The HELIOS Image Related Services.

The HELIOS Software Engineering Environment is a tool for the construction of medical ward information systems. This paper describes the image processing tools which are a part of this system. The Image Related Services can be used both as ready-to-use end-user tools and as software modules for the construction of integrated multimedia applications. The tasks and architecture of the end-user tools and their integration into the HELIOS architecture are described. It is shown how the available image processing functionality can be used to build up new applications.

Sex-dependent urolithiasis in the portacaval shunt rat. 2. Hormones and stone formation.

Sex-dependent stone formation following portacaval shunt (PCS) was investigated in 82 male and 71 female Sprague-Dawley rats. 72.6% of male PCS rats and none of the female PCS rats developed urolithiasis, in 90.2%, potassium-hydrogen-urate stones formed. Hormonal analysis revealed significant alterations in steroid hormones and glucagon postoperatively. Male PCS rats showed a significant decrease in total and free testosterone and an increase in estradiol and glucagon levels. Female PCS rats showed a marked rise in testosterone and glucagon levels as well as a decrease in estradiol plasma levels. Male PCS rats had higher urinary and plasma uric acid concentrations compared to female PCS and sham-operated rats. Loss of testosterone and rise of glucagon in males was correlated with urolithiasis in so far as stone-forming rats had higher concentrations than non-stone-forming PCS rats. Our findings suggest that hormonal alterations might contribute to sex-dependent stone formation in PCS rats.

[TULIP--transurethral ultrasound-controlled laser-induced prostatectomy: an alternative to TURP?].

Transurethral resection of the prostate (TURP) is the most effective treatment for prostatic obstruction secondary to benign prostatic hyperplasia (BPH). This procedure has a high success rate with a very low mortality rate. Over the last 30 years, however, an unchanged morbidity rate of about 20% following TURP has been reported in the literature. In addition, haemorrhage requiring blood transfusion may occur in 6.5-22% of cases, and 68-92% of sexually active men have retrograde ejaculation following TURP. The side-effects and complications associated with TURP have led to the search for alternative therapies. The TULIP system represents a new treatment modality. TULIP is an acronym for transurethral ultrasound-guided laser-induced prostatectomy. The basic TULIP system consists of an ultrasound imager and a 20-F transurethral probe with an ultrasound transducer and a laser window at the distal end. Under ultrasound guidance laser treatment results in a coagulation necrosis effect in the hyperplastic prostate tissue. Coagulation necrosis is basically a bloodless tissue removal procedure as the tissue subsequently sloughs away in the fine particles during spontaneous micturation within 4-6 weeks after the procedure. Early results obtained in the first 18 patients with symptomatic, obstructive BPH treated by TULIP and with a follow-up of 6 weeks to 6 months demonstrate a significant improvement in both subjective and objective symptoms. The peak flow rate increased by up to 142% on average; the residual volume decreased by up to 75%; and there was a 77% decrease in average symptom scores compared with preoperative scores.(ABSTRACT TRUNCATED AT 250 WORDS)

[Circulatory changes in acute sauna hyperthermia after heart transplantation].

The authors report on the influence of a single sauna-stay of patients following heart transplantation (HTX) during the rehabilitation phase III. Investigations of blood pressure, heart rate, changer in hemodynamics in the small and large vessels in 8 male patients following HTX (medium age: 42 years) showed that sauna-hyperthermia is well tolerated. We watched significant decreases of blood pressure (systolic and diastolic values), an improvement of the microcirculation in the small vessels, an increase of the left ventricular ejection fraction (LVEF) and a decrease of the total peripheric vascular resistance (TPVR). Possibilities and limitations of sauna-therapy for the therapy of risk factors following HTX especially under immunosuppression are discussed. First results are demonstrated.

[Autogenic training in hypertensive dysregulation after aortocoronary venous bypass operation of coronary heart disease].

The authors report on the influence of autogenous training on blood pressure at rest and under stress conditions in patients with coronary heart disease following coronary bypass operation. We investigated the influence of autogenous training on the microcirculation and the left ventricular ejection fraction (LVEF) as well. The results showed a significant decrease in blood pressure at rest and also an increase of muscle circulation F and of LVEF.

[Sauna therapy in coronary heart disease with hypertension after bypass operation, in heart aneurysm operation and in essential hypertension].

It is reported about the influence of the sauna therapy on blood pressure, heart frequency, peripheric hemodynamics (Xenon-133-muscle-clearance) and the reaction of the cardiac output or left ventricular ejection fraction with hypertonia patients, patients with coronary heart disease (CHD) and hypertension and after aneurysm resection after heart infarction. It was shown that sauna therapy has a positive effect on hypertonic regulations troubles. One of the reasons of lowering blood pressure is the significant improvement of the peripheral hemodynamics. Sauna therapy does not result in any improvement of the left ventricles pumping function after operation. All described groups of patients showed a good tolerance and compliance with sauna therapy.

[Potential use of the sauna in the long-term treatment of hypertensive cardiovascular circulation disorders--a comparison with kinesiotherapy].

The authors report about the long-term response (one and three years) of blood pressure and heart frequency under rest and load (50 W) in patients with hypertension, coronary heart disease, essential hypertension and after aortocoronary venous-bypass operation (ACVB) (n = 65) under regular visits (twice a week) to the Finnish sauna. In comparison, 68 hypertensive patients who took a regular kinesiotherapy (running and swimming) were studied. Besides the parameters of heart circulation mentioned above, peripheric microcirculation (M. tibialis anterior) by means of xenon-133 muscle clearance and central hemodynamics by means of LVEF (single probe with In 113) were studied in CHD-patients. Cardiac output at rest and under 50 W load was recorded in hypertensive patients. It was shown that regular balneotherapy had a positive effect on regulation of blood pressure and hemodynamics in patients with hypertension or CHD with hypertension, as had kinesiotherapy in hypertensive patients.

Microsurgical arteriovenous revascularization in the treatment of vasculogenic impotence.

We studied 35 patients with vasculogenic impotence who underwent an operation since 1986 and who were followed postoperatively for a minimum of 1.5 years. In all patients a microsurgical anastomosis was performed between the inferior epigastric artery and the arteriovenous shunt of the dorsal penile vessels. Potency was restored or improved in 77% of the patients during the long-term observation. Doppler or duplex sonography confirmed the same percentage of patent anastomoses. Through microsurgical modifications of the Hauri procedure and the interposition of free venous grafts, it was possible to operate further in 3 patients with short epigastric arteries and to treat intraoperative complications effectively in 1.

[The effect of health resort therapy on cardio-regulation following heart transplantation with special reference to CO2 balneotherapy].

It is being reported about the influence of a CO2-bathing series with 12 patients after heart-transplantation in connection with a light-motion therapy on blood pressure and haemodynamics. The comparative group was a group of 12 heart-transplantation patients of the same age without cure during the comparative time. It was shown that the balneotherapy after HTPL has positive effect on the development of the systolic blood pressure under rest and exercise. The peripheric microcirculation (musculus tibialis anterior) improved significantly. The left ventricular ejections fraction (LVEF) kept without change. Generally, the balneotherapy is well tolerated by heart-transplantation patients. There is no danger of a cardiac overcharge of the left ventricle.

Primary retroperitoneal tumors.

The pathological classification, diagnosis and therapy of primary retroperitoneal tumors are presented. Three typical cases from our clinical experience are described. Ultrasonography is the most important screening method for the early diagnosis of primary retroperitoneal tumors. Early diagnosis and total surgical excision offer the only hope for successful treatment.

Interferon-alpha 2b instillation prophylaxis in superficial bladder cancer--a prospective, controlled three-armed trial. Project Group Bochum--Interferon and Superficial Bladder Cancer.

Sixty-seven patients with recurrent pTa G1-G3 to pT1 G1-G3 tumors were randomized into three groups receiving either Intron A at 10 MU/instillation, Intron A at 10 MU and mitomycin C (MMC) at 20 mg/instillation or MMC at 20 mg/instillation. After a mean follow up of 6.2 months no tumor recurrence has been seen in the group receiving combined therapy, whilst four out of 22 in the interferon group and five out of 23 in the MMC group suffered a recurrence. Side effects were slight. These preliminary results suggest that a combination of the two drugs is more effective than either drug alone.

Bursting strength of the rat aorta: comparison of suture and laser anastomoses.

The bursting strength of sutured and laser-welded vascular anastomoses were assessed in a rat aorta model. Postoperative investigations included postmortems, light and scanning electron microscopy and measurements of bursting strength at set intervals (30 min, 3, 14 and 28 days postoperatively, p.o.). Laser-bonded anastomoses consistently demonstrated lower bursting strengths than suture controls, with statistically significant differences at 30 min and 3 days p.o. (p less than 0.01). Laser anastomosis was easier and faster to perform, the foreign body reaction was markedly lower than in sutured controls. Formation of aneurysms was not observed.

[Micro-plasma treatment of dental pulp in animal experiments].

The direct and indirect effect on the endodont after thermic treatment with the micro-plasma unit from EGIDE were examined in animal experiments. The EGIDE unit is considered by its producer to be an alternative for CO2 lasers. The histological evaluation showed that both the plasma-therapy of dentin and the exposed pulp lead to massive pathological changes of endodont and was inferior to traditional methods as the indirect pulpcapping.

[Construction and managing of maintenance care in periodontology].

The maintenance care of periodontal treated patients is the necessary prerequisite for the individual long-life tooth preservation. The recall interval derives from the recidivity, the compliance of the patient, and from the individual disposition. The use of a self-developed data-bank-system relieve evaluation and processing of the extensive data which will be raised site-specific in the recall session. It will be given some directions to the content of the periodontal maintenance care.

[Computer-assisted video processing of Phast gels with ELBA].

Referring to one-dimensional micro electrophoresis-gels, produced at the division of nephrology (Klinikum Göttingen) by using the Phast-System (Pharmacia, Freiburg), the ELBA-program for image evaluation of electrophoretic gels (interactive systems, Heidelberg) was investigated. The digital image, obtained with a computer-controlled video-camera, can be stored up in a personal computer. Independend of time, the digitized data may be retrieved and utilized for evaluation of specific urinary protein patterns. Changes in intensity and constellation of the protein patterns, which may occur during the progress of a disease, can be figured directly in one graphical plot. Therefore, any urinary protein pattern of one or a number of gels can be exactly compared to each other in a short time.
