[Conception of the Klausenbach-Clinic for pre-geriatrics: Diagnostic, therapeutic and scientific aspects (author's transl)].

Risk-groups, cerebrovascular insufficiency without neurologic symptoms, transient ischemic attacks of the A. Carotis and/or A. Vertebralis, state after cerebral infarction. There are multidisciplinary possibilities for the consideration (internistic examination, neurologic-psychiatric examination with REM-phases patterns, permanent EEGs, power-spectrums, ultrasonic examinations, examination of the metabolism of the brain, psychologic examination with psychometric and psychopathometric procedures). There are various therapeutic employments, too (medical, psychologic-psychiatric therapy with brain-function-training, work-therapy (hobbies), and recreational activities, dietetic and physical therapy, health-talks, pension and social debates). By example of the health-talks there is shown what we mean with patient-adapted information. The scientific examinations concern retrospective studies of the anamnesis and psychometric investigations. Finally the patient-compliance is discussed.

[Circadian variations in insulin concentrations and blood glucose in non-diabetics as well as insulin requirements in insulin-dependent diabetics].

On the basis of the estimation of the plasma glucose and the insulin concentrations in 10-minute intervals during 24 hours by the mathematical analysis (multiple regression) over relations between the behaviour of plasma glucose and insulin concentration insulin secretion rates independent of plasma glucose can be calculated. In non-diabetics a circadian rhythm of the disposal of insulin - independent of food - is established. Thus the secretion rate increases in the early morning hours still before before the first meal and is maintained in different size up to nearly 18 o'clock. Then it established itself to the level of the nocturnal basal rate. Adequate changes of the insulin need during night are proved in diabetics without endogenic residual insulin secretion by means of the artificial B-cell Biostator. The circadian rhythm of the disposal of insulin take place parallel with adequate changes of other biological parameters and is of importance for the practice of the conventional insulin therapy as well as in the future use of non-measuring value-regulated insulin infusions (open-loop system).

Assessment of an algorithm for the artificial B-cell using the normal insulin-glucose relationship in diabetic dogs and men.

Insulin secretion rates after glucose loading were calculated from peripheral venous IRI concentrations considering half life and distribution space of exogenous insulin in normal men and dogs. The coefficients of multiple linear regression analysis between insulin secretion rates and plasma glucose (level and order and rate of change) were used as algorithm parameters in glucose-controlled insulin infusions. These were carried out in each dog based on individual estimations before the induction of diabetes but in the diabetic patients based on values derived from a group of normal subjects. Using this formula, nearly normal patterns of glucose and of insulin were observed in diabetic men and dogs under basal conditions and after IV glucose loading but not after meals. This algorithm enables selection of the parameters prospectively. The effect of a parameter combination depends on insulin sensitivity and it should be appropriately adapted. In the diabetic patients there was no predictable influence of the brittle or stable characteristics of the disease nor of insulin antibodies on the glucose curves obtained with glucose controlled insulin infusions.

[The social-psychiatric night ward of the nerve clinic of the University of Halle].

The conception of social-psychiatric night-hospital will be showed in a balanced system of psychiatric preventing and rehabilitating measures in the industrial centre of Halle. The care based on a physician-patient contract and will be objectivated by documentation of behavior and psychologic tests. First results confirm expectations and necessity of this special care.

[Intelligence testing in early old age. The correlation between the multiple-choice vocabulary test (MCVT-B) and the progressive matrices test (PMT) in a patient cohort in early old age].

Taking a random sample of 147 patients a substantial correlation of 0.54 between the "Mehrfachwahl-Wortschatz-Test (MWT-B)" by Lehrl and the "Progressiver Matrizen-Test" by Raven could be found. The MWT-B did not show significant differences between the age groups of 34-44 years, 45-54 years and 55-64 years. In the PMT differences between the age groups only showed up when the raw-scores were converted to IQ-scores (reffering to the corresponding age groups). Using the raw-scores no significant differences could be determined. Investigating an average IQ of 109 in the PMT, we found an average IQ of 97 in the MWT-B for the same patients. A main reason for the superelevated IQ-scores in PMT seems to be the reference to different age groups. A new standardization of the test should be undertaken.

Chromophore equilibria in bacteriorhodopsin.

An investigation of the dark equilibria between different chromophores of bacteriorhodopsin (BR) and studies of the kinetics of their interconversion and photochemical activity have led to the following conclusions. (a) A component of the 605-nm chromophore of BR decays in the millisecond range and is likely to be identical to the intermediate O of the photochemical cycle of BR and is assumed to be formed from the purple complex (PC) by the binding of one proton to BR. (b) An acidic form the PC, PCaL-, arises from the 605-nm chromophore by selective binding of anions L- (F- greater than Cl- greater than Br- greater than I- greater than Cl04-) to BR. (c) The isomeric equilibrium between 13-cis and all-trans retinal is approximately 0.15/0.85 in PCaCl-, 0.3/0.7 in the 605-nm chromophore as compared to 0.5/0.5 in the PC. (d) The 500-nm chromophore is formed from the PC by release of nearly one proton from BR. (e) The pH range in which the PC exists is reduced in a high-temperature structure of the purple membrane as compared to its low temperature structure. A model for the chromophore structure is proposed as a hypothesis, which allows a comprehensive interpretation of the results. In this model the absorption spectrum of the retinylidene lysine Schiff base is modulated by its protonation state and the interaction with an anionic group.

[Cerebro-organic syndrome during gold therapy. Possible correlation between gold therapy in chronic seronegative polyarthritis and the appearance of a cerebro-organic syndrome. A case contribution].

In this article the possible relationship between gold therapy of a chronic seronegative polyarthritis and the appearance of a cerebro-organic syndrome in a 56-year-old postmenopausal woman will be discussed. We take into consideration the basic illness, the biography, the hormonal situation, the simultaneous chloroquin therapy and the overdosage of the gold drug. After a careful consideration of the different diagnostical possibilities following the above mentioned factors, not only a temporal but also a causative relationship of gold therapy and cerebro-organic syndrome seems to be possible.

Derivation and experimental proof of a new algorithm for the artificial B-cell based on the individual analysis of the physiological insulin-glucose relationship.

Normal dogs were submitted to oral glucose loads or to intravenous glucose infusions. Insulin secretion rates (CISR) were calculated considering the resulting peripheral venous concentration differences in short intervals and the experimentally determined half life and apparent distribution space of exogenous insulin. Multiple regression analysis was done between CISR and both the level and the rate of change of plasma glucose. The regression coefficients were used as algorithm parameters for continuous plasma glucose dependent intravenous insulin administration in the same animals after induction of an insulin-dependent diabetes. Normal glycemic regulation over the day could be resotred by this sytem. The insulin responsiveness, however, varies from day to day; tusing this insulin dosage pattern we observed nearly normal plasma glucose curves and slightly elevated insulin reactions after glucose loading. This kind of algorithm could also be used in diabetic humans.

[Vagotomy and exocrine pancreatic secretion (author's transl)].

In dogs supplied with chronic pancreatic fistulas, there is no change of the basal exocrine secretion of volume three weeks after a bilateral truncular vagotomy. The normal initial reflex secretion during meal ingestion or glucose loading are abolished. On the whole, the secretory activity is reduced by about 50% after feeding. Secretions of volume and of enzyme protein are inhibited to the same extent, but that of bicarbonate is more strongly influenced. After vagotomy the inhibitory influence on all secretions of a rabidly produced hyperglycemia is reinforced. In pancreatic juice there is immunocreactive insulin activity (less than 1% fo the daily secreted amount). Insulin secreted in pancreatic juice after feeding is strongly reduced after vagotomy. This is the best reproducible proof of an influence on the exocrine pancreatic function of vagotomy.
