[Pathology of the aorto-coronary bypass by autologous saphenous vein (author's transl)].

The success of aorto-coronary bypass operations, employing an autologous saphenous vein, depends mainly upon graft patency. Fortyone grafts have been examined by light micrsocopy, 35 of which were obtained from 23 autopsy cases and 6 were recovered at reoperation. Thirtythree veins had been in place for less than 20 days (early group) and 8 from 3 to 39 months (late group). In the early group, minimal to moderate mediointimal fibrosis was always detected. Graft occlusion due to recent thrombosis was observed in 13 cases; the cause of thrombosis was severe coronary narrowing distal to the insertion of the graft and atherosclerotic plaque ulceration at the site of the veno-coronary anastomosis. Among the late group, 3 veins were occluded by an old thrombus, and 3 by intimal leiomyocellular proliferation, with Alcian positive ground substance, while 2 exhibited a severe stenosis due to intimal phlebosclerosis. One of these last disclosed a vein atherosclerosis. The multiple pathogenetic factors, which are involved in the proliferative lesions, are reviewed.

Coronary Artery Bypass↗

[Atrial myxoma. Clinical, diagnostic, surgical and pathological features of 13 subjects (author's transl)].

The clinical, surgical and pathological features of 13 patients (3 males, 10 females) with cardiac myxoma are reported. Eleven masses were situated in the left atrium and 2 in the right one. The incidence of signs and symptoms usually ascribed to cardiac myxoma is evaluated. Particular emphasis is given to embolization phenomenon which occurred in 3 patients with villous-surfaced tumors. The results of phonocardiographic, echocardiographic and isotopic angioscintigraphic examination are illustrated and the use of these incruent investigations is highly recommanded. Final diagnosis may be achieved by cardiac catheterization and angiocardiography. Successful surgical excision was carried out in 12 patients. Structural profile of atrial myxoma is discussed. Particular attention is given to the surface of the tumors (smooth or villous), to the pseudovascular structures on the light microscopy and to the endotelial like features, together with secretory activity of the myxoma cells, on the electron microscopy. The morphologic findings support the neoplastic origin of the myxoma. The distinction between embolization and malignant metastasis is underlined.

[Removal of an endocavitary electrode with cardiopulmonary bypass for septicemia associated with right atrial thrombosis. Report of a case (author's transl)].

A case of staphylococcus epidermidis endocarditis associated with a right atrial thrombosis around a pacemaker electrode is presented. Stuce the antibiotic therapy had proved uneffective, the electrode had to be removed with cardiopulmonary bypass; the infection was subsequently eliminated. When a foreign body cannot be removed by closed techniques, open heart surgery with cardiopulmonary bypass may be necessary.

[Polygraphic indices of wearers of Hancock bioprostheses normally functioning in the aortic position].

A phonomecanocardiographic study in 33 patients who had undergone aortic valve replacement with a glutaraldehyde-preserved Hancock bioprosthesis, has been carried out 6 to 42 months after surgery. All xenografts were considered well functioning on the basis of clinical, electrocardiographic and radiographic evaluation. Typical phonocardiographic features of a well functioning porcine heterograft in aortic position were considered 1) an opening snap of variable intensity recorded in about 50% of the patients; 2) a normal closing sound; 3) a protosystolic ejection murmur which was always present and whose intensity was inversely related to the prosthesis size; and 4) the absence of a diastolic murmur. The anatomical and functional characteristics of the aortic valve of the pig are suggested to explain both the absence of a diastolic murmur and the pattern of the closing sound, which is undistinguishable from that of a normal aortic valve. Moreover, the reduced pliability of the right coronary cusp of the porcine valve, due to the muscular shelf at its base, with consequent limitation of the effective prosthetic orifice and relative stenosis of the device correlates well with both the opening snap and the sytolic ejection murmur.

Anomalous origin of the right coronary artery from the main pulmonary artery.

A case of anomalous origin of the right coronary artery from the main pulmonary artery, in a previously asymptomatic young woman, is presented. Direct transposition into the aortic root of the anomalous vessel was performed. To our knowledge this is the 4th case of this malformation to be successfully corrected.

[The coarctation of the aorta in the adults. Long term results over 10 years (author's transl)].

From May 1964 through December 1976, at the Department of Cardiovascular Surgery, University of Padova, Medical School, 148 patients underwent surgery for coarctation of the aorta; 62 of them where 18 years of age or older. The Authors deal particularly with the clinical symptoms, the operative findings, the surgical technique and the postoperative complications of their series. The early (only three operative deaths) and late results are considered satisfactory, and are discussed according particularly to the type of surgical intervention.

[The experience with permanent cardiac pacing at the Surgical Clinic of the University of Padua Medical School (author's transl)].

A computer analysis is reported of the most significant data concerning 1624 pacemaker implantations and 1386 battery replacements. The patients were treated by endocardial stimulation with transvenous electrodes in 91% of the cases; the remainder: 9% received epicardial electrodes. The mode of stimulation was 2189 times "demand", 138 "synchronized" and 683 "asyncronous". Pacers of 13 different brands were used. At the beginning of our experience, the catheter was introduced through the superficial or profunda giugular vein: since 1969 the cephalic vein has been preferred. Epicardial electrodes were implanted via a left thoracotomy but in the most recent years the extrapleural phrenico-pericardial approach has been adopted. In a number of cases external pacemakers have been employed for permanent stimulation. The most common post-operative complications were infection (3.9%), skin erosion (2.2%), catheter rupture (3.5%), electrode dislodgement (1.7%). Such complications are becoming less frequent since 1969, due to technical improvements.

[Subaortic stenosis due to antero-lateral muscular band, associated with ventricular septal defect and underdevelopment of the aortic arch. Anatomical features in three cases (author's transl)].

The pathological findings in three cases with ventricular septal defect and severe coarctation or tubular hypoplasia of the aortic arch are reported. The left ventricular outflow tract was partially obstructed by muscle situated on its antero-lateral wall. The haemodynamic relationship between this type of subaortic stenosis and aortic arch underdevelopment, due to unequal partitioning of the blood flow between the ascending aorta and the pulmonary artery during foetal life, is emphasized. The normal anatomy of the left ventricular infundibulum is described and some embryological hypotheses to explain this antero-lateral muscular band are forwarded.

Aortic Coarctation↗

[Rupture of the posterior wall of the left ventricule after replacement of the mitral valve: a description of 8 cases (author's transl)].

Eight cases of left ventricular rupture at the atrio-ventricular groove among 693 mitral valve replacements, isolated or associated with other surgical procedures, are reported. In the authors' opinion, the surgical technique of the mitral valve's removal, especially if calcific or with subvalvular disease, as well as the method and the site of implantation of the prosthesis, are the main factors in determining this severe complication. In order to avoid the rupture of the posterior wall of the left ventricle, a simple modification of the surgical technique which reinforces the valvular annulus is suggested.
