[Differential diagnosis of breast tumors with eczematous breast changes: two rare cases].

It is reported upon two patients who were presented to us to evaluate suspicious tumors of the breast: A 64-year-old patient had very painful tumors in both breasts measuring four centimeters in diameter with additional eczematous lesion of the left mamilla, which has developed within the last weeks. For many years she suffered of a pseudoxanthoma elasticum (PXE) of the skin combined with arterial occlusion disease. Histologically these tumors showed necrosis and typical signs of PXE, which is to our knowledge the first case reported in literature. A 79-year-old patient showed an inverted eczematous nipple, combined with a retromamillar painless tumor of the left breast. The histology was Paget's disease with an underlaying ductal carcinoma. --The clinical picture and the diagnosis are presented, followed by the discussion and the differential diagnosis.

[Significance of the intercostobrachial nerve in axillary lymph node excision].

The anatomy of the intercostobrachial nerves is described; generally two or three of them exist on each side. We followed up 101 modified radical mastectomies with axillary dissection: 53 cases (52.5%) had no sensory disturbances of the intercostobrachial nerves, 48 (47.5%) had either a loss of sensibility or dysesthesias. 8 patients (7.9%) showed an intercostobrachial nerve entrapment syndrome grade I, whereas 4 (3.96%) patients complained of a more intense pain syndrome (2 x grade II, 2 x grade III).

[PERGYN: perinatal-gynecologic information system].

PERGYN is a scientific research project of a purely medical nature. Its goal is to procure and evaluate the entire body of perinatal and gynaecological data in all its complexity. Data input is based on standardized medical concepts. Further details on the medical concept are stored in a course tree. In conjunction with the interview machine the course tree creates storable medical data. Data evaluation criteria are definable in dialogue on screen. Other medical areas can also be integrated into the data model. The conversion of medical constituents of the course tree calls for a process as "knowledge engineering".

Weight percentile at birth. II. Prediction by endocrinological and sonographic measurements.

In a prospective study of 847 singleton pregnancies, the importance of various endocrine methods (serum estriol, HPL, SP1, beta-HCG, estradiol-17 beta, urinary estrogen excretion) and of two sonographic measurements (biparietal and thoracic diameter) for the diagnosis of growth retardation in the third trimester was studied. HPL and estriol determinations were best suited for the diagnosis of growth retardation. The thoracic diameter correlated most closely with the birthweight of the newborns. Sensitivity in relationship to growth retardation was between 17 and 35% for the HPL and estriol determinations as well as for both sonographic methods. Specificity was around 90% for these methods. The validity for all methods improved as the time of birth approached. Through the simultaneous measurement of one of the hormones and the thoracic diameter, an antepartal diagnosis of up to 50% of the hypo- and hypertrophic growth disorders was achieved. In the first two years of life a relationship between development and the HPL and estriol concentrations could be observed which was independent of the weight percentile at birth.

Continuous pH-measurement on anesthetized patients.

43 patients under general anesthesia were monitored by continuous tissue pH measurement and intermittent micro-blood-samples. The values obtained for the capillary pH and tissue pH were correlated. A highly significant correlation was found between tissular pH and capillary pH. Different methods of general anesthesia apparently do not influence the continuous tissue pH measurement.

A new application tool for the ROCHE pH electrode.

Using this tool the application of the pH electrode is nearly as easy as the attachment of a common ECG-scalp-electrode [3]. Visual control and lithotomy position is not necessary any more. The different tools are adapted to each other so that incision place and the tip of the pH electrode are automatically centered perpendicularly. The first results by using this tool promise to be successful. Time of application is short, the maintenance of the pH electrode is good and the application for the patient no longer disagreeable.

Continuous tissue pH measurement on the fetus during delivery.

Tissue pH values of 70 fetuses have been measured continuously during labor using the micro-electrode designed by Stamm. Significant correlation between continuous tissue pH and corresponding micro blood values can be shown. However, the relationship between patterns of tissue pH and capillary pH values seems not to be very constant. Therefore, the reliability of individual tissue pH measurements is still questionable.

[Fetal damage due to mushroom poisoning with amanita phalloides during the first trimester of pregnancy (author's transl)].

A 25-year old para II suffered of mushroom poisoning by amanita phalloides at 9 weeks gestation. When the life threatening maternal illness was overcome a therapeutic abortion was carried out at 12 weeks gestation. Prostaglandin F2 alpha was injected retro-amniotically. The damage to the fetus by the toxins of amanita phalloides is described and discussed.
