[Palliative surgical treatment of bone metastases. Improved quality of life by early intervention?].

OBJECTIVE: To find out to what extent early operative treatment of osteolysis or pathological fractures, resulting from improved interdisciplinary treatment of malignant tumours, affects quality of life. PATIENTS AND METHOD: Using a standardised scheme of self-evaluation in a retrospective study, quality of life was assessed in 67 patients (18 men, 49 women; mean age 64.5 +/- 11.6 years) who were operated for osteolyses or pathological limb fractures due to malignant tumour metastases. RESULTS: Interlocking osteosynthesis was performed in 37 patients, endoprosthetic joint replacement in 30. Duration of operation and hospital stay were significantly shorter in those 17 patients operated for osteolysis than those 50 patients with a pathological fracture (P < 0.01 and P < 0.07, respectively). There were seven hospital deaths. Postoperative survival time averaged 13.4 months. 41 of the 60 patients discharged to outpatient follow-up were again capable of leading a normal life, 13 were clearly impaired, six required nursing care, 46 patients did not or only sporadically require analgesics postoperatively. CONCLUSION: Early operative treatment of osteolysis before the occurrence of fractures is justified by the low operative mortality and complication rates and the better quality of life that is achieved.

Aged

[Late results of surgical treatment of Tossy III acromioclavicular joint separation with the Balser plate].

Various surgical procedures have been recommended for the treatment of complete acromioclavicular joint separations. The results have been similar in case reports by various authors. From 1984 to 1992, 35 patients were operated on for acromioclavicular joint separations (Tossy III) in the Department of Surgery, Marien-Hospital, Düsseldorf, using Balser's hook plate. The postoperative morbidity rate was 14.3%. Follow-up examinations were performed on 30 patients (85.7%) with average follow-up of 4.1 years, using a rating system that include subjective, objective, and roentgenographic criteria. Fifty percent of patients had no complaints, but residual dislocation was found in 36.7% of cases. While 26 patients (86.7%) were satisfied with the results of the operation, 30% demonstrated a fair result according to the scoring system. The complaints and clinical findings showed no correlation with the X-ray results, e.g. calcification or arthrosis, redislocations.

Acromioclavicular Joint

[Dislocation of the knee joint with lesion of the popliteal artery. A case report].

Complete dislocation of the knee is rare but frequently associated with popliteal artery disruption. Prompt recognition and early revascularisation are paramount for a successful and functional result. We report on a case of anterior dislocation of the knee complicated by concomitant popliteal artery injury. The value of pre-operative diagnostic methods including pre-operative arteriography and Doppler-sonography is discussed. Review of literature is presented.

Angiography, Digital Subtraction

[Follow-up care in colorectal adenoma].

From 1980 to 1985, 319 adenomatous colorectal polyps were removed in 297 patients. All patients were entered into a computerized follow-up programme with yearly follow-up. The recommended follow-up was rejected by 58 patients (22.1%). 93 new adenomatous polyps were found in the course of the follow-up in 53 patients (35.8%), five of them histologically invasive carcinomas. The proportion of carcinomas was lowered from 10.6% at first examination to 5.4% in the follow-up period. Size of polyp correlated significantly with carcinomatous change. Size of the adenomas removed at follow-up was markedly smaller than that removed at first examination (69.4% smaller than 1 cm compared with 44.2% at follow-up). In six patients with early invasive carcinoma, further resections were undertaken, while in six others only the polyps were removed. But local recurrences occurred in both groups. Regular follow-up after polypectomy of colorectal adenoma is thus essential.

[Endothelial function and arteriosclerosis].

Elevated levels of plasma LDL have been correlated with morphological endothelial damages, e.g. reduced antithrombogenity of endothelial cells. Numerical and functional defects of endothelial cells probably influence the necessary separation of blood fractions from subendothelial tissues in a negative sense. In order to describe an imaginable endothelial dysfunction with respect to LDL transport we examined human umbilical venous endothelial cells (HUVEC) in culture under physiological flow conditions. HUVEC in culture after incubation with a LDL concentration of 100 micrograms/ml medium were exposed to defined levels of physiological shear stress, fixed with formalin and stained with oil-red-O. After documentation of oil-red-O staining with a photomicroscope, the amount of stain has been characterized semiquantitatively with the help of an image-analysis system (IBAS II/Zeiss). HUVEC not exposed to shear stress only showed a marginal oil-red-O staining at the cell surface. If they were exposed to shear stress of 0.5 dyn/cm2 they showed in orthogonal view an average increase of 1.3%, in cross-sectional view an average increase of 0.3%. HUVEC exposed to shear stress of 2.5 dyn/cm2 showed in orthogonal view an average increase of 4.6%, in cross-sectional view an average increase of 8.7%.

Stress fibres (SF) in human endothelial cells (HEC) under shear stress.

Human umbilical veinous endothelial cells are cultured on artificial substrates precoated with extracellular matrix from bovine corneal endothelial cells. These cultural conditions help cells to keep an elevated maintenance level for otherwise unusual time intervals. Integrity of endothelial cell layers is of special interest for nondisturbed blood circulation. Therefore we look after mechanisms of cell--cell and cell--substrate--adherence. Here we focus on stress fibres, microfilaments, which seem to play an important part in cellular adherence phenomena. Since endothelial cells in vivo are constantly shear stressed by the blood stream we investigated cell--substrate interactions in vitro under dynamical conditions with the help of a modified cone--plate rheometer originally designed for hemorheology. It can be demonstrated in these shearing experiments that toxical substances influence the organization of stress fibres.

The single erythrocyte rigidometer (SER) as a reference for RBC deformability.

The SER allows the "deformability" of individual red blood cells to be quantitated by determining their passage time through a pore (d = 5.8 microns, 1 = 50 microns) under the shear stresses of 1.5 Pa-4 Pa. Using this system, we examined the influence of: 1. cytoplasmic viscosity, 2. membrane viscoelastic properties, 3. area to volume relationship. To change these determinants of RBC-deformability, the cells have been altered with 1. Acetylphenylhydrazine (0.016 mol/l), 2. diamide (0.5 mmol/l), 3. osmotic swelling (200 mosm/l) and osmotic shrinking (480 mosm/l) by suspending the cells in hypo- and hypertonic saline. The passage time has been found to be primarily influenced by changes in cytoplasmic viscosity. The same cells when tested in 4 other systems considered to measure RBC-deformability (filtrometer, packed cell viscometry, rheoscope and ektacytometry) behaved differently.

Blood Viscosity

Production of single pore membranes for the measurement of red blood cell deformability.

For the determination of the deformability of individual red blood cells, a measuring device was developed, simulating the passage of individual red blood cells in capillaries of the nutritive microcirculatiuon. The kernel of this device, described here, is a single pore membrane 4--6 microns in diameter and 10 to 50 microns in length. The combination of a heavy ion accelerator (containing a faster shutter system) with nuclear tract technology makes possibly the manufacture of a large number of single pore membranes necessary for clinical and pharmaceutical tests.
