companies and governmental offices start getting involved.

Photodynamic effects in vitro in fresh gynecologic tumors analyzed with a bioluminescence method.

Photodynamic therapy (PDT) is a promising alternative method for clinical cancer treatment. In the present study, cells from four breast carcinomas, seven ovarian carcinomas of various stages of differentiation, and ascites from a diffuse metastatic tumor were treated by PDT in vitro. Tetra(m-hydroxyphenyl)-chlorin (m-THPC) was used as the photosensitizer. Surviving cell rate was evaluated by the ATP-Cell-Viability-Assay (ATP-CVA), which measures light production as an interaction of intracellular ATP with the luciferin-luciferase complex. The most effective PDT of the tumor cells was achieved at an m-THPC concentration of 0.2 microgram/ml following incubation of the cells with photosensitizer for 24 hours. PDT toxicity resulted in a cell survival rate of 1% to 42% compared to untreated control cells (survival rate of control = 100%). The inhibitor concentration IC50 of m-THPC was determined both in the dark (dark toxicity) and in combination with laser irradiation. IC50 was defined as the concentration of photosensitizer which caused 50% of cell death. The IC50 values were heterogeneous in all tumor specimens examined. IC50 values for dark toxicity were on average 0.14 microgram m-THPC/ml for primary ovarian carcinoma, 2.16 micrograms m-THPC ml for refractory ovarian carcinoma and 0.3 microgram m-THPC/ml for breast carcinoma. After PDT, average IC50 value for refractory ovarian carcinoma was 0.04 microgram m-THPC/ml, for primary ovarian carcinoma 0.05 microgram m-THPC/ml and for breast carcinoma 0.03 microgram m-THPC/ml. These data might indicate that clinical PDT of gynecological carcinoma requires individual treatment conditions to achieve optimal results.

The role of the frequency-volume chart in the differential diagnostic of female urinary incontinence.

OBJECTIVE: To investigate the differential diagnostic capacity of the frequency-volume chart in 132 women with urinary incontinence. MATERIAL AND METHOD: For each patient, the readings of two 24-h frequency-volume charts were compared to the urodynamic diagnosis which was used as the gold standard in 73 genuine stress incontinent women, in 23 urge incontinent women, and in 36 women with mixed incontinence. RESULTS: The total voided volume, the mean voided volume, the largest single voided volume, and the smallest single voided volume were statistically differentiating single parameters among the three groups (p<0.05). Applying logistic regression to the two well-defined groups of patients, the one with urge incontinence and the one with genuine stress incontinence, the frequency of micturition during nighttime revealed the best discriminatory power for these conditions. CONCLUSION: This study shows that in the frequency-volume chart micturition during nighttime discriminates best between urge and stress incontinence.

Virtual reality based surgery simulation for endoscopic gynaecology.

Virtual reality (VR) based surgical simulator systems offer very elegant possibilities to both enrich and enhance traditional education in endoscopic surgery. However, while a wide range of VR simulator systems have been proposed and realized in the past few years, most of these systems are far from able to provide a reasonably realistic surgical environment. We explore the basic approaches to the current limits of realism and ultimately seek to extend these based on our description and analysis of the most important components of a VR-based endoscopic simulator. The feasibility of the proposed techniques is demonstrated on a first modular prototype system implementing the basic algorithms for VR-training in gynaecologic laparoscopy.

Quality assessment in gynecology and obstetrics. Projects and experience in Switzerland.

The first steps in quality assessment of results of performance in gynecology and obstetrics were taken by the Swiss Association of the OB/GYN Clinics of the Swiss Society of Obstetrics and Gynecology in the 1980's. Currently, medical data of more than 800,000 patients are available, allowing a good comparison of the quality of results. Concerned that purchasing coverage for health services was made exclusively on the basis of cost, resulting in lower public health quality, the Swiss Society of OB/GYN set up a Board of Quality Assessment in 1995. Not only the quality of results, but also the quality of indications, structures and processes are considered by using a modified Donabedian model. Moreover, standardized patient information forms have been worked out for 15 gynecological and five obstetrical operations. Since hysterectomy is the most common major gynecological operation in Switzerland, the evaluation of the quality of the indications is of substantial interest. Guided by the RAND Appropriateness Method, criteria for quality assessment with respect to appropriateness and necessity of hysterectomy have been drawn up. Swiss guidelines and 125 different clinical scenarios are defined by a panel of 22 OB/GYN experts. The aim of this project is to offer an interactive second opinion system, called "Swiss Guidelines for Hysterectomy", and anonymous self-assessment of quality. Appropriateness and necessity of medical procedures have to be defined by medical professionals providing a proper balance between the patient's desires and financial demands.
